Georgia Medicaid Provider Enrollment for Therapists

Start Georgia Medicaid provider enrollment at the official GAMMIS Enrollment Wizard. For an LPC, LCSW, LMFT or psychologist, first match the application to your license, the services you will provide and the billing arrangement. A portal account gives you access; it does not establish an approved Medicaid enrollment or a CMO contract.

Open the GAMMIS Enrollment Wizard

Choose Provider Enrollment → Enrollment Wizard on GAMMIS. The current public training catalogue links the February 2021 initial-application guide. Its menu example points to the Wizard. This is an enrollment application, separate from creating a portal login.

Which Georgia Medicaid provider type should a therapist choose?

The October 2026 Psychological and Therapy Services manual includes licensed psychologists, clinical social workers, marriage and family therapists and professional counselors in section 601. Its private-practice licensing requirements apply to fully licensed practitioners. The program described in sections 701 and 902 serves members under 21; enrollment in this category does not establish coverage for every adult therapy service.

GAMMIS calls the program Category of Service (COS) 570. Its 2022 COS 570 enrollment presentation describes the expansion of LPC, LCSW and LMFT enrollment from the older COS 998 CMO-only route. It lists these specialties:

  • 041 — Professional Counselor (LPC).

  • 107 — Clinical Social Worker (LCSW).

  • 110 — Marriage and Family Therapist (LMFT).

Those numbers come from the dated presentation. Confirm the current specialty choices in the Wizard before submitting. For a psychologist, match the current psychologist option and license requirements; ask enrollment support through GAMMIS if the choices do not fit. Do not select a different profession just to move through the application.

The initial-application guide shows the application-type and Provider Type controls. It warns that dropdowns and panels change with earlier selections. Use the current Wizard options for your actual license and application; this example does not establish a therapist specialty code.

The public enrollment coversheets also label COS 820 as Medicare-only clinical social worker enrollment. That is a different purpose from enrollment to provide Medicaid therapy. Prepare the practitioner and service-location information

Under Part 1, sections 105.1–105.2, enrollment records and approval are tied to the practitioner and service location. Assemble the license, NPI confirmation, tax/payee information and the documents requested for the application you actually selected. Use the current enrollment-document catalogue as a starting point, then follow the Wizard’s requests. Its older COS 570 coversheet is psychologist-specific, so it is not a complete checklist for every counselor or social worker.

For the upload step, the initial-application guide, page 37 shows Choose File and upload attachment. Attach the materials required by your selected application; a sample physician document list is not a universal therapist checklist.

If credential verification is required, the CVO checklist lists supporting materials such as professional history and malpractice coverage. Conditional items apply only when relevant. Resolve gaps or missing documents rather than assuming a saved application has been approved.

A group billing NPI needs its own enrollment and affiliations

The March 24, 2026 Group/Billing Enrollment FAQ requires applicable professional billing groups to enroll when the billing NPI differs from the rendering NPI. It describes service-location-specific group records and affiliations between the billing group and rendering practitioners. A solo clinician’s approval does not establish all of those group records.

The FAQ says the rendering provider’s specialty controls claim adjudication; the group’s billing specialty is informational. Review the billing NPI, payee, location and each rendering affiliation before sending claims. There are setting-specific exceptions, including FQHC and DBHDD arrangements, so follow the category that applies to the practice.

Medicaid enrollment and CMO contracts are separate steps

Georgia’s centralized CVO shares credentialing decisions with the CMOs. DCH directs contract questions to the plans themselves. After state enrollment, confirm the plan contract, practitioner roster, location and participation effective date before scheduling on the assumption of in-network payment. Keep the state approval notice and check its effective date. Part 1 says services before the approved enrollment effective date are not reimbursable. For ongoing operations, use the GAMMIS eligibility guide and keep each enrolled location current through GAMMIS revalidation.

Frequently Asked Questions

Can an LPC, LCSW or LMFT enroll in Georgia Medicaid through GAMMIS?

Yes. The October 2026 Psychological and Therapy Services manual includes these fully licensed professions. Match the current application and specialty to the services you intend to provide. The COS 570 program described in that manual is for members under 21; other programs and CMO coverage require their own checks.

What are Georgia Medicaid specialty codes 041, 107 and 110?

The official 2022 COS 570 presentation lists 041 for Professional Counselor, 107 for Clinical Social Worker and 110 for Marriage and Family Therapist. Confirm the current Wizard choices before submission; these are not a substitute for checking the present application.

Does my Georgia Medicaid enrollment cover my group billing NPI?

Not automatically. The March 2026 Group/Billing Enrollment FAQ requires applicable billing groups to enroll and link rendering practitioners. Review the group record for the relevant service location as well as each individual enrollment.

Does a GAMMIS approval mean I am contracted with every Georgia Families CMO?

No. State enrollment, centralized credential verification and CMO contracting are separate. Confirm the contract, roster, location and effective date with each plan you intend to bill.
