Therapy EHR Comparison: Seven Alternatives and Bomi

By Bomi Team • October 14, 2026

Choosing a therapy EHR means choosing a clinical workflow, a monthly bill, and a way to manage the work between an appointment and payment. This survey compares SimplePractice, TherapyNotes, Sessions Health, Carepatron, TheraNest, TherapyAppointment, Jane, and Bomi using one stated setup.

Disclosure: this guide is published by Bomi. It is a sourced pricing and purchasing guide, not an independent product test or a ranking based on measured clinical outcomes.

How we compare costs at 20 sessions a week

Our scenario is one clinician, individual telehealth, secure client messaging, and one scheduler or biller user. We model 20 completed sessions weekly: 20 × 52 ÷ 12 = 86.67 monthly on average, rounded to 87 for transaction examples. Time off, cancellations, and your actual usage can change the bill.

Prices were checked October 14, 2026. We use standard US monthly rates after promotions, with optional AI for one note per session. Annual license fees are listed separately. These are selected configurations, not a complete feature-equivalence audit or the cheapest possible plan for every clinician.

Software totals exclude claim and eligibility transactions, card processing, taxes, paid reminders, other third-party charges, and human billing services. Jane’s optional insurance tools are also excluded. Bomi’s required billing fee is additional in every Bomi total; it cannot be omitted from a complete purchasing estimate.

SimplePractice: Plus is selected for scheduler or biller access; it is not the lowest solo plan. A $20 annual AMA fee is shown separately. Insurance transactions are additional.

TherapyNotes: Group is selected because the scenario includes a staff user. Basic telehealth is modeled; premium telehealth and insurance transactions are excluded.

Sessions Health: The total is $39 Professional plus $10 telehealth. Claims start at $0.25 each and eligibility reports are $0.15 each; those transaction fees are excluded.

Carepatron: Advanced is selected for staff permissions. The monthly rate is $49 per team member from the subscription help center, not the $39 annual-equivalent rate. Free and Plus may suit other setups.

TheraNest: Advanced supplies the modeled telehealth and messaging features. The $19.50 annual CPT license fee is separate; usage above included allowances is excluded.

TherapyAppointment: The total is $59 standard subscription plus $15 integrated telehealth. Lower introductory tiers are excluded. Electronic claims, eligibility, and card processing are additional.

Jane: Practice avoids Balance’s 20-appointment monthly cap. Insurance Billing, its clearinghouse account, and group telehealth are excluded from the displayed software total.

Bomi: The $29 subscription is not available without required billing at 4% of net collections. The first-three-month EHR waiver is excluded; third-party charges remain separate.

Read the table as a starting point

The modeled configuration includes a staff user, so lower solo or free plans are not interchangeable with every row. A practice without staff should price that smaller setup separately. AI usage can also reverse an apparent saving: a lower base subscription can become more expensive once every session needs a paid AI note.

If you take insurance, add your claims and eligibility volume. If you hire a biller, add the biller’s fee. If you do the work yourself, keep that labor separate from cash expenses. Do not present recovered revenue or estimated time savings as guaranteed reductions to an invoice.

What to know about SimplePractice

SimplePractice is worth considering if you want a tiered mental-health practice platform and expect to add staff or clinicians. The plan decision matters: a solo clinician without staff access may need less than the Plus configuration used here.

Starter, Essential, and Plus have different feature sets. Essential may be sufficient when you need telehealth and secure messaging but do not need the staff access modeled here. Compare the recurring plan rate after the introductory offer, not the discounted first invoice.

Note Taker is priced separately from the broader Care Aide suite. The comparison uses the standard standalone Note Taker rate, not a promotional price or the price of the entire suite.

What to know about TherapyNotes

TherapyNotes makes an important distinction between a sole-user account and an account with administrative staff. A practice with one clinician and a biller should compare the Group subscription rather than assume the Solo price applies.

The Solo subscription is $69 monthly for a sole user. The modeled Group subscription starts at $79 and includes nonclinical users. Basic telehealth is included; premium telehealth and other optional services have their own prices.

TherapyFuel is an optional clinician subscription. Its published overview says it is not currently available for group therapy sessions; verify your session types rather than treating every AI offering as equivalent.

What to know about Sessions Health

Sessions Health is a useful shortlist option when you want a base subscription with separately priced telehealth and AI. Its published pricing makes those additions visible, which helps a solo practice build an estimate around actual use.

Professional is $39 for the first practitioner, and integrated telehealth is $10 more. Administrative users are included. The free plan supports up to three active clients, so it is a different scenario from the ongoing practice modeled here.

AI Assist is a flat monthly add-on. The pricing page distinguishes supported workflows from features labeled coming soon; check in-person recording and any other required input before buying.

What to know about Carepatron

Carepatron belongs in this comparison because its free entry point and paid team plans answer different needs. A free clinician account should not be confused with a paid workspace that needs role-based staff permissions.

The subscription help center lists Advanced at $49 per user on monthly billing, or $39 per user on annual billing. Two monthly seats therefore total $98. Its marketing page displays discounts and annual-equivalent amounts; confirm the billing selector, currency, and renewal price before checkout.

Paid plans advertise included AI with unlimited tokens. The free plan lists 1 million AI tokens and 1 GB of storage; tokens are not a guaranteed number of completed therapy notes. Confirm applicable terms and workflow limits.

What to know about TheraNest

TheraNest is worth comparing when the choice turns on insurance features and plan allowances. Its Essentials, Advanced, and Premier tiers have materially different inclusion rules, so the lowest subscription is not a stand-in for every practice.

Essentials is $29, Advanced $59, and Premier $89 per therapist monthly. Advanced includes telehealth and secure messaging and lists monthly practice-level allowances for claims, eligibility checks, and text or phone reminders. Premier advertises unlimited versions subject to its usage policy.

Ensai notes enhancement included in a plan is distinct from the paid Session Assistant scribe. This comparison uses the $35 per-provider Session Assistant add-on, not a claim that all AI costs extra.

What to know about TherapyAppointment

TherapyAppointment is useful to evaluate when you want free administrative accounts and usage-based AI notes. Its low-volume introductory subscription should be separated from the standard rate for an established caseload.

At 40 or more booked sessions in a month, the published standard subscription is $59. Integrated telehealth adds $15 per provider. Administrative accounts and secure messaging are included. The lower introductory tiers do not represent this 87-session month.

AI Notes are $0.59 per chart note with unlimited free edits. A generated chart note and a recorded session are different billing units; the model assumes one paid note for each completed session.

What to know about Jane

Jane is worth evaluating when you want to compare clinic scheduling and documentation alongside therapy-specific platforms. For this caseload, its Practice plan is the useful baseline rather than its limited-volume Balance plan.

Practice is $79 monthly and includes unlimited appointments, staff profiles, one-to-one telehealth, and secure messaging. Balance is $59 but limited to 20 appointments per month, not 20 per week. Insurance Billing is a separate $20 monthly add-on for the first practitioner; the clearinghouse account is not included.

Jane lists five free AI notes per month and a $15 monthly unlimited AI Scribe subscription per opted-in practitioner. Check recording and dictation workflows against your documentation needs.

What to know about Bomi

Bomi is a different purchasing decision: its native EHR comes with managed billing. It is designed for practices that want to evaluate the clinical software and the people responsible for insurance follow-up together.

The ongoing EHR subscription is $29 per clinician per month. Bomi Billing is required at 4% of net collections. Individual telehealth, secure messaging, and scheduler or biller access are included. Optional AI notes from recorded telehealth sessions cost $1 per session.

Bomi AI notes are pay-as-you-go for recorded telehealth sessions. They are not a flat unlimited subscription and should not be presented as less expensive for every caseload.

Choose around the work you want to keep

  • Self-billing: test the claim queue, payment reconciliation, reports, and escalation work you will own.

  • Existing billing team: test permissions, visibility, and the cost of all required staff accounts.

  • Managed billing: compare service scope, collections-based fees, credentialing work, and accountability.

  • Cash-pay practice: avoid buying insurance features you do not need, and verify whether a managed-billing bundle suits your business.

How Bomi approaches the comparison

Bomi is not VC-backed and our billing team is in-house. We aim to make the required service fee and optional charges clear. Those facts do not mean Bomi is the least expensive option for everyone, nor do they establish anything about a competitor’s staffing.

Before switching EHRs

Ask for a written export and import inventory, a sample reconciliation, a cutover date, and continued access to historical records. Define who handles unpaid claims from the old system and verify future appointments before turning on reminders in a new one.

Frequently Asked Questions

What is the best EHR for a therapist seeing 20 sessions a week?

There is no universal winner. Compare the plan supporting your staff and telehealth needs, the expected AI-note volume, insurance transaction costs, and who performs billing follow-up. This survey models 87 sessions monthly and links every pairwise comparison.

Does the lowest subscription mean the lowest total cost?

No. AI, additional users, insurance tools, claims, payment processing, annual license fees, and billing services can change the total. Bomi’s EHR requires its collections-based billing service.
