CAQH Rejected Your Malpractice Certificate? How to Fix It

CAQH rejects a malpractice document when the provider name, policy number, or expiration date on it doesn’t match the Professional Liability Insurance entry in your profile, or when the file itself is unreadable, expired, or combined with other documents. Read the rejection reason, decide whether the profile entry or the document is wrong, correct it, and upload again to the same slot.

The current Provider User Guide states the matching rule directly: the provider name, current expiration date, and policy number in the profile must match the face sheet, or the document will be rejected. What CAQH Checks on a Malpractice Upload

  • Policy number: Enter it exactly as the face sheet prints it, including letters, leading zeros, and allowed punctuation. The portal only accepts the special characters . - / & ( ) and #, so a number with other symbols needs a careful check against the document.

  • Expiration date: Make sure the profile shows the same period as the certificate you are uploading, not last year’s policy.

  • Provider name: The insured or covered individual on the document has to be you. If the certificate is for a group policy and lists only the practice, the carrier has to issue evidence that names you; re-uploading the same certificate will not clear it.

  • The file itself: CAQH’s document guidance also lists blurry or illegible scans, expired documents, and several document types merged into one PDF as rejection reasons. Upload the liability evidence on its own in the liability slot.

Fix the Profile or Get a New Document?

Compare the rejected document with the profile entry field by field. If the document is correct and the profile was typed wrong, edit the profile and upload the same document again. If the profile is right and the document is wrong, out of date, or names the wrong insured, ask the carrier or broker for a corrected certificate or declarations page. Don’t change the profile to match a document that is itself wrong.

Renewed Policy? Use Renew, Not Edit

For a policy that has expired or is about to, the user guide requires the Renew button on the policy rather than Edit when a document is already on file. Enter the new dates and upload the renewed face sheet or certificate in the same step. Replacing only the PDF leaves the old dates in the profile, which recreates the mismatch.

  1. Open Professional Liability Insurance and select Renew on the current or expired policy.

  2. Enter the policy number and dates exactly as printed on the renewed certificate.

  3. Upload the certificate to the liability slot on the Documents page, then review and attest the profile.

  4. Allow CAQH’s review time, then check the Documents page for the result before telling a payer the issue is fixed.

Renew Early If You Are Mid-Credentialing

An approved certificate can still stop an application. Some payers check how much time is left on the policy in CAQH at submission. With Evernorth, we have twice held a clinician’s submission because the policy on CAQH had less than the 60 days of remaining coverage it requires, and asked for the renewal certificate before filing. When a renewal is close, update CAQH with the renewed certificate first.

Our habit: before asking a clinician for new malpractice proof, we check what is already on the CAQH Documents page and whether it was accepted. In one recent case the current certificate was already there and no request was needed.

Frequently Asked Questions

Why did CAQH decline my malpractice insurance?

Usually because the certificate and the profile disagree on the insured’s name, the policy number, or the expiration date, or because the scan is unreadable or expired. Start with those fields on the rejected document.

Can I just upload the new PDF over the old one?

Not for a renewal. Use Renew on the policy so the new dates are saved with the new document; swapping only the file leaves the old dates behind and triggers another mismatch.

How long does CAQH take to review a new certificate?

CAQH’s own guidance says to allow two to three business days, and it notifies you by email if the document is rejected.

Does an approved certificate mean my credentialing is done?

No. It clears one item in CAQH. Each payer still runs its own review and sets its own effective date.
