Meridian Medicaid Michigan: MHPMI

Meridian’s billing page lists MHPMI for Meridian Medicaid Michigan dates of service on or after April 1, 2022. Its electronic claims section covers professional and facility claims and says payer IDs are not interchangeable. Follow its member-card instruction when checking the product rather than importing an ID from another Meridian line of business.

McLaren Medicaid and Healthy Michigan: 3833C

McLaren’s billing page lists 3833C for Medicaid/Healthy Michigan in its Optum EDI table. The same table lists 38338 for commercial HMO claims. Keep those products distinct when selecting the destination in your clearinghouse. Our recommendation is to retain the product label beside the ID in the payer setup record.

HAP CareSource Michigan Medicaid: MIMCDCS1

HAP CareSource’s Michigan Medicaid provider contact page lists MIMCDCS1 for EDI and directs providers to their clearinghouse for setup. Confirm that your clearinghouse maps its payer selection to this Medicaid destination before using it for a claim.

Upper Peninsula Health Plan: 38337

UPHP’s 2026 provider manual identifies National Electronic Insurance Code 38337 in its electronic claims instructions. Our recommendation is to verify the member’s exact UPHP product and intended transaction with the plan or clearinghouse before using a saved payer entry. A familiar plan name alone is insufficient routing evidence.

UnitedHealthcare: resolve conflicting manual entries

The 2026 Michigan Community Plan manual lists 95467 in its key-contacts claims table, while its electronic-claims chapter also states 87726. Because those entries differ, this guide does not choose a universal UHC destination. Ask the plan and clearinghouse to confirm the exact Michigan product and claim transaction before changing an existing route.

MDHHS publishes separate 837 professional, institutional, dental, and 835 companion guides. Use the appropriate state transaction instructions and your clearinghouse’s confirmed mapping.
