The notice covers three licensed categories
Geisinger’s October 23, 2024 provider update says Pennsylvania licensed social workers should follow the same incident-to process as licensed associate professional counselors and licensed associate marriage and family therapists. These categories are not independently credentialed or listed as independent practitioners in its directory. For a practice considering one of those hires, the notice establishes a real supervised billing route to investigate.
This is narrower than permission for any unlicensed person working toward a degree or license. Check the candidate’s actual Pennsylvania credential. A counseling student or graduate without the named credential is not established as eligible by this update. Do not turn that evidence gap into a categorical ban, but do not budget insured visits as though the named license requirement had disappeared.

Resolve the incident-to arrangement before hiring against revenue
The update refers providers to CMS Chapter 15, section 60 and a Novitas incident-to tool. That reference matters: the practice must establish the underlying professional service, treatment plan, continuing practitioner involvement and the relevant service and personnel conditions. A recurring signature after the therapist conducts care does not itself demonstrate the complete arrangement.
The federal framework also requires attention to which practitioner has the applicable benefit. A physician’s or clinical psychologist’s incident-to authority should not be assumed to belong to every counselor or social-work supervisor. Geisinger’s notice supports the named license categories, but it does not provide a complete product-by-product list of supervising practitioner categories or electronic claim fields. Resolve those details for the proposed position.

Separate clinical supervision, participation and claim identity
Build the practice’s pre-hire review around the clinician who will actually treat patients, the person responsible for clinical supervision, and the practitioner through whom reimbursement is permitted. Those roles may be related, but the update’s direction to use incident-to does not justify choosing a group owner’s NPI simply because that owner is already participating.
The notice is tagged All plans. That broad label does not eliminate the need to confirm the member’s benefit, setting and governing practitioner requirements. In particular, a plan’s commercial administrative guidance should not silently create a Medicare statutory benefit or override Medicaid requirements. Ask about the exact arrangement when the category, setting or product is unclear.

Plan the move to independent participation
Geisinger says the clinician may apply for empanelment after completing the clinical supervision program, required training hours and professional licensure. Record that transition in the onboarding plan. The same therapist may move from a supervised category to an independently eligible category without changing their name or NPI; the participation decision still needs attention.
The supported hiring answer is therefore a conditional yes for the named licensed categories, followed by an incident-to arrangement review and eventual independent enrollment. This gives the owner a concrete staffing path while keeping unresolved supervisor and product details visible before the first insured appointment.
