Resolve the current presence condition before planning the hire

Lucet’s current July 2026 manual retains an entire-session presence condition in its general Billing Submission section (PDF page 73) for services delivered by interns and provisionally licensed providers. Read that condition alongside the availability language and applicable Alabama or Florida appendix. Appendix approval and supervisor availability alone do not establish that remote-only oversight is billable. Obtain an applicable written reconciliation before staffing or budgeting on a different presence arrangement.

Choose the appendix before choosing the clinician

Lucet’s July 2026 manual permits reimbursement for some intern and provisionally licensed services, subject to health-plan approval, state law and benefit coverage. Section 2 expressly sends readers to the plan appendices. The appendix covers fully insured, FEP and self-funded accounts, but that umbrella does not mean each account approves supervised services. Identify the member’s benefit administrator, product and employer group before using a general plan entry.

The BCBS Alabama network-through-Lucet entry, including Southern Company, marks interns and provisionally licensed professionals approved. Florida Blue PPO approval excludes Medicare Advantage. The Florida Blue HMO entry names Hillsborough and Palm Beach counties and excludes Medicare Advantage and BlueMedicare Classic Plus HMO. Florida Blue FEP and Medicare Preferred HMO entries mark these clinicians not approved. Do not expand that county-labelled HMO entry into statewide approval.

Why payer ID 00590 does not answer eligibility

Tampa General Hospital and Polk County Public Schools have separate employer-group appendices. Both explicitly mark interns and provisionally licensed professionals not approved. Both also list electronic payer ID 00590 and a 180-day filing period. Those routing and deadline instructions do not override the eligibility restriction.

Hypothetical example: a practice sees 00590 on a claim setup and assumes the general Florida PPO approval applies to a Tampa General Hospital member. Appendix A.1 defeats that assumption. Changing the rendering identity to the supervisor or adding a modifier cannot establish permission that the applicable appendix expressly withholds. This example identifies a scope conflict, not a predicted claim outcome.

Supervision is an organizational responsibility

Lucet credentials supervisors rather than directly credentialing or contracting with interns and provisionally licensed professionals. The supervisor must work for the same organization as the supervised clinician and meet applicable state supervision requirements. Clinicians must maintain applicable licensure or registration, practice within competence and use their actual credential title. The manual does not supply a complete Alabama or Florida license-class crosswalk.

Before services, the clinician must disclose the supervisory relationship, provide supervisor contact information and obtain the member’s consent. Supervisors must verify disclosure and consent, assess whether the clinician’s skills fit the member’s needs, and remain available whenever services are provided. Supervisors or employers must verify credentials, background and suitability. Supervisors must not accept payment from supervisees in exchange for supervision. An outside paid-supervision arrangement should therefore not be assumed to satisfy this pathway.

Separate the group’s billing NPI from individual rendering identity

The Florida PPO and HMO claims sections require a Type 2 NPI when billing with a tax ID, with an express exception for billing using a Social Security number. They identify the group/Type 2 NPI as billing provider and the individual NPI as rendering provider. The supervised-intern subsection separately directs billing under the supervisor’s NPI. Read together, these instructions preserve group billing identity and the supervisor’s individual identity for the supervised-intern service; a Type 2 NPI does not become the rendering clinician.

The Alabama and Florida supervised-intern instructions list GC, HO and HP as applicable. GC describes resident services under a teaching physician; HO describes master’s-level educational attainment and HP doctoral attainment. The list is not an instruction to append all three, or to use GC for every associate. Because the filing subsection is labelled “Supervised Interns,” its application to each provisionally licensed non-intern category still needs confirmation.

For the reviewed Florida PPO/HMO instructions, electronic submission uses 00590 and timely filing is 180 days from service. Do not copy those Florida instructions to Alabama. No exact CMS-1500 supervisor qualifier, separate supervisor field or 837P loop mapping is established here.

Resolve telehealth and the Alabama APP distinction separately

All Alabama providers must have a physical Alabama location; telehealth-only providers also need an Alabama physical address for each TIN. Meeting that general prerequisite does not establish associate-specific patient telehealth approval or coding. Florida telehealth reimbursement remains subject to plan guidelines. Supervisor availability is likewise not a patient telehealth coding rule.

BCBS Alabama’s advanced-practice resource prohibits billing services performed solely by named CRNP, CNM, CRNA, PA or PA/SA practitioners under a physician’s name and NPI. That resource addresses those APP categories. It should not be substituted for Lucet’s express behavioral-health intern/provisional approval, nor should Lucet’s approval be extended to unrelated benefits. Establish the applicable pathway before applying either document.

Frequently Asked Questions

Does 00590 mean a supervised clinician is eligible?

No. Tampa General Hospital and Polk County Public Schools list that ID while expressly disapproving interns and provisionally licensed professionals.

Can the Florida group NPI serve as rendering NPI?

The reviewed Florida instructions distinguish the Type 2 billing NPI from individual rendering identity. The supervised-intern instruction identifies the supervisor’s NPI.

Does Lucet directly credential the intern?

Section 2 says Lucet does not directly credential or contract with interns or provisionally licensed professionals; supervisors must be Lucet-credentialed.

Does an Alabama address make associate telehealth payable?

No. The address requirement is a general prerequisite. Member coverage, associate-specific eligibility, supervision and claim conditions still need resolution.
