Can Associates or Interns Bill Illinois Medicaid?

By Bomi Health • October 11, 2026

Sections

Can an associate or student intern see these clients?

Licensed associate: A qualified post-degree clinician can fit an Illinois FQHC/RHC subclinical pathway, or the distinct approved-agency staffing pathway. This does not establish ordinary private-practice LPC/LSW billing for this destination. Fully licensed LCPC/LMFT coverage must not be denied using the superseded June 2021 handbook exclusion.

Unlicensed graduate: A graduate without independent clinical licensure may qualify in the covered encounter-clinic or approved-agency setting if the actual degree, MHP/QMHP role and supervision requirements are met. Graduation alone does not create private-practice reimbursement.

Student intern: A student placement alone does not satisfy the completed-degree FQHC/RHC pathway. An agency must assess any independently held MHP qualification and its program rules; no general student-intern private-practice payment permission is established.

Before assigning insured clients

Classify the employer as ordinary practitioner/group, enrolled FQHC/RHC, or approved CMHC/BHC before budgeting supervised visits.

For an FQHC/RHC hire, verify the qualifying completed degree, clinic-site IMPACT enrollment, and supervision by one of the professionals named in the HFS notice.

For CBS services, verify CMHC/BHC employment and the exact MHP/QMHP qualification; a license title or college placement alone is insufficient.

Implement the corrected education-based T1040 modifiers for encounters, and obtain the specific plan subclinical rendering/supervisor instructions separately.

Scope: Illinois HFS fee-for-service and separately identified HealthChoice Illinois products. LPC/LSW or other preclinical status is distinct from independent LCPC/LMFT licensure. FQHC/RHC subclinical coverage requires an applicable completed graduate degree; CBS MHP/QMHP status requires the actual qualifications and approved agency setting. No universal private-practice MCO trainee permission is established.

Policy research reviewed October 17, 2026. Apply the policy version and participation terms governing the date of service.

A supervisor’s license does not, by itself, establish an Illinois Medicaid billing pathway. First identify whether the service belongs to an independently enrolled practice, an approved community behavioral services agency, or an FQHC/RHC encounter. Each setting has different qualification and enrollment requirements.

Start with the enrolled setting

Check the provider’s actual IMPACT enrollment and the service being delivered. A group practice is not automatically a Behavioral Health Clinic (BHC), and a clinic’s business name does not establish Community Mental Health Center (CMHC) certification. FQHC/RHC encounter policy is a separate route.

Use the comparison below to identify the policy family. Then verify the service date, clinician qualifications, supervision relationship and responsible payer. Enrollment and coverage evidence come before claim-field selection.

Decision

Independent private practice

Individually eligible and enrolled practitioner; documented authority for the actual renderer.

Do not substitute a supervisor’s credentials for an unestablished LPC/LSW or trainee reimbursement pathway.

Use individual-practitioner policy; obtain controlling authority for any proposed supervised arrangement.

CBS agency: CMHC or BHC

CMHC certification or BHC approval plus appropriate IMPACT enrollment; applicable staff qualifications and employment.

MHP/QMHP service and supervision rules apply within the approved agency pathway.

Use the agency service definition and staffing requirements.

FQHC or RHC

Clinic encounter enrollment, eligible graduate subclinical professional and specified supervisor.

HFS permits general supervision for the covered subclinical encounter pathway.

Use corrected T1040 encounter policy, then obtain payer-specific submission instructions.

Independent practice: establish the renderer’s own pathway

HFS expanded coverage to enrolled LCPCs and LMFTs for dates of service from December 1, 2021. These clinicians use the appropriate IMPACT provider type and specialty. The older June 2021 practitioner handbook’s exclusion of their independent services was superseded by that notice.

For APRNs, licensed clinical psychologists and licensed clinical social workers, 89 IAC 140.400(a)(2) limits practitioner billing to personally provided services. The general ancillary-staff language elsewhere in the handbook does not establish a private-practice LPC/LSW psychotherapy arrangement.

If an LPC, LSW or graduate trainee will deliver treatment in an ordinary private practice, require the current authority for that exact arrangement before selecting an NPI. The researched sources do not establish a universal HealthChoice Illinois permission or prohibition. Ask for the applicable product, service, clinician enrollment and supervision requirements.

CBS agencies: approval, employment and staff qualifications matter

The CBS handbook distinguishes CMHC certification under Rule 132 from BHC approval under 89 IAC 140.499 and Table O. Both use organization enrollment through IMPACT. BHC approval includes desk and on-site review and annual compliance; some services also require separate program approval. Enrollment is site-specific.

Under 89 IAC 140.453(b)(2), credentialed staff covered by that section must be CMHC/BHC employees unless they are independent practitioners or fall within its peer-support exception. A supervision agreement alone does not replace that employment requirement.

MHP and QMHP describe qualifications, not merely license initials. A relevant master’s or doctoral degree meets one QMHP route when combined with 1,000 supervised practicum/internship hours or one documented year of QMHP-supervised clinical experience. MHP includes QMHPs and specified other qualifications under QMHP supervision. Check the full rule against the employee’s credentials.

Agency therapy: match the service to staffing and medical necessity

CBS therapy/counseling requires at least MHP qualifications and generally an LPHA-reviewed and signed IATP completed within 180 days. The handbook describes a separate, limited LPHA brief-intervention exception for children under 21 before IATP completion.

The linked CBS fee schedule separates individual H0004 HN/HO, group HN+HQ/HO+HQ, family HN+HR/HO+HR, and brief intervention TF+TL. HN identifies MHP, HO identifies QMHP and TF identifies LPHA. These staff-level meanings must not be carried into the FQHC/RHC modifier selection below. The handbook’s FFS claim instructions do not establish MCO submission fields.

FQHC/RHC: a distinct graduate subclinical encounter route

HFS’s 2024 notices cover eligible graduate subclinical professionals, including people completing qualifying post-degree experience while complying with professional supervision and training-title requirements. The October correction includes RHCs and explicitly applies the coverage to FFS and HealthChoice Illinois.

Eligible supervisors are a psychiatrist, psychiatric-specialty nurse practitioner, LCP, LCSW, LCPC or LMFT. The October 18, 2024 correction replaces the earlier direct-supervision requirement with general supervision. The clinic must have the relevant encounter enrollment; this policy is not an ordinary private-practice trainee rule.

The CBS employee rule belongs to the CBS agency pathway. These encounter notices do not establish a blanket answer about contractor arrangements or individual subclinical enrollment. Verify the proposed clinic relationship and enrollment requirements rather than transferring rules between settings.

T1040: use the corrected education-based modifier

The December 22, 2025 correction requires a BH modifier on T1040 and selects it by the rendering subclinical professional’s education: AJ for social work, AH for psychology, and HO for counseling or marriage and family therapy. A physician/APN supervisor does not justify leaving the modifier off.

The special filing relief in that notice concerned specified 2025 claims and required a request within 180 days of the notice. It is historical relief, not an ongoing filing extension.

Turn the setting decision into a payer-specific question

Keep the setting approval, enrollment record, clinician qualifications, supervision arrangement and service documentation together. Identify the exact member product and date of service before requesting any missing submission instructions.

For managed care, obtain the plan’s rendering-identity, supervisor-reporting, telehealth and modifier-combination instructions for the approved arrangement. This guide supplies no general MCO box, loop, qualifier or supervisor-NPI recipe.

The same graduate clinician in three settings

Hypothetical private group practice: A master’s-level social worker is supervised by an LCSW, but no specific Medicaid reimbursement pathway for the arrangement has been established. Do not treat the LCSW’s enrollment as proof that the graduate’s treatment is billable. Obtain the controlling individual-practice and plan requirements.

Hypothetical approved bhc: The clinician is an employee, meets the applicable MHP or QMHP qualification route and delivers agency therapy supported by the required treatment plan. Evaluate the CBS agency service and staffing rules. The agency’s approval, employee qualifications and supervision are part of the eligibility analysis.

Hypothetical enrolled fqhc: The clinician meets the qualifying social-work educational and post-degree requirements and works within the covered subclinical encounter pathway under an eligible supervisor. Evaluate the encounter route and corrected T1040 + AJ education modifier. Confirm actual submission fields with the responsible payer.

Frequently Asked Questions

Does a supervisor’s signature make a private-practice LPC or LSW service payable?

A signature does not establish the missing enrollment or reimbursement authority. Determine the actual setting and obtain the controlling policy for the arrangement.

Can an ordinary group practice use CBS staff qualifications?

Not merely because its clinicians meet MHP/QMHP definitions. The agency approval, enrollment and employment requirements also matter.

Are LCPCs and LMFTs excluded from independent Illinois Medicaid practice?

The November 2021 notice expanded coverage for enrolled LCPCs and LMFTs. Do not rely on the superseded exclusion in the earlier practitioner handbook.

Does an FQHC physician supervisor mean T1040 needs no BH modifier?

No. The December 2025 correction requires the modifier corresponding to the subclinical professional’s education.

Does this guide identify the rendering NPI for every HealthChoice plan?

No. State coverage notices do not supply one claim-identity instruction for all products and settings. Obtain the responsible plan’s instructions.
