Can Associates or Interns Bill Harvard Pilgrim?

By Bomi Health • August 31, 2026

Policy research reviewed October 17, 2026. This hiring guide separates a documented pathway from the conditions still to confirm for your clinician and member benefit.

Sections

There is an affirmative commercial pathway

A practice accepting Harvard Pilgrim commercial coverage has a concrete policy to use when considering an associate or a graduate pursuing licensure. Point32Health’s behavioral-health policy expressly includes supervised practitioners and clinicians pursuing their license, with a participating supervisor responsible for submission. The hiring decision should therefore focus on whether the proposed clinician, supervisor, services and member product meet that arrangement, rather than treating the absence of a complete claim-field example as evidence that supervised care is forbidden.

Harvard Pilgrim product identification still matters. Access America and Passport have different administration relationships; the Harvard Pilgrim name alone does not settle which organization’s provider rules govern the claim. Resolve the member’s product and applicable geography before configuring the workflow. This is especially important when a practice also bills UnitedHealthcare or Optum and might otherwise reuse a familiar supervisory protocol without verifying that it applies.

Associates, students and graduates have different answers

The July 2026 Point32Health commercial BH policy expressly considers supervised practitioners and clinicians pursuing licensure for reimbursement when billed by a credentialed, participating supervisor. Implement the policy’s supervision, disclosure, consent and documentation requirements.

The modifier table includes HL (intern), but the policy does not define every eligible student placement. Do not infer that every pre-degree student qualifies from the modifier alone; confirm the specific program, credential and lawful service scope.

A graduate pursuing licensure can fit the stated supervised-clinician pathway, subject to lawful scope, participating supervisor, covered services and contract requirements. A degree without that arrangement is insufficient.

Before assigning insured clients

Confirm the member is in a checked commercial product and the supervisor is credentialed and participating.

Document the supervisory relationship, screening protocols, patient disclosure, supervisor contact information and written treatment consent.

Obtain group/supervisor loading instructions using the BH enrollment process; generic CAQH forms do not prove that every supervisee requires independent credentialing.

Obtain precise renderer/supervisor field instructions and the appropriate supervisee-level modifier; do not import NP/PA SA.

Student placement is a separate question

The commercial modifier table includes an intern designation, but that code is not an exhaustive student-eligibility policy. Ask about the exact degree program, completed education and lawful clinical role. The policy’s affirmative coverage of supervised practitioners should be preserved while a student-specific determination is obtained; neither automatic student eligibility nor an automatic student ban follows from the table.

Claim identity and product limits

Claims are submitted by the contracted supervisor with a modifier identifying supervisee level. Generic 2310B/2010AA mapping does not identify whose NPI belongs there for this pathway. Do not invent 2310D or DQ requirements.

Harvard Pilgrim commercial products covered by the policy’s applicability grid. Tufts Health Direct, Together, RITogether, One Care and senior products are not checked. Harvard Pilgrim Passport administration must be separately resolved.
