IMPACT PE: individual, rendering-only or group?

HFS publishes separate application guides for individual sole proprietors, rendering/servicing providers and groups. Match the application to how the provider will participate and receive payment. Choose an HFS enrollment guide

  1. For a clinician enrolling as an individual sole proprietor, follow that individual enrollment guide.

  2. For a clinician providing services through a billing provider, review the rendering/servicing guide and the billing-provider association.

  3. For the practice enrolling as a group, follow the group guide; review its locations, taxonomy, ownership and claims-submission sections.

Use the individual guide to review a solo application and its optional billing-provider association. Do not select a group application simply because another company handles your claims. Individual sole proprietor instructions

Prepare the NPI, taxonomy, license and W-9

HFS lists NPI and taxonomy preparation, active professional credentials and appropriate portal access among enrollment prerequisites. Providers paid directly by the Illinois Comptroller need a certified W-9; HFS expressly exempts rendering/servicing-only providers. HFS enrollment prerequisites

Where required, HFS directs applicants to send the completed W-9 and IRS Assignment Letter to HFS.W9.IMPACT@Illinois.gov with the subject W9 Certification. Follow that current page if an older slide deck lists a different mailbox. Current W-9 submission instructions

Associate the therapist with the correct billing provider

In the rendering/servicing application, the association step asks for the employer billing provider’s NPI. Confirm the returned provider name before saving. Use the employer NPI in that association, rather than substituting the rendering clinician’s NPI. Rendering-provider association guide

For a group, work through the group application’s required sections and record the application ID so you can return to the same application. The guide distinguishes continuing a saved application from starting a new enrollment. Group application and tracking guide

Finish the enrollment before treating it as approval

  1. Keep an application checklist for the individual, group and billing-provider relationship that applies to your practice.

  2. Review the saved application and any state requests before submitting; record its identifier and status.

  3. Keep the state approval and effective details together with the plan participation records you will use for billing.

These are practical recordkeeping recommendations, not a promised HFS processing time. If the enrollment type or tax certification is unclear, use the help contact published in the training library: 1-877-782-5565 or IMPACT.Help@illinois.gov. IMPACT enrollment help

IMPACT approval and MCO participation are separate

HFS directs new providers to credential through IMPACT and contract with each managed care plan individually. State enrollment alone does not establish a contract with every HealthChoice Illinois plan. HealthChoice provider participation guidance

Frequently Asked Questions

Does a rendering-only therapist need a W-9 for IMPACT?

HFS exempts rendering/servicing-only enrollments from its certified W-9 requirement. The requirement applies to the payment-receiving enrollment types listed in its Preparing to Enroll guidance.

Does my group enrollment replace my clinician association?

Use the rendering/servicing guide to associate the clinician with the correct employer billing provider. Review the individual and group records separately.

Does IMPACT enrollment put me in every Illinois Medicaid MCO network?

No. HFS distinguishes state credentialing through IMPACT from contracting with each managed care plan.
