Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

The July 2026 associate pathways are available

DCCA opened applications for associate mental health counselors, associate marriage and family therapists and associate psychologists ahead of the July 1, 2026 launch. Use the current forms and confirm that the license has actually been issued. MHC and MFT provisional licenses last one year with a statutory additional one-year renewal option; the psychology license is valid for one year and is nonrenewable, although DCCA permits reapplication. A new application does not automatically extend authority. Plan the supervision and full-license transition before the credential expires.

Use the current MHC experience guidance

DCCA’s current MHC FAQ recognizes qualifying supervised experience obtained before July 1, 2026 and explains that graduates obtaining experience outside Hawaii do not need a Hawaii provisional license solely to accumulate those hours. For practice in Hawaii after July 1, a pending application does not replace an issued license or a qualifying statutory exemption. A clinician who already meets the full-license requirements may apply directly for full licensure.

Social-work clinical training has its own pathway

A post-master’s clinical social work intern may perform supervised clinical diagnosis and psychotherapy under the statutory exemption. Qualifying supervision requires an LCSW with at least 4,500 hours of post-master’s clinical experience; the historical alternative-supervisor transition has expired. The July 2026 reimbursement provision still requires the supervising LCSW’s contracted-provider eligibility and a training-appropriate billed rate.

Exemptions depend on the profession and role

Hawaii’s MFT title restrictions, MHC practice restrictions and psychology exemptions are not interchangeable. Student placements, government roles, supervised psychological-assistant work and other licensed professions have specific conditions. An exemption for a particular role does not establish an unrestricted private-practice credential, a protected title or payer participation. Confirm the actual services and public description with PVL.

Medicaid associate implementation needs confirmation

Med-QUEST posted SPA 26-0010 to add supervised provisional and associate-level services, with a requested July 1, 2026 effective date. The published notice is a proposed State Plan Amendment, not a CMS approval letter. This review did not verify federal approval or a universal operational billing pathway. Confirm the approved coverage, provider category and plan instructions before scheduling Medicaid visits under a new associate credential.

Licensed Mental Health CounselorAn active full Hawaii license supports practice within the mental-health counseling scope and the clinician’s competence.Supervising an associate is a separate qualification and documentation check.Business registration does not replace professional licensure.Confirm the clinician’s eligibility for each intended network and product.Hawaii Mental Health Counselor licensing
Associate Mental Health Counselor — provisional licenseNo independent authority under this license. The July 1, 2026 pathway requires direct clinical supervision.Use the qualified supervisor identified on the license. Confirm issuance before practicing or accruing Hawaii hours unless a statutory exemption applies. An application, degree or LLC does not confer provisional practice authority. The law provides conditional reimbursement eligibility; verify the payer’s enrollment and billing process.HRS § 453D-7.5: associate counseling
Licensed Marriage and Family Therapist (LMFT)A full license supports MFT practice within competence. Hawaii’s MFT licensing law protects specified titles; it has express exemptions for overlapping work.Check qualifications before supervising an associate or signing off on experience.A trade name must not misrepresent the clinician’s credential.Match the licensed clinician, practice and location to the payer agreement.Hawaii Marriage and Family Therapist licensing
Associate Marriage and Family Therapist — provisional licenseNo independent authority under this license; direct clinical supervision is required. The license identifies the supervisor. Clinical-supervisor eligibility and the statute’s contracted-LMFT reimbursement condition are separate checks.Keep the supervised credential clear in public descriptions and patient paperwork.Confirm the payer’s associate criteria and the actual contracted supervision arrangement.HRS § 451J-7.2: associate MFT

Post-master’s clinical social work internNo independent clinical authority. This is a supervised statutory exemption, not a separate associate-social-worker license.Use the required intern title and qualifying LCSW supervision while completing clinical experience, including diagnosis and psychotherapy.A degree or LSW license alone does not establish the clinical-intern arrangement.Verify the supervisor-contract reimbursement conditions and the payer’s implementation before billing.HRS § 467E-6: clinical social work intern exemption
Licensed PsychologistAn active full Hawaii license supports independent psychology practice within authorized scope and competence.Training, assistant work and provisional associate practice have separate conditions.Verify the issued credential rather than relying on the doctoral degree alone.Confirm participation for therapy and assessment services separately where required.HRS § 465-7: psychologist licensure
Associate Psychologist — provisional licenseNo. This postdoctoral pathway requires direct supervision within a Board-approved training program. The supervisor must hold an active, unencumbered Hawaii psychologist license and notify the Board when supervision ends or is completed. The statute provides a one-year license; it is not a full psychologist license.Check the statutory reimbursement conditions and payer-specific credentialing.HRS § 465-7.2: associate psychology
Psychological assistant or psychology student/traineeNot independently licensed by this designation; the specific statutory exemption must apply.Assistant services require licensed-psychologist direction and applicable Board rules; students must satisfy the training exemption.Do not represent the person as a licensed psychologist or associate psychologist. An exemption does not by itself establish an enrolled rendering-provider credential.HRS § 465-3: psychology exemptions

National steps

These steps are not unique to Hawaii, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Decide Type 1 NPI versus Type 2 NPI and whether the practice will bill under an individual or organization.
  • Create or update CAQH and keep attestations current.
  • Align EIN, W-9, business bank account, and payer-facing business identity.
  • Confirm HIPAA privacy/security, Notice of Privacy Practices, consent forms, Good Faith Estimate workflows, and documentation setup.
  • Select EHR, clearinghouse, billing workflow, payment processor, ERA/EFT workflow, and denial-management process.
  • Build a credentialing packet with license, malpractice, NPI, CAQH, W-9, bank documentation, taxonomy, service address, telehealth policies, and supervisor information if applicable.
  • Track payer applications, effective dates, portals, claim test dates, ERA/EFT status, and recredentialing deadlines.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Choose the entity after reviewing the owners, professions and services with Hawaii counsel. The Business Registration Division (BREG) handles applicable formation, foreign qualification, registered-agent and ongoing entity filings. A sole proprietor does not file an entity formation with BREG. Hawaii trade-name registration is optional and provides public notice; it does not create ownership of the name or professional approval. Research existing names before using one. Do not assume every therapy profession can use a professional corporation or that adding “PLLC” establishes an authorized structure; professional-entity eligibility and ownership need a discipline-specific review. Confirm county zoning, home-office, occupancy and any service-specific program requirements for the actual location. Keep legal name, trade name, ownership, addresses, W-9, bank, NPI and payer records consistent.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Determine federal tax classification and obtain an EIN when required. Set up applicable income, estimated, self-employment and employer-tax workflows. Hawaii’s General Excise Tax (GET) applies to business activity and gross receipts rather than simply business profit; review GET registration and periodic and annual filing duties with the Department of Taxation and an accountant. The health-care exemption effective January 1, 2026 is limited to eligible taxpayers and qualifying Medicare, Medicaid or TRICARE receipts. A therapy license or receipt of an insurance payment does not by itself establish eligibility. Verify both the provider/entity category and the payment before claiming it; qualifying exempt receipts still have reporting requirements. Commercial insurance and private-pay revenue should not be treated as exempt under that provision. Review withholding, unemployment, workers’ compensation and other employer obligations before hiring.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

Confirm the client’s physical location and the clinician’s authority for that jurisdiction at each session. Use a Hawaii license or a verified applicable exception for care into Hawaii; do not assume an out-of-state license, application or compact membership alone is sufficient. Check the exact conditions and expiration of any temporary or emergency authority. As a launch workflow, establish identity, informed consent, a clinically appropriate modality, privacy safeguards, documentation and a local emergency and interruption plan. Check the rules where the clinician is physically working as well. Hawaii Medicaid’s telehealth statute protects coverage for otherwise covered services, subject to its conditions and applicable federal requirements. Its mental-health audio-only provision is conditional, not a promise that every telephone visit is payable. Confirm the member’s plan, covered modality, authorization, documentation, place of service and claim instructions separately from professional practice authority. Med-QUEST memorandum QI-2527 also requires providers unable to offer in-person care to tell patients at each appointment, document that disclosure and explain that the plan can help locate an in-person provider. Follow the memo’s specific audio-only visit and documentation requirements.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Under HRS § 350-1.1, covered professionals who have reason to believe a child has been abused or neglected, or faces the statutory foreseeable substantial risk, must immediately report orally to DHS or the police, with a written report as soon as practicable. Follow the current DHS instructions; internal notice does not replace the required report. For adults, HRS § 346-224 requires covered professionals who know or have reason to believe a vulnerable adult has incurred abuse or faces abuse without immediate action to report promptly by telephone to DHS and follow with a written report as soon as possible. Apply the vulnerable-adult criteria, not age alone. Hawaii’s online APS report meets the written component only: mandated reporters must also make the oral report. Keep current reporting channels and follow-up instructions available to staff, document what was reported, and use emergency services for immediate danger.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

Use Med-QUEST’s HOKU process for the applicable individual, rendering or group enrollment category. HOKU requires a Type 2 organization NPI for Group Billers; an individual’s NPI and a group’s record are different. Verify state enrollment, clinician affiliations and locations, then complete the separate QUEST Integration plan or other responsible program’s participation requirements. Check the member’s actual coverage and who administers the behavioral-health benefit before routing a claim. For eligible Community Care Services (CCS) members, Med-QUEST announced a transition to AlohaCare on November 1, 2026. Confirm the responsible plan for the date of service and the transition, authorization and contracting instructions; do not switch claim routing early. The new associate statutes expressly make supervised services eligible for insurance reimbursement under specified supervisor-contract and rate conditions. Those provisions do not eliminate credentialing, contracting or correct claim identification. HMSA’s July 2026 criteria include associate MHC, MFT and psychologist categories; follow its current HHIN+ “Apply to HMSA” process rather than assuming every payer uses the same application. Confirm each commercial or Medicaid product, supervision arrangement, accepted contract, activation and participation effective date in writing. Keep eligibility, authorizations, EFT/ERA and rendering-provider records current; a license, NPI or submitted application alone is not payment approval.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.
