Can QTTs or Student Interns Bill Wisconsin Medicaid?

By Bomi Health • September 17, 2026

There are two different hiring pathways

ForwardHealth reimburses services by qualified treatment trainees who are accumulating supervised hours toward independent practice. Its defined categories include professional counselors in training, marriage and family therapists in training, advanced-practice and independent social workers working toward clinical licensure, and unlicensed doctoral psychologists working toward licensure. A completed degree and the appropriate state credential or doctoral route matter.

An actual student is different. ForwardHealth says clinical students cannot enroll as providers merely because some regulations include them within a broad trainee definition. Their practicum services can instead be reimbursable under the student conditions in DHS 107.01(2). Hiring a degree-holding QTT and hosting a student placement therefore require different arrangements.

For degree-holding QTTs, establish enrollment and the biller

An eligible QTT needs their own enrollment and rendering identity, with supervision by an enrolled licensed provider. The 2024 clarification permits services through the supervising provider’s billing group and expands eligible settings beyond certified mental-health and substance-use clinics, subject to the coverage rules for the particular service and place of service.

This can support a group-practice hiring plan, including eligible practitioner-office services. It does not eliminate state scope requirements or individual and group enrollment. Report provider affiliations at enrollment, revalidation and changes. A master’s graduate without the specified training license or certification should not be treated as qualified solely because the practice has a supervisor; an unlicensed doctoral psychologist has a separately stated route.

For students, build a genuine supervised practicum

The student pathway does not reimburse the student directly. Current outpatient mental-health modifier guidance describes direct, immediate, on-premises supervision by an enrolled provider, with the supervisor documenting the services in writing. Establish the educational placement, allowable service and billing setting before assuming that a private-practice internship can use the degree-holding QTT program.

The older student implementation guidance identifies the supervisor as rendering and requires U7 in addition to the applicable professional-level modifier. Read that instruction alongside current handbook guidance rather than giving the student a fictitious enrolled-QTT identity. Ordinary remote supervisor availability should not be assumed to satisfy an on-premises student rule; any proposed exception needs applicable current authority.

Preview different claims and plan the license transition

For enrolled QTT services, the QTT’s NPI is rendering and an allowable agency, hospital, group or independent professional is billing. The 2024 update specifies U6 for master’s-level QTTs and HP for doctoral-level QTTs, while clinical students use U7 under their separate pathway. Naming the supervisor as rendering for every supervised encounter would erase this important distinction.

Before the first visit, verify the member’s benefit, coverage, supervisor enrollment, individual QTT approval where required, affiliation and claim preview. For managed care, establish the actual plan participation and implementation rather than assuming FFS enrollment is sufficient. When full licensure is earned, update the provider record. Finally, distinguish reimbursement from training credit: ForwardHealth warns that payment does not guarantee that a licensing board will count the supervised hours.

Frequently Asked Questions

Can students enroll as QTT providers?

No. The student practicum route is distinct from individual degree-holding QTT enrollment. Sources S1 and S5.

Whose NPI is rendering?

The enrolled degree-holding QTT uses their own rendering NPI. Student guidance uses the supervisor’s rendering NPI, with U7 and applicable professional modifier; verify the current service rules. Sources S1, S2 and S6.
