APEP Enrollment for Arizona Behavioral Health Therapists

Start Arizona Medicaid provider enrollment at the official AHCCCS Provider Enrollment Portal (APEP) page. Use its User Registration or Existing Users entry. Creating a user account is the first access step; the provider enrollment application and approval are separate.

Which therapist provider type should you use?

The AHCCCS Provider Enrollment Glossary (PEP-903) lists these individual provider types. Match the application to the clinician’s current license rather than choosing a similar-sounding group or facility type:

  • 85: Licensed Clinical Social Worker (LCSW).

  • 86: Licensed Marriage and Family Therapist (LMFT).

  • 87: Licensed Professional Counselor (LPC).

  • 11: Psychologist.

The glossary identifies these as individual enrollments requiring an NPI. It lists no enrollment fee for these four types. Do not apply an institutional-provider application fee to every therapist. Check the current glossary for the exact provider type and its supporting requirements.

This guide addresses independently licensed practitioners. The October 2026 behavioral health services guide sets separate requirements for associate-level staff and supervised services; it does not make an associate license equivalent to an independent biller.

Choose sole proprietor or rendering/servicing correctly

The individual enrollment guide distinguishes an Individual/Sole Proprietor from a Rendering/Servicing applicant. A rendering provider supplies services billed by another billing provider. A sole proprietor bills under the individual’s own tax identification arrangement. The choice needs to match how claims will actually be submitted.

For a practice using a business tax ID, the APEP FAQ explains group enrollment plus the individual practitioners’ enrollments. A group record does not replace the clinicians’ individual records. The group’s payment ID is a billing identity, not the clinician who rendered the therapy.

Prepare the application before starting

AHCCCS’s APEP application page names the NPI, W-9 and current license among enrollment preparation items. Use the provider-type glossary and individual guide to identify additional requirements for your application. Keep legal names, tax information and license details consistent across the submitted records.

Register your own APEP user account

Use the official APEP registration entry. If the practice already has an enrollment domain, arrange access to that existing record instead of starting a duplicate application.

Select the individual enrollment and applicant type

Select Individual/Sole Proprietor or Rendering/Servicing Provider, then Submit. Confirm the clinician’s provider type and whether the clinician bills as a sole proprietor or renders services billed by someone else.

Complete provider basics and associations

Complete Provider Basic Information with the clinician’s own details. Select the applicant type and answer who bills for the services and who is completing the application. Continue through locations, correspondence, specialties and billing-provider associations in the individual guide.

Add the license and finish the remaining checklist

In Step 6, add the license or certification, complete the required fields, then select Confirm License/Certification/Other and OK. The available license choices depend on the specialty selected earlier. Complete ownership/control, taxonomy and populations served, then upload the required documents in Step 10 and resolve missing items before submitting.

Save the application and check the approval date

Save the application identifier and monitor requests for information. Use the approved enrollment record and effective date when deciding whether a service date falls within enrollment. Submission alone is not approval.

The application sequence above follows PEP-916, Individual Enrollment. AHCCCS’s APEP home page explains that the effective date is generally the approval date and that retroactive requests are reviewed case by case. Do not promise a processing date or assume retroactive enrollment.

Check EFT and group access separately

AHCCCS requires EFT enrollment for specified billing-provider categories, including group billers and individual sole proprietors who are not rendering providers. Rendering enrollment does not automatically have the same EFT requirement. Use the conditions and instructions on the current APEP page for your applicant type.

AHCCCS enrollment is followed by plan participation

An approved AHCCCS ID is not an executed managed-care contract. Next, identify the plans you want to join using our ACC, ACC-RBHA and FFS credentialing guide. Before billing a session, check the member’s date-of-service eligibility and plan enrollment and follow the correct behavioral health claim route.

Frequently Asked Questions

Where do Arizona therapists apply for AHCCCS enrollment?

Use the official APEP page linked above. Register a user account if needed, then complete the appropriate provider enrollment application.

Does creating an APEP account mean my enrollment is approved?

No. User registration provides account access. Provider enrollment requires an application, agency review, and an approved provider record with an effective date.

Does an AHCCCS group enrollment cover every therapist automatically?

No. A group’s billing enrollment and individual clinicians’ rendering enrollments are separate. Match each clinician’s enrollment and billing-provider association to the practice’s actual claim arrangement.

Do LCSWs, LMFTs, LPCs and psychologists pay an APEP enrollment fee?

The Provider Enrollment Glossary reviewed October 9, 2026 lists no enrollment fee for individual provider types 85, 86, 87 and 11. Check the current glossary for your actual provider type rather than assuming institutional-provider fees apply.
