Aetna Resources for Living EAP Claims: EAP20 and Billing Forms

By George Ruan • October 22, 2026

Public guidance reviewed October 22, 2026. US therapy-practice billing; documentation review, not a live claim submission.

Aetna Resources for Living has an EAP-specific authorization and reimbursement process. Before creating a standard Aetna psychotherapy claim, identify whether your referral uses the member-specific billing form or an approved electronic arrangement. The EAP contract, the clearinghouse transaction and the patient’s ordinary health plan are different pieces of the setup.

Sections

Stedi lists Aetna EAP under primary payer ID EAP20 and its internal identifier LTVUZ, with professional 837P support and enrollment-required 835 ERA support. The listing does not support 270/271 eligibility or 276/277 claim-status checks. These are Stedi-specific connectivity facts, not proof of your referral’s authorization or billing-field requirements. Stedi Aetna EAP entry.

Use the current payer record in the clearinghouse you actually submit through. Do not place LTVUZ in every EHR payer-ID field: it is Stedi’s identifier, and another vendor may expect a different supported alias. Do not substitute the ordinary Aetna medical payer entry simply because the client also has Aetna insurance.

The authorization/billing-form route

Aetna’s public EAP manual says a member-specific authorization/billing form provides identity, authorization, session count and valid dates. Complete it after EAP services and use its printed address or fax destination. The manual sets filing within 90 days of the last service date and bases reimbursement on the EAP contract. If your practice intends to use EAP20 electronically, confirm that the EAP accepts that route for the assignment and obtain the exact mapping of authorization, service, units and provider fields. A published connection does not by itself explain how a clearinghouse converts the member-specific EAP billing form into an accepted claim.

CPT codes, HJ and telehealth fields

The public form-based reimbursement section does not establish one universal electronic CPT/HJ configuration for every RFL referral. Obtain the current EAP coding instructions before entering 99404, a psychotherapy code or an HJ modifier. An EHR’s generic EAP template is not evidence that Aetna requires that combination.

For phone or video care, also confirm that the assignment covers the modality and how it must be reported. Keep the EAP instructions separate from the payer’s ordinary outpatient behavioral-health billing configuration.

Prepare a submission that can be reconciled

  1. Compare the authorization to the member, approved clinician, service dates and remaining visits. Resolve an identity or date mismatch before using the authorization.

  2. List attended service dates accurately and complete the required billing fields. Save the finished form or outbound electronic claim, not just an EHR appointment report.

  3. Use the approved submission channel once. Keep a fax confirmation, mailing evidence or clearinghouse acknowledgment with the claim record.

  4. Match the resulting remittance to the dates and contracted EAP rate. An ERA enrollment is a payment-information setup; it is not a replacement for submitting the bill.

Denied or unpaid RFL services

The manual directs appeals to the address on the remittance with supporting evidence of timely submission; its remittance disclosure supplies the appeal deadline. It also prohibits billing the member for covered EAP services. Do not assume a missed filing deadline transfers the balance to the client. RFL reimbursement and appeals.

Compare EAP networks or review authorization and session-limit checks.

Frequently Asked Questions

Is EAP20 the same as ordinary Aetna medical routing?

It is the primary ID on Stedi’s separate Aetna EAP entry. Confirm the EAP product and the current entry in your own clearinghouse before submission.

Does an active Aetna EAP ERA connection submit my claims?

No. An 835 connection returns remittance information. The EAP bill must still be submitted through its approved billing-form or electronic route.

Sources and scope

Use the cited program and transaction-specific instructions with your current referral and agreement. Clearinghouse listings establish connectivity, not authorization or a guarantee of payment.
