Can Virginia Residents Bill Medicaid, Anthem or Molina?
By Bomi Health • October 29, 2026
For a Virginia counseling resident, psychology resident or social-work supervisee, the answer depends on the product. DMAS Medicaid outpatient psychiatric rules allow the defined LMHP-R, LMHP-RP and LMHP-S pathways with board approval and qualified supervision. The reviewed Anthem Virginia commercial manual does not establish a comparable resident-under-supervisor billing exception. Molina Complete Care Virginia D-SNP authority remains unestablished because usable current product-policy evidence was not retrieved. None of these findings means a resident can simply use any supervisor’s NPI.

Virginia Medicaid: a qualified resident pathway with claim-identity questions
DMAS Psychiatric Services Chapter II, revised July 17, 2025, pages 18–19 recognizes LMHP-R, LMHP-RP and LMHP-S in outpatient psychiatric services. LMHP-R includes counseling, marriage and family therapy, and substance abuse treatment residency categories; psychology residency and social-work supervision have their own definitions. The chapter requires written board approval for the specific duties and site, continuous compliance, direct personal supervision by a qualified licensed Medicaid-enrolled provider, dated clinician and supervisor signatures, and supervisor cosignature within one business day. A generic associate title does not establish these qualifications.
The claim field is a separate question
DMAS Chapter V, revised December 5, 2025, instructs the applicable CMS-1500 Box 24J to contain the NPI of the provider who performed the care. Its special note requires the provider numbers in 24J and 33 to match unless DMAS has approved the group/billing-provider relationship. Those generic instructions do not expressly resolve which NPI applies to each qualifying resident in each enrolled setting. Confirm the enrolled provider type, approved group relationship and resident-specific claim instructions. Do not automatically substitute the supervisor based on the supervision paragraph. Mental health clinic physician-direction requirements also remain setting-specific.
Anthem Virginia: independent credentialing is not a resident billing exception
The Virginia Professional Provider Manual v2 is effective October 1, 2026. Its credentialing scope, printed page 57, addresses practitioners licensed, certified or registered to practice independently. The behavioral-health criteria on printed pages 69–70 discuss independent licensure and profession-specific requirements. These provisions establish a credentialing framework; they do not establish a general prelicensed therapist exception or permission to bill under a supervisor. The full manual was retrieved for this review, but no complete resident/supervisee rendering pathway was established. Confirm the exact commercial contract, behavioral-health administrator, network and service date. Anthem Medicaid, Medicare Advantage, FEP and delegated arrangements remain separately unreviewed.
Molina Complete Care Virginia D-SNP: verify the plan and service date
A dual-eligible special needs plan has a Medicare product identity as well as Medicaid coordination. Virginia Medicaid supervision language alone cannot establish this plan’s rendering permission. The public Molina Medicare index and Virginia manual retrieval returned error pages during direct verification, so this review does not establish the current plan year, continued availability, supervisory policy or network requirements. An older indexed manual or a directory listing is insufficient to settle those questions. Obtain the dated plan document and exact administrator before applying any policy; do not infer either permission or prohibition from this retrieval gap.
Categories that still need their own decision
Do not collapse residents in counseling, marriage and family therapy, substance abuse treatment and psychology into one credential. Social-work supervisees, LMSWs, LBSWs, psychological practitioners, QMHP trainees, pre-degree students and nursing categories require their own applicable service and payer evidence. The reviewed DMAS resident provision supports only its defined categories and conditions. It does not complete every category’s billing identity, and it cannot be carried into Anthem commercial or Molina D-SNP coverage.
Independently licensed but not credentialed is a different question
A fully licensed LPC, LCSW or LMFT awaiting a network effective date is not automatically an LMHP resident or supervisee. The DMAS supervised-category provision does not resolve that clinician’s enrollment problem, and Anthem’s independent-practitioner credentialing rules do not supply a workaround under a colleague’s NPI. These sources were reviewed October 29, 2026; older service dates and other products need their own applicable policy.
Frequently Asked Questions
Does Virginia supervision approval authorize every payer claim?
No. Board approval, payer participation and the claim rendering identity are separate requirements. Medicaid FFS, commercial and Medicare products need their own evidence.
