Can Associate Therapists Bill Blue Cross of Idaho?

Can an associate or student intern see these clients?

Licensed associate: PAP900 expressly provides a pathway for Idaho LMSW, LPC, LAMFT and LPA clinicians, through an actual direct contract or an enrolled sponsored-extender arrangement. The possibility of direct contracting is not confirmation that this applicant has a contract.

Unlicensed graduate: A graduate degree without the required license or registration does not establish eligibility under the reviewed sources. Obtain a candidate-specific covered-provider determination before budgeting insured visits.

Student intern: The reviewed sources do not establish payment for an actual student placement. A graduate or licensed-associate provision cannot be applied to a student solely because the practice calls the role an intern.

Before assigning insured clients

Determine whether the candidate will have a direct contract or sponsored-extender enrollment, and complete the enrollment packet and performing-provider designation.

For sponsorship, use an eligible MD, DO, psychologist, LCSW, LMFT or LCPC; the last three need required supervisory credentials. Verify state scope and supervision.

Load the billing and performing NPIs correctly; generic electronic claim instructions supplement, rather than create, extender eligibility.

Scope: PAP900 contract and covered-service conditions; no automatic adoption by Regence BlueShield of Idaho, BlueCard home plans, Medicare or Medicaid.

Start with the clinician's enrollment path

Blue Cross of Idaho recognizes two arrangements that a practice should distinguish before setting up claims: a clinician may have an independent contract, or may work as a sponsored mental health extender. PAP900 says the plan can contract independently with licensed master social workers (LMSWs), licensed professional counselors (LPCs), licensed associate marriage and family therapists (LAMFTs), and licensed psychological associates (LPAs). A license category alone does not tell you which arrangement an individual clinician has.

Start the review with the clinician's enrollment record and agreement. Identify the member's benefit, the treating clinician, the practice receiving payment, and any sponsoring provider. Record which arrangement applies before assigning electronic claim identities. An independent contract does not remove supervision duties imposed by the clinician's license.

Confirm eligibility, sponsorship and enrollment

PAP247's commercial policy distinguishes covered providers, service extenders, other clinical staff, and noncovered providers. These categories have different reimbursement consequences. Extenders must apply for their own performing-provider number; supervision does not make the supervisor the performing clinician on the claim. PAP247 says payment for extender services goes to the supervising provider or employing clinic/business.

For the sponsored mental health arrangement, PAP900 permits MDs, DOs, psychologists, LCSWs, LMFTs and LCPCs as sponsors. LCSWs, LMFTs and LCPCs must hold the relevant supervisory credentials; psychologists are limited to three extenders. Eligible extenders need the corresponding enrollment packet on file and their own performing-provider number. Services must stay within professional scope and meet applicable supervision and state licensing or registration requirements.

Do not substitute a similarly named credential. For example, PAP247 separately lists LSW among other clinical staff, while PAP900 names LMSW as an eligible mental health extender. Use the actual credential and applicable enrollment category. Keep the license, enrollment confirmation, sponsor information and agreement together so the billing team can review the arrangement without guessing.

Keep the electronic billing and performing identities distinct

PAP203, whose current header says Reviewed/August 2026, requires electronic professional claim submission in CMS-1500 format or its replacement. Its billing-number instructions place the billing NPI in loop 2010AA, NM108=XX and NM109=billing NPI. Its performing-number instructions place the individual performing/rendering NPI in loop 2310B, NM108=XX and NM109=performing NPI.

Read that crosswalk with PAP247 and PAP900: the enrolled extender remains the performing clinician, while the appropriate payee occupies the billing role. For a clinic arrangement, those identities differ. PAP203 also explains that a solo practitioner's billing and performing numbers can be the same. Receiving payment and performing the service are separate questions; supervision alone does not justify replacing the extender's rendering identity.

Check the outgoing electronic claim, rather than relying only on the clinician name displayed in your billing software. Confirm that the billing and performing roles match the enrolled arrangement, and review any rejection before changing an NPI.

Resolve the details these policies do not settle

These sources do not establish an extender-specific separate supervising-provider qualifier or modifier. They also do not establish a current paper 24J instruction for this pathway. This guide therefore stays with the documented electronic billing and performing identities.

Before filing a variant that needs additional supervisor, modifier or telehealth instructions, obtain the applicable plan or contract guidance. Confirm the member's coverage and the clinician's effective enrollment. The published administrative policies explain provider and claim requirements; they are not a member-specific benefit authorization.

Frequently Asked Questions

Can an LMSW or LPC have an independent contract?

PAP900 says independent contracting is possible for its named categories. Verify the individual's actual enrollment; the credential alone does not establish an executed contract.

Should a sponsored extender use the supervisor's rendering NPI?

PAP247 identifies the extender as the performing provider. PAP203 maps that performing identity to2310B; payment to a supervisor or clinic does not change it.

Which supervision modifier should we add?

No extender-specific modifier was established in these reviewed policies. Obtain applicable guidance rather than borrowing a modifier from another payer.
