Guide 3 of 8Bomi is an insurance concierge for therapy practices.

Credentialing: payers, rates, and timing.

We apply to the insurance networks you choose, under your own practice, so the contracts belong to you. Your practice’s first four applications are included; after that it’s $50 per payer per therapist. Payers decide approval, effective dates, and rates; we confirm each effective date before you bill.

  1. 1

    Week 1

    Applications go out

Within about a week of your setup being complete.

  2. 2

    Around day 40

    We follow up

We confirm receipt early, then chase every open application.

  3. 3

    60-90 days

    Typical approval

Recent examples: Aetna about 30 days, Medicare about 35, BCBS about 45. Payers set the pace.

  4. 4

    Slower payers

    Plan ahead

Medicaid can take about six months. Some payers waitlist in some states.

  5. 5

    Then you bill

    Live

Direct deposit and ERAs set up, payer IDs loaded, first claims out.

Send requested documents promptly; that’s the one delay you control.

Who holds the contract

A payer signs one contract with one holder, and clinicians are listed on that holder’s roster. We work with individual and practice-held contracts; your attorney and CPA decide the entity question. Already contracted under your own practice? We work with your existing contracts after confirming effective dates and billing setup.

Platform or group

Contract holder

Headway, Alma, or a group

On the roster

  • You
  • Others

Their contract, under their tax ID. It doesn’t come with you when you leave.

Individual

Contract holder

You (SSN + Type 1 NPI)

On the roster

  • Just you

Your contract. Works with major payers; adding people later means re-credentialing.

Your practice

Contract holder

Your LLC, PLLC, or PC (EIN + Type 2 NPI)

On the roster

  • You
  • Future hires

Your contract, with room to add people. This is what we recommend for most practices.

Every application, in one place

Status per payer, what each one is waiting on, and a button to request more. The same list comes to you in the Monday email.

Rates

We don’t estimate rates before you join; your contract with each payer sets your rate. Once a year we go back to each of your payers and try to negotiate a better one.

Medicare and Medicaid

Medicaid can take up to six months, so it goes in first. A recent Medicare application took about 35 days; timing varies. Both have enrollment steps and sometimes fees, which we walk you through.
