Start with Regence BlueShield of Idaho
Regence’s Adm148 permits qualifying graduate behavioral-health associates under its psychological behavioral-health exception. The listed categories include master’s- or doctorate-prepared clinicians registered with their state board on a formal licensure track and LMSWs subject to state requirements. This creates a conditional hiring pathway rather than a universal wait-until-independent-licensure rule.
Regence BlueShield of Idaho and Blue Cross of Idaho are separate payer identities. Blue Cross of Idaho’s PAP900 direct-contract/extender program should not be copied into a Regence arrangement. First identify the actual member card and product, then use the applicable agreement and the policy that governs the work.

For an LMSW, arrange the state plan and the payer supervisor
Idaho’s social-work board instructs LMSWs to formulate a clinical supervision plan, with an LCSW providing at least half of the clinical supervision. Its current guidance says clinical-social-work supervisor registration is no longer required. Those are state practice conditions, not confirmation that Regence has enrolled or approved the applicant.
Regence adds a different organizational condition: the supervising practitioner must be licensed and credentialed, in the same provider group and educated to at least the associate’s level. A clinically suitable external supervisor therefore does not, by itself, establish a qualifying Regence arrangement. Review both requirements when deciding who will supervise the new hire.
Supervision and Supervisor Information
Separate licensed associates, graduates and students
The policy is not restricted to independently licensed clinicians: its graduate-prepared, board-registered licensure-track categories can include clinicians who have not reached independent licensure. But a degree without the required registration is insufficient evidence of payment eligibility. Match the individual’s current status to the actual policy category.
A pre-degree student intern is a separate question. This policy does not establish a college-practicum reimbursement benefit. Calling a student an associate, or adding the supervisor’s signature, does not supply the completed education and registration described by Adm148.
Make supervision part of the actual job
The behavioral-health exception requires treatment monitoring, review of the member’s overall documentation, supervision meetings and availability for consultation by telephone or telehealth. Associates cannot supervise other associates or operate these services independently in their own practice. Assign enough qualified same-group supervision capacity before promising an insured panel.
These are the psychological behavioral-health requirements. Do not automatically import the policy’s separate palliative-care initial-service and direct-presence language into every psychotherapy arrangement. Equally, telephone availability for the supervisor does not establish patient-video coverage or a telehealth modifier sequence.
Use the correct identity and product limits
Regence requires SA and the eligible supervisor’s name and identification, but does not fully allocate rendering, billing and separate-supervisor claim fields. Resolve that implementation and any associate loading with the actual agreement. The routing directory’s 00611 identifier identifies a destination; it cannot approve a clinician.
Adm148 labels Commercial and Medicare Advantage. Medicare eligibility and enrollment remain separate from the policy’s commercial supervisor examples. Do not count an MA caseload as approved solely because an Idaho license appears suitable for the commercial arrangement. Complete the product and clinician-specific review before the first insured assignment.

Frequently Asked Questions
Is this Blue Cross of Idaho’s extender policy?
No. This is Regence’s Adm148 pathway. A different Idaho insurer’s enrollment and claim rules do not transfer.
Can the board supervision plan replace Regence’s same-group rule?
No. The state plan and the payer’s same-group qualified-supervisor condition are separate requirements.
