MO HealthNet Enrollment for Therapists: MMAC to MHD

Effective September 16, 2026, provider enrollment responsibility transferred from Missouri Medicaid Audit and Compliance (MMAC) to MO HealthNet Division (MHD). MHD says existing procedures continue and pending applications keep being processed. Its transition notice still directs questions to MMAC.ProviderEnrollment@dss.mo.gov. Can LPCs, LCSWs, LMFTs and psychologists enroll?
The current Behavioral Health Services Manual, section 1.2, lists licensed professional counselors, clinical social workers, marital and family therapists and psychologists as eligible providers. It requires a valid license and an accepted participation agreement. The manual permits these disciplines to provide behavioral health services to eligible children and adults within their scope. The application’s program requirements group these behavioral health disciplines under provider type 49. That attachment page still contains an under-21-only statement for counselors and social workers; it conflicts with the current manual’s child-and-adult language. Use the current manual for coverage and ask enrollment staff to clarify a conflicting application instruction before submitting.
Prepare the documents for your actual provider type
Use the official program requirements to identify required attachments. For an EIN, the application requires an IRS-preprinted document showing the legal name and tax ID; it specifically says a W-9 is insufficient. Counselors and social workers submit their license; psychologists submit their permanent license and Medicare number if enrolled in Medicare. The LMFT entry requires the applicable current license.
Keep the individual clinician’s license, NPI, practice locations and payee information consistent. Submit only the documents required for your provider type; a clinic’s application is not a substitute for individual enrollment.
The application instructions exempt individual nonphysician providers from the enrollment application fee. Do not extend that exemption to a group or clinic: organizational enrollment can have different requirements. Find the applicable forms on the official enrollment forms page.

Choose the provider type for your application
Start New Provider Enrollment and select the provider type required for your clinician. The current public enrollment guide uses physician type 20 as an example in this dropdown; therapists must follow their own type 49 requirements, not copy the example selection. Complete the provider and contact fields
Enter the provider and contact information requested by the application and use its field-level help. This blank form from the public guide illustrates the name, mailing address, contact and security-question fields. Official enrollment guide, Online Provider Enrollment Instructions.

Finish the application and send the required package
Complete the online application. Use the field-level Help instructions and keep the retrieval details if you save a partial application.
Finalize the application and retain its pages for your records.
Send the provider agreement signature page and all required attachments together in one fax transmission to 573-634-3105. Keep the signature page and attachments on separate pages.
Check that the transmission completed. Respond to requests for missing information using the enrollment unit’s instructions.
The instructions accept the listed electronic-signature methods as well as an original wet signature. They warn against combining pages into a reduced-size page or sending the signature and attachments separately. Verify the current instructions before transmitting your package.
Individual clinician and group enrollment
For group or clinic services, the current manual calls for the group/clinic NPI on the claim and the individual clinician NPI as the performing provider. Confirm both enrollment records and the correct practice location before billing. MHD current Behavioral Health Services Provider Manual landing page.
Provisional practitioners have additional limits. The manual says PLPCs, PLMFTs and provisional psychologists cannot operate as independent practitioners or receive direct MHD payment. A provisional license should not be treated as interchangeable with an independently licensed clinician’s enrollment. MHD current Behavioral Health Services Provider Manual landing page.
Enrollment approval is only one part of readiness
Next set up eMOMED and biller access and confirm each managed care network contract. The state’s managed care participation bulletin says state enrollment does not automatically make a provider in network with a managed care plan. Verify member eligibility and service authorization before treating enrollment approval as permission to bill a particular session.

Frequently Asked Questions
Is Missouri Medicaid enrollment still called MMAC enrollment?
Many public pages and forms retain that label. Enrollment responsibility transferred to MHD effective September 16, 2026; the state says to continue existing requirements and procedures during the transition.
Can Missouri LPCs and LCSWs serve adults through MO HealthNet?
The current Behavioral Health Services Manual says LPCs and LCSWs may provide behavioral health services for children and adults, subject to eligibility, coverage and scope. Older enrollment-page text still says under 21 only; ask the enrollment unit about conflicting instructions.
Does my group enrollment cover every therapist?
Do not assume it does. Individual enrollment and the group/clinic billing record must support the claim; the manual requires the clinician’s performing NPI for group services.
Does MO HealthNet enrollment enroll me in Healthy Blue or another plan?
No. State enrollment and plan-specific credentialing and contracting are separate. Check the exact plan and product before scheduling in-network services.
