A facility exception gives a concrete hiring answer
BlueCare’s October 2026 manual permits a non-independently licensed master’s-level clinician’s professional services under a supervising professional’s NPI when the clinician is an employee of a qualifying licensed Mental Health Outpatient Facility and the facility’s BlueCare contract expressly allows that arrangement.
This can support a facility hiring a qualified post-degree clinician. An ordinary practice cannot assume that a master’s degree, an NPI or a supervisor makes it the licensed facility described in the exception. Confirm the employer’s licensure and actual contract before offering that insured caseload.

For a post-degree hire, arrange employment, supervision and the roster
The clinician must appear on BlueCare’s prescribed nonlicensed roster, updated at least quarterly. The manual expects documented qualifications, regular individual supervision and service-specific supervision, including direct supervision at service initiation with later general supervision at the supervising practitioner’s discretion.
The facility exception directs the supervising professional’s NPI as rendering and rejects using the billing entity’s NPI in that place. Level-based modifier instructions and taxonomy matching still apply. The separately linked candidate-for-licensure policy was inaccessible, so do not invent additional candidate terms from its title.

Actual graduate students have a separate placement pathway
TennCare’s graduate-student policy requires an eligible accredited program, a school-required placement confirmed in writing and a formal school-agency agreement. The licensed outpatient facility’s MCO contract must permit the services. The qualified licensed supervisor must be enrolled under the facility; placement, supervision and medical records must be maintained.
Under PRO 22-001 section 3.4, each supervisor may oversee a maximum of four qualified graduate students providing services to TennCare members per academic year. Each student must receive at least one hour of individual face-to-face supervision per week from the primary supervising professional, in addition to other supervision.
The primary supervisor must be present and practicing at the same licensed facility and available for emergency consultation. If absent, another qualified supervisor must be present and available for consultation. The policy permits supervision requirements to be fulfilled through videoconferencing while retaining qualified-supervisor onsite coverage whenever the student is practicing; videoconferencing does not remove that onsite requirement.
BlueCare’s own manual repeats the onsite facility supervision condition. A post-degree employee does not become a student merely because that route exists, and a student cannot be assumed to meet the completed-master’s employee exception.

Finish the student-specific setup before scheduling
Student policy directs billing under the licensed supervisor’s NPI and Medicaid ID. BlueCare’s generic professional renderer locations do not establish a mandatory extra supervisor loop. The manual’s HO/HP education-level instructions also do not expressly assign the pre-degree student modifier.
Confirm student modifiers and any proposed patient telehealth arrangement with the plan. The reviewed manual contains conflicting telehealth coding statements; its general language should not be used to remove the student’s specific onsite-supervisor requirement. The student policy’s header and revision-history dates also differ, so avoid assigning a new substantive effective date from the header alone.

Keep the card product attached to the decision
Apply this answer to the actual BlueCare/TennCareSelect Medicaid facility arrangement. References elsewhere in the manual to CoverKids or BlueCare Plus do not establish identical eligibility for those products. Verify the member’s benefit and the service the facility is contracted to provide.

Frequently Asked Questions
Can my private group hire any graduate and bill the supervisor?
The documented BlueCare exception requires an employee of a licensed Mental Health Outpatient Facility whose BlueCare contract permits the arrangement. A generic group practice is not automatically that setting.
Which product and setting does this hiring answer cover?
BlueCare/TennCareSelect Medicaid facility services; exclude commercial BCBST, CoverKids and BlueCare Plus adoption. Actual graduate students and post-degree non-independently licensed master’s employees use different qualification tests.
