Maryland Medicaid Enrollment for Therapists

Start with the Maryland Medicaid provider type

Maryland’s transition attachments identify Certified Professional Counselor as PT CC, Social Worker with an LCSW-C license as PT 94, Psychologist as PT 15, and Mental Health Group Therapy Providers as PT 27. Outpatient Mental Health Clinic is PT MC. These categories are distinct; do not choose a facility or group type solely because your practice offers therapy. Read the official provider-type attachments.

Recommended first step: compare the clinician’s license and the organization’s actual service model with the state’s requirements for the proposed provider type. Resolve ambiguity with Maryland before building an application around an assumed category.

Prepare individual, billing and group records separately

Recommended application worksheet: keep one row per enrollment record rather than one row labeled “Medicaid credentialing.” This makes it easier to see whether the remaining task belongs to the individual clinician, the billing entity or a group.

  • Identify the legal applicant, NPI, applicable provider type and whether the record is new or already enrolled.

  • Collect current licenses and other documents requested for that provider type. Use the current state checklist to determine the final packet.

  • Confirm practice, correspondence and payment information with the authorized owner.

  • List the individual and group records that need to work together for the intended services. Ask the state how to establish the required relationship in the current portal.

  • Retain submission and correction evidence, then verify the approved effective date rather than treating a login or application number as approval.

How does the MPRIME hold affect a new application?

PT 94-26 describes conditional backdating for newly enrolling providers who submit a complete, correct and processable MPRIME application on or before December 31, 2026. It also says backdates are not assured and providers cannot bill or be paid before enrollment approval. Do not build a cash-flow plan around an assumed retroactive approval. Medicaid enrollment and Carelon registration are separate

Maryland says specialty behavioral health providers must enroll with Medicaid and register with Carelon. Primary behavioral health services through managed care follow the member’s managed care organization (MCO) route. Identify the service before deciding which participation workflow applies. Recommended completion check: track the state’s enrollment decision and the applicable behavioral health registration as separate tasks. For the first intended service date, confirm the actual enrollment status, eligibility and any authorization requirements before treating the clinician as ready to bill.

Courtesy authorization during the transition has limits

The August 21 MDH memo creates a narrow temporary Carelon NPI-registration process for eligible unenrolled billing providers affected by the application hold. It excludes moratorium-affected providers and says rendering-only providers or existing groups enrolling rendering providers do not need that temporary process. Courtesy authorization does not guarantee payment; claims must be held until enrollment requirements are met. Read the temporary registration memo.

If this exception may apply, use the memo and the state-linked form for the exact provider situation. Do not read the temporary process as a general invitation to provide services before enrollment.

Does a Maryland Medicaid login mean I am enrolled?

No. Keep account access, application submission, enrollment approval and the approved effective date as separate checkpoints.

Do Maryland therapists also register with Carelon?

Maryland’s specialty behavioral health guidance requires Medicaid enrollment and Carelon registration. Confirm that the intended service belongs in that specialty route.

Is MPRIME enrollment backdating guaranteed?

No. PT 94-26 sets conditions for eligible newly enrolling providers and explicitly says backdates are not assured. Review the state decision and effective date.
