Match the license to the type 11 specialty
IHCP lists behavioral health providers under type 11. As of October 9, 2026, the relevant specialties are listed below. HSPP and licensed psychologist are separate specialties; choose the one supported by the clinician’s license and endorsement.
114: Health Service Provider in Psychology (HSPP).
616: Licensed Psychologist.
618: Licensed Clinical Social Worker (LCSW).
619: Licensed Marriage and Family Therapist (LMFT).
620: Licensed Mental Health Counselor (LMHC).

IHCP distinguishes an individual or organization billing provider without linked renderers from a group billing entity with linked rendering providers. A rendering provider is the individual linked to the group. Our recommendation is to map the actual payment arrangement before starting the application.

The enrollment module allows billing, group, and rendering classifications for these specialties. HSPP requires a current license with the HSPP endorsement; licensed psychologist, LCSW, LMFT, and LMHC specialties require the corresponding current license documentation. Use the specialty’s current requirements for the selected classification.

Use the enrollment matrix to prepare documents
The matrix identifies application, agreement, W-9, and license requirements by specialty and classification, with Medicare or other supporting documentation where applicable. Distinguish HSPP authority from specialty 616
The behavioral health module permits HSPPs, LCSWs, LMHCs, and LMFTs to certify diagnoses, supervise treatment plans, and sign prior authorization requests within scope. Specialty 616 psychologists may enroll and render covered services, but do not have the same diagnosis-certification or treatment-plan-supervision authority; their PA request requires a qualifying professional’s signed order.

Start the appropriate enrollment action in PHP
For a new application, open the public PHP homepage, choose Provider Enrollment, then Provider Enrollment Application. Read the Welcome page and select Continue. This public application does not require a registered portal login. Use the action matching a new enrollment or an existing record’s change. Our recommendation is to retain the application reference and confirmation and resolve requests for missing documentation through the official process.

Confirm managed care participation separately
IHCP enrollment comes before managed care network enrollment. Contact the relevant MCE after state enrollment; a portal account or state provider number does not by itself establish the MCE’s participation decision.

Is every psychologist specialty 114?
No. Specialty 114 is HSPP; 616 is licensed psychologist. The HSPP endorsement and clinical authority differ.
Can LMHCs, LCSWs, and LMFTs enroll under their own specialty?
Yes. IHCP lists specialties 620, 618, and 619 respectively; select the classification matching the billing arrangement.
Does IHCP enrollment automatically credential me with an MCE?
No. Apply to the relevant managed care network separately after IHCP enrollment.
