NJ Medicaid Provider Enrollment for LPC, LCSW, LMFT and Psychologists

Start New Jersey Medicaid provider enrollment at the official NJMMIS Provider Enrollment Applications page. For LPCs, LCSWs, LMFTs and psychologists, the first decision is whether the service and provider type need full fee-for-service enrollment or the managed-care-only Cures Act route. Those routes do not grant the same billing rights.

Check the license and billing route first

The Behavioral Health Integration Provider Readiness Packet describes fully licensed psychologists, LCSWs, LPCs and LMFTs in NJ FamilyCare behavioral health. That does not establish that every profession has a full FFS enrollment option. An LSW or LAC under supervision should not use an independently licensed practitioner’s route as proof of independent enrollment eligibility.

As of October 9, 2026, the live NJMMIS catalog includes Psychologist/Other Psychotherapy providers, including LCSWs, as individual and group choices. It does not display a distinct LPC or LMFT full FFS choice. Ask the Gainwell enrollment unit which packet applies before submitting under another profession’s category.

Open Provider Enrollment Applications and locate Provider Type. The shared website control below comes from a current state training manual; its palliative-care application instructions do not apply to therapist enrollment.

Choose full FFS or the Cures Act MCO-only route

The readiness packet distinguishes a provider-type-specific full FFS application from a 21st Century Cures Act MCO-only application. MCO-only enrollment does not permit FFS billing. Confirm the intended service and participating plan before choosing.

In the dropdown, the Cures routes have separate individual and group entries. Select the packet that matches your enrollment role and entity. The manual example shows these entries near the top; scroll for other recognized provider types in the current catalog.

Match the application to the person or group

The readiness packet distinguishes individual Type 1 NPI enrollment from a group’s Type 2 NPI and tax identity. A group of one still needs its individual rendering clinician considered separately. Do not substitute a group approval for the clinician’s required enrollment or plan association.

For full FFS, follow the applicable individual, group or existing-group linking instructions. For Cures MCO-only groups, the packet places rendering-provider links in the MCO credentialing/contracting process rather than promising automatic state group linkage. Confirm the actual relationship before billing under the group.

Compile the current packet and supporting documents

Download the current application for the confirmed type from NJMMIS. Follow that packet’s provider agreement, signatures, license, tax identity, ownership and attachment requirements. Requirements depend on the applicant; do not use a generic checklist to assume that every individual therapist owes an institutional fee or needs a DEA registration.

The readiness packet identifies license and tax documents and additional group-entity documentation where applicable. Use current forms rather than a saved packet from a prior year. Review the person, group, service address and tax identity together before submission.

Submit through Gainwell and keep the approval separate

Use the current packet’s submission instructions and keep a complete copy. The current state provider resources list Gainwell Provider Enrollment at 609-588-6036 and njmmisproviderenrollment@gainwelltechnologies.com. Ask that unit about an unmatched profession, missing application, existing Medicaid ID or requested correction. Do not submit duplicate packets through multiple channels just to accelerate a response.

After approval, confirm the provider’s active record and the effective enrollment scope. State approval does not mean the therapist is in-network with every plan on the official DMAHS MCO list. Track plan credentialing, contracts and rendering associations separately.

What to do after state approval

Set up NJMMIS and separate biller access, then confirm the member’s MCO and behavioral-health benefit. Before the first session, verify eligibility for the service date.

Frequently Asked Questions

Which NJMMIS form does an LPC or LMFT use?

Confirm the route with Gainwell. The current catalog has no distinct LPC or LMFT full FFS choice; MCO-only Cures Act enrollment is separate and cannot authorize FFS billing.

Can an LCSW use the psychologist enrollment category?

The current catalog labels Psychologist/Other Psychotherapy providers, including LCSWs, with individual and group options. Confirm the appropriate packet and service scope with enrollment staff.

Does Cures Act enrollment let me bill NJ Medicaid FFS?

No. It is the MCO-only enrollment route. Full FFS provider eligibility and enrollment require their own confirmation.

Does a New Jersey Medicaid ID make me in-network with an MCO?

No. The plan must separately confirm credentialing, contracting, effective participation and any rendering-provider association.
