MO HealthNet Managed Care vs FFS Credentialing

Missouri Medicaid enrollment and managed care credentialing answer different questions. The official MO HealthNet managed care health-plan list is the place to confirm the current plan names. As of October 9, 2026, it lists Healthy Blue, Home State Health, United Healthcare and Show Me Healthy Kids.
State enrollment establishes participation in fee-for-service (FFS); it does not automatically include a provider in a managed care network. MHD’s August 25, 2026 bulletin requires plan-specific credentialing and a participation contract for each managed care network you intend to join. MHD managed care participation bulletin 49-12.

Three general plans and a separate specialty plan
Healthy Blue, Home State Health and UnitedHealthcare are the general managed care options. Show Me Healthy Kids (SMHK) is the specialty plan administered by Home State Health for eligible foster care, adoption or guardianship subsidy, and other specified children and youth groups. It is not a fourth general plan that every member can choose. See the official plan list and state SMHK explanation.
Treat the member’s assigned plan and the service being billed as the routing decision. The general Home State product and its SMHK specialty product should be confirmed explicitly with the network team.
FFS enrollment and managed care contracts
For state enrollment, use the therapist enrollment guide. Responsibility moved from MMAC to MHD on September 16, 2026, while existing procedures continue. MHD transition notice.
For managed care, the participation bulletin separates professional credentialing from the contract that establishes rates and administrative requirements. A provider who has not completed both remains out of network. An eMOMED user account does not complete either step.
Verify your active state enrollment and the clinician/group records that will bill.
Identify the member product and behavioral health network you intend to serve.
Apply through the plan’s appropriate behavioral health credentialing and contract route.
Confirm the effective participation date and whether your clinician, group and location are recognized before representing yourself as in network.
Healthy Blue behavioral health routing
The Healthy Blue joining page has a specific behavioral health section: new behavioral health practitioners or groups that are not multispecialty, and existing groups notified of contract assignment, are directed to Carelon Behavioral Health. Do not assume its generic medical Availity enrollment steps apply to every therapy practice. Start with the behavioral health instructions and confirm which route fits your group.
Home State Health and Show Me Healthy Kids
Home State Health provides a join-network page with a new-contract request route and separate follow-up for adding a practitioner to an existing contract. For specialty participation, use its SMHK provider resources and ask the network team to confirm the specific SMHK product, clinician and location. Do not infer specialty participation solely from a general Home State contract.
UnitedHealthcare Community Plan behavioral health
The Missouri UnitedHealthcare Community Plan page directs behavioral health providers to Optum Provider Express network joining. Its medical network process is listed separately. Follow the behavioral health route appropriate to the therapy services you provide.
Check service carveouts before choosing the payer
SMHK coordinates medical and behavioral health care, but the state identifies pharmacy, Applied Behavior Analysis and Department of Mental Health services as services obtained through MO HealthNet outside SMHK coverage. A plan assignment alone does not tell you the payer for every behavioral health service. MHD Show Me Healthy Kids.
Keep a separate record for state enrollment, plan credentialing, executed contract and effective date. Before each session, use the member’s current eligibility and the service-specific rules to select the claim destination.

Frequently Asked Questions
What are the current MO HealthNet managed care plans?
As of October 9, 2026, the official list includes Healthy Blue, Home State Health, UnitedHealthcare and Show Me Healthy Kids. The first three are general plans; SMHK is the Home State Health administered specialty plan for specified eligible groups.
Does state enrollment make me in network with every Missouri Medicaid plan?
No. The August 2026 MHD bulletin requires plan-specific credentialing and an executed participation agreement. State enrollment is separate.
Is Show Me Healthy Kids the same as general Home State Health?
SMHK is a distinct specialty product administered by Home State Health for specified eligible children and youth. Confirm specialty network participation and service routing rather than assuming the general product contract applies.
Do all behavioral health claims for SMHK go to the plan?
No. Missouri lists ABA and Department of Mental Health services among services obtained through MO HealthNet outside SMHK coverage. Confirm the payer for the actual service before filing.
