Can Residents and Supervisees Bill Virginia Medicaid?

There is a documented supervised-clinician pathway
Virginia Medicaid expressly recognizes outpatient psychiatric services furnished by defined residents and supervisees. Its participation chapter identifies LMHP-R, LMHP-RP and LMHP-S categories, and the approved state-plan amendment includes residents/supervisees working under qualified licensed professionals.
This is a useful affirmative hiring answer even though the sources do not complete the claim-NPI mapping. A practice considering a qualifying resident can plan around the stated credential and supervision conditions. A general associate job title or a master’s degree alone does not establish that the person meets one of those categories.

Get written approval for the actual duties and site
The participation chapter requires the relevant Virginia board’s written preapproval of supervision for specific clinical duties at a specific site before the person functions as the named resident/supervisee. Check the applicable counseling, psychology or social-work category and keep its approval with the personnel records.
The covered services require direct, personal supervision by a qualified licensed Medicaid-enrolled professional. Schedule that supervision as part of the role rather than treating later signature collection as the entire requirement.

Plan for one-business-day signoff
Therapy documentation needs the dated signature of the resident/supervisee and the qualified licensed supervisor. The supervisor must add a dated cosignature within one business day of the service to show review. The program’s licensed supervisor or manager can fill the direct-supervisor role when qualified.
An actual pre-degree student is a different category. The cited resident/supervisee provision does not establish general student-intern reimbursement. A graduate should first obtain the actual qualifying status rather than being relabeled a resident for billing.

Aetna Better Health of Virginia needs its own acceptance decision
Aetna’s current manual covers the July 2026–June 2027 contract year and supplies generic professional supervisor and renderer locations. It does not establish acceptance of every proposed resident, student or supervised practice arrangement. Confirm the exact clinician’s contract/roster handling and rendering identity before assigning Aetna-covered clients.
The manual’s literal supervisor row lists 2310D/2420D or Box 17B with DQ, separately from the rendering row. Those fields do not confer trainee eligibility or prove that every therapy claim needs the supervising row.

Resolve claim identity without erasing eligibility
The DMAS billing chapter and SPA do not settle resident-versus-supervisor NPI use or the group/clinic interpretation of the generic claim instructions. Ask for that exact mapping for the enrolled setting. Telehealth has its own service, consent and coding requirements and does not remove the resident’s supervision conditions.
A practice can distinguish the documented qualifying resident pathway from those remaining billing questions. Keep any managed-care acceptance decision attached to the actual product and clinician relationship.

Frequently Asked Questions
Does an unresolved NPI field mean residents are excluded?
No. Defined resident/supervisee eligibility is documented. The actual enrollment and claim identity still need confirmation for the setting and plan.
Which product and setting does this hiring answer cover?
Virginia DMAS outpatient psychiatric-services rules and the approved state-plan resident/supervisee category. Managed-care contract acceptance and claim identity remain separate; this is not automatic Aetna Better Health approval.
