PECOS Login: Provider Account Setup and Biller Access

TL;DR: use your own login, and give your biller their own

Use your own I&A account to open PECOS. Your biller uses their own account with approved Staff End User access, or an Access Manager role when they need to manage staff and connections. Creating a login is separate from enrolling a clinician or practice in Medicare. CMS enrollment guidance

Who needs their own PECOS login

Every person uses their own name, account and multi-factor authentication (MFA). Never share your password. An Authorized Official (AO) or Access Manager (AM) grants Staff End User (SEU) access for the correct employer and the PECOS business function; only an AO can invite an AM. One person can have approved access to multiple employers, with permissions set for each. I&A guide, pages 4–5 and 66–69

Your own I&A login opens approved PECOS records; your biller has a separate Staff End User login approved by an AO or AM.

Before you create access

Have the provider’s NPI and correct Medicare enrollment organization ready. PECOS manages enrollment; it is not the MAC claims or eligibility portal. A biller’s access to prepare enrollment information does not itself give authority to sign the application. CMS PECOS and enrollment guidance

How do I create a PECOS account?

  1. Open PECOS and choose Register for a user account, which routes account creation through I&A. If you already have an I&A account, use it instead of creating another.

  2. In I&A, choose Create Account Now. Enter your own unique email address, complete the registration prompts, and set up your own MFA.

  3. Use My Profile to request the appropriate employer relationship. An AO or AM must approve staff access; selecting an employer is not approval.

  4. Return to PECOS with your I&A credentials and check that the intended provider or organization is available.

Add your biller to PECOS

For staff access, the AO or AM opens I&A → My Staff → Add Staff. Enter the biller’s own name and email, select the employer, choose Staff End User, and select PECOS. An AO may instead assign Access Manager when the biller needs those broader duties. The biller accepts the invitation using their own account; check that the employer and PECOS access are active. A separate billing company can use an approved surrogate connection between organizations. Its AO or AM requests the connection and the provider approves the requested PECOS function; the company must also authorize its own staff. PECOS login errors and missing organizations

Use the official Forgot Password or Forgot User ID links for credential problems. If sign-in works but a practice is missing, check the employer relationship, approval status and PECOS business function in I&A before creating another account. Frequently Asked Questions

How do I log in to PECOS?

Open pecos.cms.hhs.gov/pecos/login.do and use your own I&A user ID and password. Your employer or provider access must also be approved.

Is my PECOS login the same as I&A?

PECOS uses I&A credentials. An account alone does not grant access to every provider record or replace Medicare enrollment.

How do I give my biller access to PECOS?

Your AO or AM can invite the biller as a Staff End User and grant PECOS access for the correct employer. A separate billing company may need a surrogate connection and approval of its staff.

Why is my practice missing in PECOS?

Check the I&A employer relationship, its approval status and the PECOS business function. Use the missing-organization guide or CMS support if those do not resolve it.
