Apple Health Provider Revalidation

By George Ruan • July 14, 2026

Last verified: July 14, 2026.

Revalidation is not just a checkbox. It is the moment when stale addresses, ownership, licenses, taxonomies, EFT, and group relationships can interrupt billing if they were ignored.

Short version: Revalidation is a full review of the ProviderOne record, and HCA says Medicaid providers must revalidate at least every five years under federal rules.

Sections

What HCA requires

HCA’s revalidation page cites federal Medicaid requirements for state agencies to revalidate Medicaid providers once every five years. Practices should treat revalidation as a provider-file audit, not a last-minute form.

Audit before the due date

  • Legal entity, DBA, W-9, ownership, and managing/control information.

  • Service locations and mailing/payment addresses.

  • Type 1 and Type 2 NPIs, taxonomies, licenses, and specialties.

  • Group affiliations and rendering clinicians.

  • EFT, ERA, user access, and backup System Administrator coverage.

Revalidation and MCOs

A clean ProviderOne revalidation does not automatically update every managed-care plan. After ProviderOne maintenance, verify CAQH/DataSpring, MCO rosters, directories, and contract records as needed.

Where Bomi Fits

Bomi helps therapy practices keep the operational layers aligned: HCA/ProviderOne enrollment, CAQH/DataSpring profile maintenance, MCO applications, portal access, eligibility checks, claim routing, denials, revalidation reminders, and first-paid-claim verification. The goal is not just an approval letter; it is billable access for the Apple Health members you actually see.

Operational note: This is general billing and credentialing education for Washington therapy practices, not legal, compliance, or payer-specific billing advice. Confirm current HCA, ProviderOne, CAQH/DataSpring, OneHealthPort, MCO, provider-manual, authorization, telehealth, and contract requirements before submitting enrollment, claims, or portal requests.

Frequently Asked Questions

When should I start ProviderOne revalidation?

HCA says no action is required until you receive the revalidation letter; maintain current mailing information and follow that notice.

Does every record use the same final BPW step?

No. The state instructions distinguish group/FAOI from individual solo records.

Should I change my working EDI setup during revalidation?

Only when applicable. HCA’s guide warns that unnecessary changes can delay payments.

Follow the revalidation letter for the listed NPI

HCA says no action is required until the revalidation letter arrives. Billing providers have 30 days from its date before a deactivation notice, followed by an additional 90-day completion period. Follow the actual notice’s deadline and checklist; keep the ProviderOne mailing address current. HCA also links an off-cycle initiative for certain providers, so do not rely solely on a five-year anniversary.

Review the Business Process Wizard

The November 2025 instructions select EXT Provider File Maintenance or EXT Provider Super User, then Manage Provider Information. Review the Business Process Wizard for the actual enrollment type. Its final submission step differs between group/FAOI and individual solo records; do not assume identical step numbers.

When reviewing service locations, choose the Location address type, then Address on Manage Provider Location Address. HCA’s guide walks through validating an updated address and saving the change; review the current information even if no change is needed.

Update disclosures and submit required documents

HCA requires updated ownership/managing-control disclosures in Step 4, a signed Core Provider Agreement and Debarment Statement, a W-9, and provider-type-specific documents. Use the checklist for the NPI on the letter. Our recommendation is to retain the submitted documents and confirmation together.

For the individual solo route, the final screen includes Submit Provider Modification and Upload Attachments. Follow the instructions for the enrollment type on your notice and retain submission confirmation.

Avoid unintended EDI routing changes

The revalidation instructions say to update EDI submission, software, submitter, and contact steps only if applicable, warning that changing a working electronic-claims setup can delay payments. Our suggested review assigns a billing owner to confirm any intended routing change before final submission.

Use the correct route for nonbilling providers

HCA’s page says nonbilling providers submit through the HCA support portal or fax. The same page also contains a conflicting nonbilling instruction to log into ProviderOne. Confirm the route with HCA before acting if your access differs; do not assume every nonbilling provider has portal access.
