Bomi, the insurance concierge for therapy practices
You care for your clients. We handle the insurance side: credentialing, keeping your credentials current, billing, and the insurance work of growing your practice. See what we handle, what it costs, and how to get started.
What Bomi is
It is your practice: your clients, your notes, your rates, your decisions. Bomi is the team that deals with insurance companies so you don’t have to.
01
We apply to insurance networks under your own NPI.
We apply to the payers you pick, follow up until they answer, and track every application in one place. The contracts belong to your practice, not to a platform.
02
We track the ongoing requirements and tell you when we need something.
Payers keep asking for things after you are in network. We handle CAQH attestations, re-credentialing, roster updates, and direct-deposit setup, and remind you before a license or malpractice policy expires.
03
Benefits before, claims after, follow-up on anything unpaid.
We verify benefits two weeks ahead, send claims the next morning, charge the copay, post payments, and chase anything denied or stuck. Insurers deposit directly into your bank account; Bomi invoices its fee separately.
04
The insurance side of adding clinicians, payers, and locations.
Add a clinician and we credential them under your practice, set up supervisory billing where payers allow it, and report by clinician. Add a payer or a location any time. We review your rates with payers once a year. We don’t send you clients; your practice stays yours.
Who we are
She wanted to see clients. The insurance side kept pulling her away. George went deep on billing and credentialing, got AAPC certified, and started running it for her. We helped her friends next, then practices across the country. Everything about how we work comes from what she needed: month-to-month, email answered in hours, and one clear update a week.

Days off feel like days off again
“Since going with Bomi, my days off are truly my days off. Now my work week looks exactly how I want it to look.”
“This is the first time owning my own practice. However, I have had some experience with navigating billing. I was actually SHOCKED to see how fast claims were getting paid out. From my previous experiences, other billing professionals told me weeks, maybe months. Bomi has been quick and effective.”

Brianna RossbachMS, LCPC
Rossbach Counseling, PLLC
“I have had nothing but a positive experience working with Bomi. They are quick and easy to get ahold of, are top notch at problem solving and make my life so much easier. I don't even think about billing. Bomi has it handled. What a relief!”

Stephenie Austin-BrusatteLCSW PLLC
“Bomi has impressed me with how quickly claims are filed and paid. Their weekly A/R sweeps help me stay current on my end. Bomi is like a trusted in-house billing team. The process of opening my own practice was daunting, but Bomi was an integral part of creating a seamless process on the billing side.”

A week with Bomi
The insurance side of a session, start to finish. You see the client, sign the note, and answer when we need something. We handle the rest.
Two weeks out
We verify every new client on your schedule: copay, deductible, network status. Automated check first, then a call when it is unclear.
Bomi
The session
You hold the session, keep the appointment status right, and sign your note.
You
Next morning
We send the claim and charge the copay to the card on file. Not the same day, so you have time to fix a code.

About 4 days
Median from claim to first payer payment, measured on Bomi-billed claims (Jun 2025 to Feb 2026). Direct deposit to your account; Bomi invoices its fee separately.

If stuck
Denials and rejections worked in 1-2 days. Stale claims chased at 14-30 days depending on the payer.

Operating targets from Bomi’s current workload. Payer timing still varies. We run on email; put URGENT in the subject to jump the queue.
An update from your billing team: where credentialing stands, what we are chasing and why, balances and credits, and anything we need from you, in plain language. Reply to it and the right person picks it up.
Groups: each clinician gets their own, with the owner copied. On Bomi EHR, the details between emails live on the Billing and Credentials pages.

Your EHR, or ours
We work as a billing user inside the EHRs below. For routine billing we use your schedule and insurance details, not your clinical notes.
Built around this way of working: your week, your notes, your client portal, telehealth, and the insurance side in the same screens. $0 per clinician for your first three months, then $29. Same team, same 4% either way.
Everything in the EHRSchedule in-person and telehealth appointments, set your weekly availability, and see insurance readiness and billing status right from each appointment.
Pricing
At the end of the month we look at what landed in your bank account from insurance, copays, and self-pay, and invoice 4% of that. Unpaid claims never generate a fee. No setup fee, no minimum, month-to-month. Example: $8,000 collected in a month is a $320 Bomi fee. If we help you see one more client a week, for most practices it pays for itself.
First three months
$0/ clinician / month
+ 4% of net collections for Bomi Billing
After the introductory period
$29/ clinician / month
+ 4% of net collections for Bomi Billing
Only the EHR subscription is waived. Bomi Billing fees apply throughout. Payment processing, payer, and other applicable third-party fees are separate.
Your practice’s first four payer applications are included (four per practice, not per clinician), then $50 per payer per clinician. CAQH upkeep, attestations, re-credentialing, and roster updates are included for as long as you bill with Bomi.
Separate from Bomi’s fee: card-processing fees on client card payments (charged by Stripe on your own account) and any fee a payer charges to enroll.
Credentialing
We credential your own entity and NPI, so the contracts belong to you. Here is what the timeline actually looks like.
Week 1
Within about a week of your setup being complete.
Around day 40
We confirm receipt early, then chase every open application.
60-90 days
Recent examples: Aetna about 30 days, Medicare about 35, BCBS about 45. Payers set the pace.
Slower payers
Medicaid can take about six months. Some payers waitlist in some states.
Then you bill
Direct deposit and ERAs set up, payer IDs loaded, first claims out.
A payer signs one contract with one holder, and clinicians are listed on that holder’s roster. Being on Headway’s or a group’s roster is not the same as being credentialed: the contract is theirs, under their tax ID. We credential your practice and roster you under it, so the contract is yours and there is room to grow.
Platform or group
Contract holder
Headway, Alma, or a group
On the roster
Their contract, under their tax ID. It doesn’t come with you when you leave.
Individual
Contract holder
You (SSN + Type 1 NPI)
On the roster
Your contract. Works with major payers; adding people later means re-credentialing.
Your practice
Contract holder
Your LLC, PLLC, or PC (EIN + Type 2 NPI)
On the roster
Your contract, with room to add people. This is what we recommend for most practices.
You can be credentialed individually, and it isn’t wrong. But a PLLC or PC with its own EIN and Type 2 NPI is the setup that causes the fewest problems with payers.
An individual NPI still linked to an old group can cross-link or slip its effective date. We check that your practice and clinician records are linked correctly.
You render care (Type 1 NPI). Your practice bills and gets paid (Type 2 NPI and EIN), not you under your SSN.
Add associates or employees under your existing Type 2 NPI instead of re-credentialing every payer later.
An entity separates you from the business. Your attorney and CPA can tell you what that means for you.
From George’s post Bill under a PLLC or PC, not your SSN. State rules differ (California uses a professional corporation); your attorney and CPA have the final word.
Send requested documents promptly, and review and sign payer agreements when they arrive; we flag anything waiting on you. Don’t assume earlier sessions will be covered: we confirm each payer’s effective date before you bill. Recent examples, not guarantees.
Stay on the platform while we credential you. Once a payer is effective, move one client as a test, then the rest. Payers pay you directly at your rate. The honest tradeoff: platforms send you clients, and we don’t.
Where you are
Work backward from your first insurance client. At every step there is a short list for you and a longer one for us.
6 months out
You
Form the entity if you want one, get the EIN, Type 1 and Type 2 NPIs, CAQH, malpractice.
Bomi
We tell you exactly what is missing and start Medicaid applications, the slowest ones.
3-4 months out
You
Finish setup and pick your payers.
Bomi
Commercial and Medicare applications submitted within about a week. We confirm each payer received it.
Around day 40
You
Sign payer contracts the day they arrive.
Bomi
We chase every open application and work any missing-information requests.
2-4 weeks out
You
Add your first clients and invite them to the portal.
Bomi
Direct deposit and ERAs set up with every payer, payer IDs loaded, benefits verified for your first clients.
Day one
You
Hold the session, sign the note.
Bomi
Claim out the next morning, copay charged, first Monday email. The 4% starts here.
Already credentialed? You start at step four: add us to your EHR, we set up direct deposit and ERAs, and claims go out the next morning. The EHR can wait until a few weeks before your first client.
The same team handles the insurance side of adding people, payers, and locations. Here is who does what.
Your first associate
You: Invite them from the Team page and set the supervisor.
Bomi: We credential them under your practice, one application per payer, and set up supervisory billing where the payer allows it.
A new payer
You: Click Add applications on the Credentials page.
Bomi: Same process as day one: submit, follow up, load the payer ID, set up direct deposit and ERA.
A second location
You: Add the location in settings.
Bomi: We update every payer roster so claims from the new address pay.
Every week, at any size
You: Each clinician gets their own Monday email; you are copied.
Bomi: Reporting by clinician, payer, and location. Balances and credits tracked per clinician.
Every year
You: Tell us if your rates feel low.
Bomi: We review your rates with each payer once a year. Some payers offer group rates as you add clinicians.
Joining
Nothing is charged at sign-up. Billing starts when you collect, and the EHR fee starts after your free months.
Your name, email, and practice name. One checkbox accepts the Terms, Privacy Policy, and our Business Associate Agreement, which sets out Bomi’s responsibilities for using and protecting client information.
We send a sign-in link. There is no password. The link opens your practice dashboard.
Five steps, each with an explanation beside it. Progress saves automatically, so do ten minutes now and the rest later.
This is what you see after your first sign-in. You enter it once; we use it for every payer application. Use the same legal name and address everywhere, because mismatches are the top reason payers send applications back.
Coming from SimplePractice? Tick the import box at sign-up. We bring clients, insurance details, appointments, documents, and billing history over while you keep working, and you review everything before switching. See how the move works.
One e-signature request by email: the Bomi Billing agreement and the EHR addendum.
Contact info, office and telehealth locations, and your team. Skip the team step if it is just you.
Services and rates, no-show and late-cancel fees, weekly hours, NPI, license, and your CAQH login.
EIN and tax classification, Type 2 NPI if you have one, the bank account for insurance payments, and client card payments.
Submit for review. We check it, set up direct deposit and ERAs, and start your payer applications while you keep using the EHR.
Same team, same 4%, inside your current EHR. The handoff is three steps and needs two documents sent by hand, so it starts with a short call instead of the setup checklist. Pick a start date, usually the first of a month; nothing is charged until we are added as a billing user.
Book a 15-minute handoffAfter a 15-minute handoff call, we email the BAA and the Services Agreement for e-signature. Same terms: 4% of collections, month-to-month.
As soon as you sign, George emails the intake: the same NPI, CAQH, tax, bank, license, and malpractice items as the setup checklist.
Add us as a billing user. We see the schedule and insurance details, not your notes. New practices do this once credentialing is done.
A session in Bomi EHR
Real screens from a demo practice with fictional clients. This is the workflow once your practice is ready to bill.
1 of 4
Add the client and click Invite to Client Portal. You pick what they see first: intake, consents, PHQ-9 or GAD-7, your own forms. They sign in with an email link, e-sign on any device, and upload their insurance card. We verify benefits before the first session.
2 of 4
Book from the calendar, office or telehealth. The appointment warns you if insurance or intake is not done yet. Afterwards, add the note from the appointment (SOAP, DAP, BIRP, EMDR, or free text) and sign it. The next morning we send the claim and charge the copay.
3 of 4
Insurance pays by direct deposit straight to your account; timing depends on the payer. Early next month you get one invoice from us for 4% of what actually landed. Pending claims are not counted. On the Billing page, "Needs your input" is the only number you have to watch.
4 of 4
Every clinician and payer as credentialed, in progress, or blocked. A blocked row says plainly what is missing and who acts next. Owners add new payer applications from the same page.
When you’re stuck
Routine questions are answered on the screen you are on. Anything else goes to the team by email or inside the EHR, and a person replies.
Click Help on any page and ask a question. Routine ones get a step-by-step walkthrough on the screen you are on. Anything else goes to Bomi support, and the reply shows up in the same thread.
Billing, credentialing, a payer, a client balance: email the team directly. Average reply about three hours, including evenings. Put URGENT in the subject to jump the queue.
If setup stalls or we are waiting on a document, you get a reminder with a link to that exact step.
Every week: what moved, what is stuck, what needs you. Reply to it and the right person picks it up.
Not a client yet and have a question this page didn’t answer? Email contact@bomihealth.com. A person replies, usually within a few hours.
The questions therapists ask most before they sign up.
Creating the account takes about two minutes; setup takes about twenty once your details are ready, and it saves as you go. Nothing is charged at sign-up. We review your setup within a business day and start your payer applications within about a week.
