Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

Do not label every LPC as an associate

Idaho’s counseling definition includes assessment and treatment of mental and emotional disorders within the clinician’s training. LPC and LCPC are distinct licenses, but the LPC is not simply a training permit. LCPC qualification, supervisor eligibility and each payer’s contracting criteria are separate questions. The rule allowing LPCs to serve as Board-approved supervisors ended July 1, 2026; use the current supervision rules for the intended experience. Before counting experience toward LCPC advancement, confirm its timing and eligibility; the Board rejected an applicant’s pre-LPC clinical-supervision credit interpretation in June 2026.

Social-work practice and qualifying experience need separate checks

DOPL no longer requires separate registration as a clinical social-work supervisor. The supervisor must still qualify, and the supervisee must develop and document the plan and experience. At least half of qualifying LCSW supervision must be provided by an LCSW. The rule for an LMSW providing psychotherapy and the rule governing qualifying clinical-experience supervision are not identical; satisfy both where applicable and continue required supervision until the clinical license is issued. The Board’s published interpretation treats supervised LMSW clinical practice as a time-limited path to LCSW, with a five-year maximum under the current rule. Do not treat supervision as a permanent substitute for clinical licensure or assume an extension request grants continued authority.

Use current rules and written client disclosures

A proposed rule change is not an adopted requirement. Follow the rules currently in force for MFT experience, intern registration and supervision. Counseling and MFT disclosures must be acknowledged in writing and include the credential, any required supervisor information, services and risks, confidentiality, fees, client rights and Board complaint information. Verify temporary, endorsement, compact or virtual-care authority before relying on it.

Training and temporary psychology roles have limited authority

A postdoctoral title or pending application does not itself authorize psychology practice. Confirm the applicable service-extender permit or statutory exemption. The Board’s temporary psychologist permit is a separate route for eligible out-of-state clinicians, limited to 30 days in a calendar year; it is not an ongoing Idaho license. Prescribing-related provisional certification is also distinct from a general psychotherapy credential.

Licensed Clinical Professional Counselor (LCPC)An active LCPC is Idaho’s advanced clinical counseling credential; practice within training and competence. An LCPC who supervises qualifying experience must meet the applicable supervisor requirements.Business ownership and professional authority remain separate.Verify the individual and group arrangements for each payer.Idaho Code § 54-3401: counselor credentials and scope
Licensed Professional Counselor (LPC)This is a full counseling license, not an associate credential. The practice act does not impose blanket supervision on all LPC counseling.Supervised experience is required for the LCPC upgrade. Payers may impose additional supervision or participation conditions.Do not advertise as an LCPC or assume an LLC changes the license level.Check the service, product and payer criteria rather than assuming universal eligibility or exclusion.Idaho Code § 54-3401: professional counseling scope
Licensed Marriage and Family Therapist (LMFT)An active full LMFT supports MFT practice within the statutory scope and the clinician’s competence.Confirm qualifications and Board approval before supervising interns or qualifying experience. The entity does not license other clinicians working in the group.Align the clinician, service, group and location with the payer agreement.Idaho Code § 54-3401: MFT scope

National steps

These steps are not unique to Idaho, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Decide Type 1 NPI versus Type 2 NPI and whether the practice will bill under an individual or organization.
  • Create or update CAQH and keep attestations current.
  • Align EIN, W-9, business bank account, and payer-facing business identity.
  • Confirm HIPAA privacy/security, Notice of Privacy Practices, consent forms, Good Faith Estimate workflows, and documentation setup.
  • Select EHR, clearinghouse, billing workflow, payment processor, ERA/EFT workflow, and denial-management process.
  • Build a credentialing packet with license, malpractice, NPI, CAQH, W-9, bank documentation, taxonomy, service address, telehealth policies, and supervisor information if applicable.
  • Track payer applications, effective dates, portals, claim test dates, ERA/EFT status, and recredentialing deadlines.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Choose the structure with Idaho counsel based on the actual owners, professions and services. Register applicable entities and assumed business names with the Secretary of State through SOSBiz. Idaho’s state business guidance exempts a sole proprietor using the person’s full name from that name/entity registration requirement; this does not waive professional licensing. The current professional-corporation and PLLC forms list psychology and social work among eligible professions, but do not list professional counseling or marriage and family therapy. Do not copy a professional entity designation from another clinician or state without confirming eligibility. Review ownership, naming, foreign registration, registered-agent and annual-report obligations for the chosen form. Check city or county business, zoning, home-office and occupancy requirements and any service-specific program approval. Keep legal name, assumed name, ownership, W-9, banking, NPI, addresses and payer records aligned.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Obtain a federal EIN when required and choose the tax treatment with an accountant. Entity formation and an S-corporation tax election are different decisions. Use the Idaho Business Registration process for the tax permits and employer accounts that apply to the practice, including income-tax withholding, unemployment and Industrial Commission requirements when applicable. A sales-tax permit depends on taxable activities; do not infer that every service practice needs the same permit or that all additional products and services are exempt. Plan federal and Idaho income taxes, estimated payments and payroll obligations for the actual structure. Maintain Secretary of State reports separately from tax filings, and recheck registrations when hiring, adding taxable sales, changing ownership or moving locations.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

Verify the client’s physical location, identity and the clinician’s authority for that jurisdiction at each encounter. Idaho’s Virtual Care Access Act requires appropriate professional standards, a valid provider-patient relationship and follow-up arrangements. As a practice workflow, document informed consent, privacy protections, clinical suitability, emergency access and a backup plan; apply the profession’s disclosure rules as well. An out-of-state mental or behavioral health provider may use the § 54-5714 registration route only if its license, disciplinary-history, insurance, jurisdiction and other conditions are met. This registration permits telehealth, not in-person practice or reciprocal full licensure. Confirm the issued registration and its expiration; DOPL requires a new application when the nonrenewable registration expires. The Act also contains specific exceptions, such as certain temporary continuity-of-care situations, that must be checked against the actual facts. Compact membership alone is not proof that the individual holds the necessary authority. Separately verify payer coverage, authorization, modality, documentation and claim instructions. For minors, review parental consent, custody, record access and confidentiality separately. Section 32-1015 includes exceptions and specific requirements for provider-supplied blanket-consent forms. The 2026 provisions in § 32-1016 raise additional consent and notification questions for specified transition-related requests; obtain focused legal review of their application and other confidentiality duties before setting office policy.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Idaho Code § 16-1605 requires a person who has reason to believe a child under 18 has been abused, abandoned or neglected, or observes conditions likely to produce that harm, to report or cause a report to DHW or the proper law-enforcement agency within 24 hours. Do not wait to prove the allegation. Follow applicable institutional reporting procedures without letting them delay the required report. For vulnerable adults, § 39-5303 requires covered professionals and care-provider personnel with reasonable cause to suspect maltreatment to report to the Commission on Aging’s designated APS provider as quickly as possible and within 24 hours. If the suspected maltreatment caused death or serious physical injury jeopardizing the adult’s life, health or safety, an additional law-enforcement report is due as quickly as possible and within four hours. These adult deadlines replace older three-working-day wording. Apply the vulnerable-adult criteria, not age alone. Use the current APS/Area Agency on Aging reporting channels, document the report and use emergency services for immediate danger.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

Magellan administers the Idaho Behavioral Health Plan (IBHP), including covered behavioral-health services for Medicaid members and other eligible residents. For Medicaid participation, keep the applicable Idaho Medicaid enrollment through Gainwell separate from Magellan credentialing and network contracting. Current 2026 MCPE guidance requires enrollment of applicable NPIs and taxonomies and identifies group/facility and Tribal exceptions; do not apply older “every NPI” guidance without checking the current crosswalk and exceptions. Since September 1, 2026, missing required enrollment or taxonomy can cause claim denials or rejections. An enrollment record in maintenance status can also prevent payment; resolve enrollment issues with Gainwell rather than assuming a Magellan claim dispute will cure them. Magellan’s supervisory-billing job aid requires an executed Supervisory Protocol Addendum for the participating group or organization. A supervisor’s NPI alone is insufficient; confirm covered services, practitioner qualifications, supervision, documentation and claim identities under the actual agreement. Verify the responsible payer, member eligibility, service coverage, authorization and rendering-provider arrangement. For commercial insurance, use the current product-specific requirements. Blue Cross of Idaho’s covered-provider policy includes LPC, LCPC, LMFT, LAMFT, LCSW and LMSW categories, but its credentialing and contracting requirements still apply and do not remove supervision imposed by law. Track accepted contracts, clinician/group affiliations, locations, activation and effective dates, plus EFT/ERA and portal setup. A license, NPI, application or group contract alone does not establish that every clinician and service is payable.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.
