MassHealth Enrollment for LMHCs, LICSWs, LMFTs and Psychologists

The Medicare prerequisite changed in October 2025

Effective October 1, 2025, MassHealth removed Medicare-provider status as an enrollment prerequisite for both LIBHCs and psychologists. Older regulation PDFs still contain that language; LIBHC Bulletin 1 and Psychologist Bulletin 8 clarify the change. Prepare for the January 2027 claims requirement

All Provider Bulletin 421 announces enforcement for dates of service on or after January 1, 2027 when state or federal rules require a provider appearing on a MassHealth claim to be enrolled. Read the bulletin’s role-specific rules and limited ordered/referred/prescribed-service timing exception. Existing billing and rendering enrollment requirements already apply. Choose the right provider type and participation

For Massachusetts independent clinicians, 130 CMR 462.404 identifies LICSW, LMHC and LMFT licensure. An LCSW license should not be treated as an LICSW license. Psychologists should follow 130 CMR 411.000 and the Psychologist Manual. Use the October 2025 bulletins when reviewing the older Medicare wording. FFS enrollment, ORPASO-only enrollment and MCE-only participation have different purposes. ORPASO-only and MCE-only providers cannot submit claims for direct MassHealth FFS payment. How to apply and track approval

  1. Review your manual before requesting the application. Use the LIBHC or Psychologist Manual and current bulletins for your license and services. These guides include administrative requirements and service-code sections. LIBHC Manual Psychologist Manual

  2. Request the correct application through Self-Service. The official application page links its Provider Application Request form. MassHealth sends completion instructions to the email supplied. Follow that package rather than a generic Medicaid checklist. Apply to become a MassHealth provider

  1. Track status and effective date. MassHealth’s FAQ says Self-Service can search application progress by ATN or NPI and enrollment status by provider identifiers.   1. Confirm group affiliations and plan contracting. Individual practitioners working in a group must enroll FFS and link to the group, according to the state FAQ. Before scheduling under the new billing arrangement, compare the individual and group enrollment records, service locations and written contract effective dates. Ask the payer how each provider will appear on claims. Keep unresolved enrollment or affiliation items in a work list and confirm the member’s coverage before deciding where to bill.

Frequently Asked Questions

Can LMHCs and LMFTs apply under MassHealth LIBHC rules?

Yes. The LIBHC regulations include Massachusetts LMHCs and LMFTs alongside LICSWs, subject to the other participation requirements.

Must I enroll in Medicare before applying to MassHealth?

LIBHC Bulletin 1 and Psychologist Bulletin 8 removed that prerequisite effective October 1, 2025. Medicare and other-insurance billing requirements still apply when relevant to the member.

Does an ORPASO-only enrollment let me bill MassHealth directly?

No. The October 2026 bulletin distinguishes nonbilling ORPASO-only and MCE-only enrollment from FFS payment participation.
