Oregon OHP CCO Directory by County: Provider Lookup Guide

As of October 9, 2026, use OHA’s official CCO directory and county search to find Oregon Health Plan coordinated care organizations serving a county. Use the service-area field and ZIP restrictions, then verify the member’s actual coverage before selecting a claim destination.

How to look up an OHP CCO by county
OHA instructs users to enter the county name without the word “county,” extra spaces or punctuation. Check whether the result covers every ZIP code or only part of that county. Each result also links plan information and provider directories. Open the live county directory.
For a practice expanding into a new area, use this as a shortlist for network research. For a scheduled client, start with the member’s eligibility result. A service-area match answers which plans operate locally; it does not identify the payer for that particular visit.

Partial counties and overlapping plans
OHA lists PacificSource Central Oregon in only these Klamath County ZIP codes: 97731, 97733, 97737 and 97739. Trillium Southwest’s Linn County entry is limited to 97446, while its Lane entry covers the county. Verify these ZIP boundaries in OHA’s directory.
Do not treat one example row as an exclusive county assignment. When several plans appear, keep the plan names separate in your contracting tracker and record which provider network each application concerns. If a website, map or intake template disagrees with the current directory, verify the specific address and member coverage before changing the billing record.
Lane County follow-up
PacificSource’s Lane County exit notice places the transition to Trillium by February 1, 2026. PacificSource remains listed in other regions in OHA’s current directory. Update Lane-specific intake references without removing PacificSource from every Oregon workflow. Our Lane County transition follow-up covers continuity and older dates of service.
From directory result to a billable visit
Use provider MMIS eligibility verification to establish member coverage for the date and service.
Check your participation and authorization route with the responsible plan.
Select the correct medical or behavioral health payer ID after you establish responsibility.

Does my client’s county tell me which CCO to bill?
Use county service areas to find candidate plans, then verify the member’s date-specific coverage and the service’s responsible payer.
Is PacificSource still an Oregon OHP CCO?
The current OHA directory lists PacificSource in regions outside Lane County. Its Lane County transition should not be generalized to the whole state.
