Already With Evernorth? Link Your New Group Tax ID

If you already participate with Evernorth Behavioral Health and join a practice with a different tax ID, you generally do not submit a new application. The new TIN has to be linked to your existing provider record through a demographic update, which Evernorth handles by phone at Provider Services. Your individual contract does not mean the new TIN or its locations are on file.

How this usually shows up

  • In the provider portal, the Verify network participation screen lists the practice TIN but marks it “Not associated with provider,” and you cannot continue the Provider Information Form.

  • Evernorth replies that you are already participating and an application is not needed.

  • Evernorth says you are contracted but not linked to the TIN, even when the portal seems to show the TIN associated with you.

Evernorth’s recruitment team has told us directly that the Provider Information Form is for non-participating providers and is not used to update demographics. Its network participation page also says participation is not limited to particular TINs, which is why the fix is a record update rather than a new contract.

Before you call

Have these ready. In our experience the call fails without the last one:

  • Clinician’s individual NPI and current legal name

  • Any prior name the record might still be under

  • New practice’s legal name, TIN, and group NPI

  • Service locations to add and the billing (pay-to) address

  • The service address Evernorth currently has on file. A representative declined to discuss a record because the caller could not state it. If the clinician practiced somewhere else before, it is probably the old address.

The call

Evernorth’s provider directory change page and its written replies to us give the same route: call Provider Services at 800-926-2273, say “health care professional,” then choose prompt 4 for demographic updates.

  1. Ask whether the individual record is active, and under what name and address.

  2. Ask to link the new TIN and its service locations to that record.

  3. Ask them to update or confirm the billing address for that TIN, and have them read it back. This is the step that gets missed most often.

  4. Ask for the effective date of the link and a reference number.

  5. Ask which documents they need. The representative names them; for a name change, one asked for a license verification printout showing the new name and the group’s W-9.

Following up

On one roster link, the representative quoted up to 30 business days. Status questions about a demographic update go to Evernorth’s Provider Data Management team; the recruitment team has told us it cannot see them. When a written reply comes back, check it carefully. We have received one with a misspelled name and an impossible effective date, so ask Evernorth to confirm the exact participating name and date before you rely on it.

Keep old and new dates of service separate, and do not end an affiliation that should stay active. Payment setup is separate too: see setting up Evernorth EFT and ERA.

If the group itself is not in network

Linking a TIN to a participating clinician is different from a group that has no Evernorth agreement. When we asked whether a group and clinician were both in network and the answer was no, Evernorth pointed us to its application process. Since September 1, 2026, that runs through the Provider Information Form in the provider portal, and the user needs the Contracting entitlement from the practice’s website access manager. Frequently Asked Questions

Do I need a new Evernorth application for a new practice?

Not if you already participate. Evernorth treats a new TIN for an existing clinician as a demographic update, and it has told us its application form is not the place for that.

The portal says “Not associated with provider.” What does that mean?

Evernorth has your individual record but has not linked it to that practice’s tax ID. Provider Services makes the link by phone; portal access to the TIN does not.

Can I just start billing under the new TIN?

Not until Evernorth confirms the link, the billing address, and the effective date. Claims under an unlinked TIN do not match your provider record.
