Do not assume every behavioral health professional enrolls under PT 14. Nevada’s February 2026 notice identifies separate types for physicians, psychologists, APRNs and physician assistants, as well as the licensed therapist specialties. WA 3834
Specialty 300 is limited to specified clinical interns
Web Announcement 3834 limits PT 14/300 and PT 82/300 to the specified social-work, marriage-and-family-therapy and professional-counselor clinical interns. It directs other professionals to their own provider type or specialty. A generic QMHP label is not enough to choose 300. WA 3834
If your license changed from intern to independent professional, review the state’s update/revalidation instructions for correcting the enrollment category. The notice distinguishes a specialty correction from a provider-type change requiring a new enrollment. WA 3834
Prepare the matching license and tax evidence
In an individual Flex application, select the Provider Type that matches your license and then add the matching specialty. The published individual guide shows PT 14 among the available types; it is not the correct choice for every behavioral health professional. Individual application guide

The linked 305 checklist calls for the clinical social-work license and tax-ID evidence. It allows specified tax forms and notes that an employee may use the employer’s tax ID under the stated circumstances. Read the exact checklist rather than assuming a new EIN is required for every clinician. LCSW 305 checklist
The 306 and 307 checklists identify the corresponding professional licenses. Collect the license that matches the specialty; keep the entity and individual records clearly identified in your application folder. LMFT 306 checklist
For professional counselors, use the CPC checklist linked by the state. Compare the application’s names and identifiers with the supporting documents before submission. CPC 307 checklist

Identify the individual or group being enrolled and the services it will deliver.
Open the current checklist from the state index and gather its required attachments.
Enter the application in Provider Flex, resolve incomplete fields and track the required electronic signatures.
Retain the application tracking details and respond to any request for corrections.
The state’s current Flex page requires electronic enrollment and re-enrollment, with electronic signatures. Older specialty PDFs retain original-signature wording; do not interpret that wording as permission to mail a paper enrollment packet. Provider Flex resources
Two 2026 changes to check before using an old guide
PT 82 and its specialties stopped being available for enrollment in Provider Flex on July 1, 2026 under the announced sunset. Do not start a new application from an old PT 82 tutorial; follow the state’s transition guidance for your services. WA 3763
The September 22, 2026 announcement index also lists new PT 14 specialty 309 for Behavioral Health Training Clinics. State enrollment and managed care are separate tasks
