Start with the federal provider category

TRICARE recognizes both TRICARE-certified mental health counselors and supervised mental health counselors. These are defined provider categories, not informal descriptions of who receives supervision at a therapy group. An independently eligible certified counselor follows a different route from a counselor who needs physician referral and ongoing oversight.

The supervised category requires at least a master’s degree in mental health counseling or an allied field, state licensure or certification to practice mental health counseling, and two years of post-master’s experience including 3,000 clinical hours and 100 hours of face-to-face supervision. The qualification is completed experience. A new graduate accumulating their first clinical hours does not meet it merely because the practice employs a licensed supervisor.

Evaluate an associate or intern by actual qualifications

A student still completing the relevant degree does not meet the supervised counselor requirements. Neither does a wholly unlicensed graduate simply by having a degree. A person with an associate-level credential requires a more specific review: establish whether that jurisdictional credential and their completed experience meet TRICARE’s category before promising reimbursement.

TriWest’s West-region application makes this distinction concrete. Its supervised counselor section requests the state credential, education and completed experience, and explains that physician oversight is required. It separately describes the independent certified counselor category. Certification of the supervising therapist is not a substitute for eligibility of the person who actually treats the beneficiary.

Arrange the physician role and contractor certification

For supervised mental health counselors, the federal rule requires physician referral and ongoing physician oversight of therapy. Each claim also requires certification that treatment-result communication has been or will be made to the referring physician. A counselor’s ordinary clinical supervision agreement does not itself satisfy those physician duties.

East and West administer the same federal category, but the actual provider certification and network arrangements go through the relevant contractor. The West non-network application asks for the individual’s NPI and any group affiliation. A non-network certification form is not proof of network participation, and neither process justifies reporting an ineligible new hire as though the supervisor provided the session.

Treat For Life coordination as another benefit question

TRICARE For Life is Medicare-wraparound coverage. Establish whether the service is covered by Medicare, TRICARE or both, and follow the applicable payment order. A counselor’s TRICARE qualification does not create a Medicare incident-to benefit, and a Medicare denial alone does not prove that every TRICARE-only claim is payable.

For hiring purposes, one TRICARE guide can explain this shared category while keeping East, West and For Life administration explicit. Save the candidate’s degree, credential and completed-experience evidence; confirm actual certification and any network effective date; and establish required physician involvement before assigning covered care. This supports a qualified counselor arrangement without misrepresenting it as a broad prelicensed workforce exception.

Frequently Asked Questions

Does supervised mental health counselor mean a new associate learning under an LCSW?

Not automatically. The federal category requires a state credential, completed degree and specified completed experience, with physician involvement. Source S1.

Does For Life make Medicare pay an otherwise ineligible associate claim?

No. Resolve Medicare and TRICARE coverage separately and apply the correct payment order. Sources S1 and S3.
