Texas Medicaid Enrollment for Therapists: PEMS

Texas Medicaid therapist enrollment starts with TMHP’s Provider Enrollment resources.
As of October 9, 2026, use the Provider Enrollment and Management System, or PEMS, for enrollment and provider-file work. TMHP says PEMS replaced PEP and PIMS; a portal account by itself is not enrollment approval. Read the PEMS enrollment overview.
Sections
LPC, LCSW, LMFT and psychologist prerequisites
The current handbook requires the applicable license and Medicare enrollment or its pediatric-practice exemption for LCSWs, LMFTs, LPCs and psychologists. Joining a Medicare-enrolled group requires Medicare enrollment even for a pediatric clinician. Review the current section for your discipline. Check sections 4.1.4 through 4.1.7.
Recommended first step: resolve the applicable license and Medicare/exemption evidence before completing the application. Keep the individual clinician and any billing organization distinct in your worksheet.
Create access before starting the PEMS application
For a new applicant, open Account Activation, select Create a Provider Enrollment Account and then Next. An existing enrolled provider should use the appropriate provider-linking route or its administrator. My Account account-creation guide.

Complete the required account fields, review the terms, accept them, complete the verification and select Submit. Follow the activation message for your own user before opening PEMS. This account step does not approve Medicaid enrollment. My Account enrollment-account instructions.

Select the provider type and correct PEMS request
The PEMS provider-type page lists individual and group choices for these disciplines. Match the application to the actual person or organization; a group record does not stand in for every clinician’s enrollment. Review provider-type selection.
PEMS distinguishes a new enrollment, an existing enrollment request, maintenance, revalidation and reenrollment. Select the request that matches the record’s status. If a request is unavailable, first check whether an application is already saved or in progress. Follow the application-start guide.
Prepare the application packet
Use the current checklist for the provider and enrollment type. TMHP’s help page links enrollment training and supporting resources; required material varies with the request. Open Provider Enrollment Help.
Recommended preparation worksheet:
Record the individual or organization NPI and reconcile the enrollment details with the applicable provider record.
Gather current license, ownership and disclosure information, plus the supporting documents the actual checklist requests.
Identify who owns submission, who will answer deficiencies and which account administrator provides authorized access.
Keep the submission confirmation, correction requests and final approved provider record together.
After submission: approval, effective date and MCO contracting
Recommended follow-up: distinguish a saved draft from a submitted application. Track requested corrections to completion and read the actual decision and effective date before relying on an enrollment record for billing.
