Carelon Wellbeing vs. Anthem EAP Claims: 99404-HJ and Routing
By George Ruan • October 22, 2026
Public guidance reviewed October 22, 2026. US therapy-practice billing; documentation review, not a live claim submission.
Carelon Wellbeing and Anthem EAP appear together in provider materials, but their reimbursement workflows are not interchangeable. Start by identifying the organization on the participant’s authorization. A familiar parent-company name is not enough to choose an electronic payer or upload destination.

Carelon Wellbeing: the current claims workflow
The May 2025 handbook says Carelon Wellbeing moved to CMS-1500/Availity reimbursement effective December 2, 2024, using 99404 with HJ and a valid diagnosis. It identifies BHOVO for clearinghouse submission. The change explicitly excludes Anthem EAP; California and Military OneSource use separate handbooks. Carelon EAP handbook.
BHOVO contains the letter O, not a zero. In your clearinghouse, inspect the actual destination name and transaction support before saving it. Keep this configuration distinct from a state Medicaid, Medicare or other Carelon-administered product that happens to share the brand.
Anthem EAP: use the confirmation packet
For Anthem participants, the handbook describes a Statement of Services Rendered through the Anthem Provider Center, with mail/fax alternatives; CMS-1500 with 99404 is also described. The billing address and TIN must match the confirmation. Do not infer that Anthem uses BHOVO or the same HJ instructions from the Carelon section. Anthem participant reimbursement, section 7.01.
If your Anthem agreement permits EDI, obtain the current product-specific destination and required claim fields before sending. A medical Anthem payer entry or state Blue plan ID is not automatically the EAP route. The practical distinction is whose benefit is being reimbursed, rather than which login your staff normally uses.
Build the claim from the approved encounter
Save the authorization and determine whether this is Carelon Wellbeing or Anthem EAP. Confirm the treating provider, covered dates and remaining visits.
For the selected route, compare the claim preview to its documented procedure, modifier and diagnosis requirements. Confirm current telehealth and place-of-service instructions separately.
For Anthem, use the authorized form or approved claims route and verify the identity against its confirmation packet. Resolve discrepancies with the program before filing.
Keep the form or claim preview and the delivery acknowledgment. Reconcile the payer’s response to the exact service dates instead of assuming every Carelon- or Anthem-labeled payment is this EAP claim.
Filing windows and missed appointments
Section 7.14 calls for submission within 90 days of the last authorized session or completion of care, and receipt of a complete valid bill within nine months of the last service. These are not interchangeable deadlines. The handbook also bars charging participants for missed or cancelled visits. Timely filing and missed visits.
Confirm the current agreement and apply the earlier applicable submission obligation. Do not treat the nine-month language as permission to ignore the 90-day requirement. For disputed or incomplete bills, record what was missing and the corrected-submission reference while preserving the original timely-filing evidence.
When an old CAF instruction appears in your EHR
Compare legacy templates with the applicable instructions and service date. Clarify a conflicting packet before submitting, rather than sending the same bill through two systems.
