How to apply for an NPI Type 2

By George Ruan • December 7, 2025

After you get your NPI Type 2, you'll need a billing process to match. Bomi bills under your LLC's NPI Type 2 so you're set up correctly from day one.

If you are a mental health therapist running your practice as an LLC, you will almost certainly need an NPI Type 2. Read this blog post for more details on the difference between an NPI Type 1 and an NPI Type 2.

This guide walks through exactly how to apply for an NPI Type 2, what information you need, and common mistakes therapists should avoid.

Sections

Step 1: Go to the NPPES Website

Visit the official NPPES site: https://nppes.cms.hhs.gov/

Log in using your existing individual NPPES account.

Step 2: Choose “Apply for an NPI for an Organization”

Once logged in:

  • Navigate to the section that allows you to apply for a new NPI

  • Select Apply for an NPI for an Organization

  • Select I am an Employee of the Organization

This is the key step. Many therapists accidentally apply for another individual NPI instead of an organizational one. Make sure you are clearly in the organization flow before continuing.

How to Navigate to the Correct Form

Select the Right Form

Step 3: Complete the Organization Application

You will be asked for information about the practice, not about you personally.

You should include:

  • LLC legal name exactly as registered

  • Practice location and mailing address

  • EIN

  • Contact information for the organization

  • Authorized official details

Important: Make sure to NOT answer with personal details. Answer it from the point of view of the company. For example, do Not Enter Personal SSN Information

Step 4: Review and Submit

Once submitted, most Type 2 NPIs are issued within a few business days, often faster. Confirmation should come through email.

Final Thoughts

Setting up an NPI Type 2 is a foundational step for therapists operating as an LLC. Done correctly, it makes insurance billing, Medicare enrollment, and practice growth much smoother.

If you are unsure whether your setup is correct, or if you already applied and something feels off, it is much easier to fix early than after claims start rejecting.

Frequently Asked Questions

Do I need a separate NPPES login to request an NPI Type 2?

No. Log into NPPES with the same account you used to obtain your individual NPI Type 1 and start the organization application from there.

Can I use my personal SSN on the Type 2 application?

You should not. The form asks for the organization’s EIN and business details—entering personal SSN data can delay or invalidate your submission.

How long does it take to receive the Type 2 NPI?

Most organizational NPIs are assigned within a few business days after you submit, and NPPES notifies you by email when it is issued.

What if I accidentally applied for a second Type 1 instead?

Contact the NPPES Enumerator to close the incorrect record, then restart the application ensuring you choose the organization flow and list yourself as an employee of the LLC.
