A usable pathway for the right facility
The owner’s first question is whether the practice fits the setting in the policy. The Massachusetts Carelon addendum provides affirmative permission for qualifying license-eligible clinicians in an in-network Fallon behavioral-health facility. It does not say that every private group using Carelon can adopt that arrangement. A facility planning postgraduate hiring can use the rule to identify the required supervision and participation steps; a private office must first establish that its own contract supports the same services.
This pathway is built around clinicians who have finished the education for their intended license and are accruing clinical hours. It therefore offers a different answer for a postgraduate hire than for a student still completing the qualifying degree. The latter is not established as eligible under this provision. A familiar Carelon name on the card or an available roster spreadsheet cannot bridge that educational or contractual gap.

Associates, students and graduates have different answers
The Massachusetts addendum permits license-eligible clinicians in in-network Fallon behavioral-health facilities after educational requirements are complete, with hours accruing and regular supervision by an independently licensed Carelon-credentialed/contracted clinician.
The documented Fallon pathway requires completed education. It does not establish coverage for a student still completing the qualifying degree.
A graduate may qualify as license-eligible while accruing independent-license hours if all education, supervision, facility and service conditions are met; graduation alone is insufficient.

Documentation Standards and Requirements
Before assigning insured clients
Confirm an in-network Fallon facility contract and covered/authorized service; do not broaden facilities to every private group.
Document completed education, licensure hours and regular qualified supervision.
Use the supervised-clinician roster process, not ordinary group roster automation; submission is not approval.
For WellSense patients, use current WellSense administration rather than the stale retained Carelon section.

Do not reuse the old WellSense arrangement
Carelon’s retained WellSense text is not reliable proof of current administration. WellSense’s own transition guidance moves the relevant Massachusetts products to WellSense for dates of service beginning January 1, 2026, with separate New Hampshire Medicaid and inpatient timing. A practice with both Fallon and WellSense patients needs separate current arrangements rather than one Carelon-wide workflow.

Claim identity and product limits
Fallon specifies supervisor rendering in 24J (UB-04 field 76); generic NUCC places NPI in unshaded 24J. Additional supervisor loops and current product telehealth reconciliation remain unresolved.
Fallon Massachusetts facility permission is affirmative. Carelon corporate identity and BHOVO do not extend it to other contracts; WellSense transitioned administration.
