Magellan EAP Billing: EASI Forms, Case Numbers and Filing Dates

By George Ruan • October 22, 2026

Public guidance reviewed October 22, 2026. US therapy-practice billing; documentation review, not a live claim submission.

A Magellan EAP session is not automatically a regular Magellan medical claim. Magellan documents an Employee Assistance Service Information form, known as EASI, for EAP reimbursement. Using the right case and EAP form matters more than finding a familiar Magellan payer entry in an EHR.

Sections

Start in the EAP Services application

Magellan’s current support page says the EAP portal lets providers register cases, retrieve case details and packets, and submit EASI reimbursement requests. It distinguishes federal cases and provides a fax/mail fallback when federal EASI forms cannot be processed online. Magellan EAP Services support.

Use the case packet to establish the assigned participant and provider before requesting payment. If the case is missing, resolve access or assignment with the correct support team rather than creating an unrelated medical claim as a substitute. A portal-access problem is not evidence that the authorized EAP session should be charged to the client.

EDI destination and special modifiers

Do not assume a general Magellan medical payer ID, including a listing such as 01260, replaces EASI for this EAP workflow. The EAP-specific sources here direct reimbursement through the EASI process; they do not establish a universal 837P/HJ claim pattern. A separate medical-benefit assignment must be verified on its own terms.

Do not append HJ or select a diagnostic-evaluation CPT merely to make a medical-claim template accept the encounter. Complete the EAP form’s actual service, attendance and case fields, and obtain written clarification if your agreement instructs a different submission route.

What belongs on an EASI request

The published form asks for the MIS/provider identity, tax identity, case or MAT number, client and client organization, session dates and times, attendance, modality and case information. It states standard payment is based on 45–55 minutes, with longer sessions requiring preauthorization. Both pages must be completed. Magellan’s EASI form.

  1. Reconcile each service date and the case/MAT number against the referral packet. Check the provider/payment identity and any authorization for a nonstandard session.

  2. Complete the applicable EASI fields, including required close-out or clinical information. Avoid substituting an EHR appointment list for the required reimbursement request.

  3. Submit through the supported EAP portal, or use the assigned fax/mail route when directed. Save the submitted form and proof of delivery.

  4. Reconcile the paid service dates and amounts with your request. If a session is absent, distinguish a missing request from an adjudicated payment problem before resubmitting.

Public filing instructions use different date anchors

The downloadable EASI form says to fax or mail it within 90 days of the referral-sheet end date. Magellan’s published claims-settlement material instead describes EASI filing within 90 days of the service date. These instructions should not be silently treated as the same deadline. EASI form instructions.

For the service-date version, see the EAP reimbursement subsection of the claims-settlement document. Until the applicable case rule is clarified, submit promptly within the earlier potentially applicable window and retain written guidance for the case. Magellan claims-settlement practices.

Also use a current destination. The public EAP support page and the older reimbursement-contact PDF show different federal PO boxes. Confirm the current case-packet destination before mailing; do not rely on an old address copied into your EHR.

Compare EAP networks or review authorization and session-limit checks.

Frequently Asked Questions

Can I replace a Magellan EASI form with a standard 837P claim?

The EAP process documented here requires EASI reimbursement. Verify any written exception with Magellan rather than assuming ordinary medical EDI replaces it.

Does Magellan’s EAP filing deadline always start on the same date?

The public form and claims-settlement material use different anchors. Follow the case-specific rule, obtain clarification, and avoid waiting beyond the earlier potentially applicable deadline.

Use the cited program and transaction-specific instructions with your current referral and agreement. Clearinghouse listings establish connectivity, not authorization or a guarantee of payment.
