Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

Supervision approval comes before clinical work

For LPC, MFT, and addiction associates, use the issued license and the supervision plan approved for that profession and arrangement. The statute names profession-specific supervisors; current regulations and board materials also recognize specifically approved qualified licensed mental health practitioners. An independently licensed colleague is not automatically an approved supervisor. Resolve changes before continuing services, and use current Associate terminology rather than older Intern forms.

An LMSW agency role is different from independent private practice

Section 40-63-20 prohibits private or independent LMSW practice. Regulation 110-30 defines qualifying social, medical, and governmental agencies, including some organizations, entities, and practices. The workplace must employ the appropriate supervisor, who retains responsibility for services and bills the client or third party when charged. The board’s FAQ distinguishes this workplace role from an outside board-approved contract supervisor for LISW experience. Document both when needed; obtaining outside supervision alone does not authorize an LMSW solo practice.

Clinical and advanced social work are separate scopes

LISW-CP supports independent clinical social work; LISW-AP supports independent advanced, nonclinical practice. Neither independent specialty automatically confers the other. The Social Work Board does not issue temporary or provisional licenses, so a pending application is not an interim practice credential.

Psychology staff must remain under real clinical control

Regulation 100-8 governs revenue-producing psychological services by covered unlicensed personnel. It requires administrative and professional supervision, disclosure of supervised status, supervisor review and countersignature of written communications, and control of fees and payment by the employer or supervising psychologist. The psychologist cannot be employed by the supervisee. Confirm board approval before changing the usual shared physical setting. A temporary permit or educational exception has its own limits and is not an unrestricted ongoing practice license.

Use the July 2026 Medicaid policies

SCDHHS’s final July 2, 2026 bulletin confirms updated LIP Behavioral Health Services and RBHS manuals effective July 1, 2026. Use the current manual and subsequent notices for business, screening, supervision, telehealth, documentation, and billing requirements. Individual/group LIP enrollment and an organizational RBHS program are different pathways; sharing an office does not establish RBHS qualification.

CredentialIndependent practice?Supervision issue?Entity/business noteBomi relevanceOfficial source
LPC — Licensed Professional CounselorIndependent professional counseling within authorized scope, training, and competence.A full LPC license does not automatically establish qualification to supervise associates.Resolve clinical authority separately from entity formation and ownership.Apply using the actual credential, service locations, and individual or group affiliation.Counselor, MFT, addiction, and psycho-educational practice and title restrictions
LPC AssociateSupervised counseling only; not an independent clinical credential.Have the issued associate license and board-approved supervision arrangement before providing services.Review the proposed setting, ownership, and supervisor relationship with the board; a company filing does not remove supervision.Confirm a permitted supervised-service pathway for each payer and product.Professional counselor and associate application requirements
LMFT — Licensed Marriage and Family TherapistIndependent marriage and family therapy within statutory scope, including appropriate individual, couple, and family services.Associate-supervisor qualifications are separate from an ordinary full license.Keep the professional title and services accurate in public and payer records.Confirm the contract recognizes the clinician and the particular service.Counselor, MFT, addiction, and psycho-educational practice and title restrictions
Marriage and Family Therapist Associate — LMFT AssociateSupervised MFT practice, not independent practice.Use the MFT-specific approved clinical supervision plan and authorized supervisor.A private-pay arrangement does not dispense with licensing and supervision requirements.Resolve associate coverage, roster requirements, claim identifiers, and signatures before billing.Marriage and family therapist and associate requirements
LISW-CP — Licensed Independent Social Worker, Clinical PracticeIndependent clinical social work, including private practice.Board-approved supervisor status for experience toward LISW licensure requires additional qualifications.Use the exact South Carolina clinical designation in the practice and payer packet. This is the independent clinical social-work category in the Medicaid LIP manual.Social-work scopes, private practice, telehealth registration, and compact law
LISW-AP — Licensed Independent Social Worker, Advanced PracticeIndependent advanced, nonclinical social work; not independent clinical social work or psychotherapy.Clinical work cannot be inferred from the word “independent”; follow any applicable supervised masters-level conditions.Private advanced-practice consulting is distinct from an independent therapy practice.Do not substitute LISW-AP for LISW-CP in a clinical enrollment category.Social-work scopes, private practice, telehealth registration, and compact law
LMSW — Licensed Master Social WorkerMay not practice privately or independently; supervised clinical work can occur in a qualifying organized agency setting.Regulation 110-30 requires an appropriately credentialed supervisor employed by the agency and in the supervision chain. Some qualifying agency definitions include practices. Verify the actual arrangement rather than deciding from private ownership alone.Outside LISW-training supervision does not replace workplace supervision or establish independent billing authority.Social-work regulations, including qualifying LMSW agency settings in Regulation 110-30
LBSW — Licensed Baccalaureate Social WorkerOrganized-setting social work, not independent/private practice, mental-illness diagnosis or treatment, or psychotherapy.Remain within the statutory baccalaureate scope and authorized setting.A business entity does not turn the credential into an independent therapy license.Do not enroll or bill as an independent psychotherapy clinician based on this credential.Social-work scopes, private practice, telehealth registration, and compact law
Licensed psychologistIndependent psychology within the issued license, specialization, and competence.Separate rules apply when supervising unlicensed psychological personnel.Maintain clinical control and distinguish a full license from a temporary or training authorization.Confirm specialty, service, location, and group participation with each payer.Psychology scope and interstate compact law
Supervised psychology personnel; students, interns, and postdoctoral candidatesA supervised job title, degree, or training exception is not an independent psychology license.Regulation 100-8 requires continuing supervision and generally the same physical setting unless the board approves another arrangement; training exceptions have their own conditions. The supervising psychologist may not be employed by the supervisee; fees and payment remain with the employing agency or supervisor.Use the actual supervised status and payer-authorized arrangement, including required review and countersignatures.Psychology regulations, including supervision of unlicensed personnel
LPES — Licensed Psycho-Educational SpecialistPractice within the statutory educational, personal, and social needs of children and adolescents; not unrestricted general psychology.Follow competence and referral duties; school employment brings specific private-sector service restrictions.A school-psychology certificate alone is different from the LLR private-sector license. The Medicaid LIP manual recognizes LPES, but recognition does not expand professional scope.Counselor, MFT, addiction, and psycho-educational practice and title restrictions
LAC — Licensed Addiction CounselorAddiction counseling within the license’s scope; not automatic authority for every mental-health service.Qualified supervision is a separate credentialing question when supervising associates.Assess additional program or facility licensing when offering organized substance-use treatment.Confirm the addiction-service provider category and product eligibility rather than assuming general therapy-network admission.Counselor, MFT, addiction, and psycho-educational practice and title restrictions
Addiction Counselor Associate — LAC AssociateSupervised addiction practice only.Use an issued license and approved supervision plan with an authorized supervisor.Certification, associate status, and a full LLR license are distinct authorizations.Verify the program’s staffing and supervised-billing requirements.Addiction counselor and associate requirements
Next step: confirm your state board rulesUse the official South Carolina licensing source before you decide whether to setup independently, stay supervised, or join a group first.

National steps

These steps are not unique to South Carolina, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Define services, client population, solo or group staffing, in-person or telehealth locations, and each clinician’s authorized scope before choosing an opening date.
  • Resolve board supervision, workplace control, ownership, and any program licensing before filing a payer-facing practice structure.
  • Choose the lawful business and tax structure, obtain an EIN when required, establish banking, and align the legal name, public name, W-9, and practice address.
  • Obtain or update individual Type 1 NPIs and an organizational Type 2 NPI where applicable; reconcile taxonomy, licenses, CAQH, and payer records.
  • Prepare clinical disclosures, consent and financial policies, privacy/security arrangements, records access and retention, emergency coverage, and continuity plans.
  • Configure the EHR, scheduling, eligibility and authorization checks, claims, payment posting, ERA/EFT, balances, and denial follow-up; establish required Good Faith Estimate workflows for uninsured or self-pay clients.
  • Track individual and group enrollment, affiliations, covered services, contracts, roster updates, and effective dates separately for Medicaid, each MCO, and commercial products.
  • Before launch, verify claim readiness and assign responsibility for renewals, supervision changes, payer recredentialing, telehealth location checks, and ongoing policy updates.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Review a sole proprietorship, LLC, or professional corporation with South Carolina legal and tax advisers using the actual profession and ownership arrangement. Sole proprietorships do not file formation documents with the Secretary of State. LLC formation does not expand a clinician’s practice scope. A professional corporation has specific qualified-shareholder, governance, naming, and service rules; mixed-profession purposes depend on each profession’s licensing law. Obtain advice for outside investors, management arrangements, or supervised-clinician ownership. South Carolina has no general statewide DBA registry: check local trade-name documentation and the special assumed/fictitious-name rules for the particular entity. Use authorized professional titles; §40-75-30(E) prohibits LPCs, LMFTs, and LACs from using the title “psycho-therapist,” while their services remain governed by their statutory scopes. Confirm city/county licensing, zoning, occupancy, and home-office requirements for the actual address. DPH separately regulates facilities for chemically dependent or addicted persons, including outpatient programs; determine whether the proposed service model needs additional approval.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Separate federal tax identification, South Carolina tax accounts, and local business licensing. Obtain an EIN from the IRS when required and keep tax, banking, NPI, and payer identities consistent. Use SCDOR’s MyDORWAY application for applicable accounts, and review income-tax treatment, estimated payments, and business personal property with a tax adviser. SCDOR says a service provider whose service is not specifically taxed under Sales and Use Tax does not need a Retail License; assess taxable goods or additional services rather than assuming every therapy practice needs one. Local business licenses are separate and may be required by the city, county, or both; there is no statewide general business license. For employees, address withholding, unemployment registration, new-hire reporting, employment verification, and applicable workers’ compensation requirements before hiring.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

Verify the client’s physical location and the clinician’s authority for that jurisdiction at each encounter. Chapter 40-42 requires appropriate scope, the in-person standard of care, confidentiality, evaluation, follow-up, and technology competence; a relationship conducted solely by telehealth includes identity/location verification and disclosure of clinician credentials and location. Profession-specific rules still apply. Qualifying out-of-state counselors, MFTs, addiction counselors, LPESs, and LISW-CP-equivalent social workers may use their board’s separate behavioral telehealth registration route; the boards prohibit registrants from opening a South Carolina office or providing in-person care there. As reviewed November 1, 2026, the Counseling Compact’s official state table lists South Carolina as not live, and Social Work Compact multistate licenses remain unavailable. PSYPACT is available to qualifying psychologists with the required individual authorization: telepsychology requires E.Passport and APIT, physical presence in the declared licensed home state, and a client in a participating jurisdiction. Membership or a supervisor’s authorization is not an individual privilege. Build consent, privacy, emergency contacts, technology-failure procedures, and documentation into intake. Confirm each payer’s covered services, modality, locations, and claim coding separately; the current Medicaid LIP manual generally covers synchronous audio-video unless otherwise specified.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Mental-health and social-work professionals are among the mandated child reporters under §63-7-310. Report when professional information gives reason to believe a child has been or may be abused or neglected; conclusive proof is unnecessary. Report promptly to DSS or appropriate law enforcement. Allegations involving someone other than a parent, guardian, or person responsible for the child’s welfare must go to law enforcement. Telling a supervisor or starting an internal investigation does not discharge the individual duty. DSS’s reporting line is 1-888-227-3487; use 911 for an immediate emergency. Vulnerable-adult reporting is a different rule: §43-35-25 requires covered reporters to report suspected abuse, neglect, or exploitation within 24 hours or the next working day. Vulnerability is defined by functional impairment or facility residence, not every adult therapy client. Route covered state-operated/contracted facilities, including the specified Veterans’ Affairs settings, to SLED’s Vulnerable Adults Investigations Unit; other covered facilities to the Long Term Care Ombudsman; other settings to DSS Adult Protective Services. In emergencies, serious injury, or suspected sexual assault, contact law enforcement immediately as well. Follow additional death-reporting duties under the applicable statute.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

Healthy Connections Medicaid enrollment is separate from an MCO contract. Obtain official SCDHHS enrollment approval for the correct clinician, organization, service, and location; screening can include fingerprint checks and pre-enrollment site visits. The current LIP manual lists licensed psychologists, LISW-CPs, LPCs, LMFTs, and LPESs as eligible independent categories. Individual LIPs enroll before group affiliation, and the group needs its own NPI/enrollment; group linkage requires the individual’s written request and notarized signature. For supervised LIP services, the July 2026 manual requires a same-discipline enrolled supervisor with a board-approved supervision contract, housed at the same practice location and immediately accessible in person, by phone, or electronically; maintain the signed crisis protocol and required signatures. The supervisor is identified as rendering provider on covered supervised claims. This payer pathway never overrides LMSW setting restrictions or any board’s scope and supervision rules. Confirm how it applies to remote services before billing. Each MCO administers its own enrollment, authorizations, and reimbursement; verify member enrollment and the responsible plan for the service date. For commercial care, CBA’s published independent-entry standard covers specified behavioral-health networks and requires independent licensure; its process uses MyPEP. That standard is not a universal rule for every Blue plan or every supervised-service arrangement. Obtain the applicable payer/product’s written rules for associates, rosters, locations, identifiers, documentation, and effective dates. A CAQH profile, group NPI, signed note, or pending application does not establish participation.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.

Where Bomi can help in South Carolina

Bomi supports therapy practices in every state across the U.S. This guide organizes the South Carolina-specific setup questions that sit around the national NPI, CAQH, billing, and credentialing workflow.

Once the clinical and business arrangement is ready, Bomi can help carry the practice through insurance administration. Credentialing support is available within an ongoing Bomi Billing relationship and is not offered as a standalone service. State-board approval, entity advice, and payer eligibility remain decisions for the responsible board, advisers, and payer.

  • Align NPI, CAQH, W-9, practice locations, and the credentialing packet.
  • Track payer applications, individual/group affiliations, roster updates, and effective dates.
  • Organize eligibility, authorization, and properly approved supervised-service billing workflows.
  • Manage claims, payment posting, denials, balances, and revenue reporting.
