Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

Practice authority, ownership, and payer participation are separate

A business filing, EIN, NPI, CAQH profile, or supervisor’s signature does not establish clinical authority or an effective payer contract. Confirm the actual credential, service, approved setting, ownership, and claim requirements before accepting clients.

Approve candidate employment and supervision before counting hours

The LPC, LMFT, and LBP rules require employee status when candidates accrue hours in private practices. The board’s current instructions require examination results, a W-4, and an approved supervision agreement; hours count from board approval. Social-work clinical supervisees cannot simply work as independent contract labor. Supervision does not automatically permit billing under a supervisor’s NPI.

Check profession-specific clinical and program requirements

Oklahoma reserves private clinical social work to LCSWs and requires HSP certification for independent psychological health services. Addiction credentials have distinct scopes and conditional private-practice rules. Separately evaluate whether the proposed treatment program needs certification; an individual license does not settle facility requirements.

LPC — Licensed Professional CounselorA full license supporting independent counseling within authorized scope and competence.Maintain active status, continuing education, and any individual license conditions. An entity filing does not replace the professional license.Align the licensed clinician with the practice’s payer records.LPC full rules, effective September 1, 2026
LMFT — Licensed Marital and Family TherapistThe full license supports independent practice within its marital and family therapy scope.Candidate status is a separate supervised pathway.Use Oklahoma’s exact title: Marital and Family Therapist.Match applications to the actual license and services.LMFT full rules, effective September 1, 2026
LBP — Licensed Behavioral PractitionerThe full license supports independent practice within its authorized behavioral-health scope.Observe competence, renewal, and license conditions.LBP is distinct from a behavior analyst credential and the Medicaid umbrella term LBHP.Confirm the payer’s exact practitioner category.LBP full rules, effective September 1, 2026
LPC, LMFT, and LBP candidatesCandidate status does not authorize independent private practice.Accruing hours in private practices requires employee status and an approved, immediately available supervisor under the applicable profession’s rules.Do not substitute a self-owned entity or contractor arrangement for the approved employment and supervision pathway.Resolve candidate coverage and claim identifiers before insured services.Candidate supervision approval and employee documentation
LCSW — Licensed Clinical Social WorkerThe social-work license required for private clinical practice.Maintain continuing education and any individual restrictions.Independent clinical authority remains separate from ownership and payer approval.Prepare eligible individual and group enrollment records.Social-work scope, provisional credentials, and addiction licensure statutes
LMSW — Licensed Master’s Social WorkerDoes not authorize independent social-work practice or private clinical practice.Clinical work toward LCSW requires the board-approved supervision pathway. An MSW degree or supervision agreement is not an independent psychotherapy license.Verify the eligible setting and supervised-service billing rules.Social-work scope, provisional credentials, and addiction licensure statutes
LSWA — Licensed Social Work AssociateDoes not authorize independent social-work practice or private clinical practice.Work within the credential’s scope and approved supervision requirements.A bachelor’s-level or other qualifying degree does not establish independent clinical authority.Check the covered service rather than assuming therapist eligibility.Social-work scope, provisional credentials, and addiction licensure statutes
LSW — Licensed Social Worker; LSW-ADM — Licensed Social Worker with Administration SpecialtyIndependent nonclinical work may be permitted within the respective scope; private clinical practice requires LCSW.These are distinct graduate-level credentials with supervised-experience requirements.Nonclinical contract work is different from independently providing psychotherapy.Use the actual credential and service in enrollment decisions.Social-work scope, provisional credentials, and addiction licensure statutes
Provisional social-work licenseAuthority depends on the specific level issued and its conditions; provisional is not synonymous with beginning clinical supervision. The board describes a one-time, one-year exam-pending license that is not renewable.Verify the issued credential and expiration before continuing licensed services.Track expiration and the payer’s recognition of the actual status.Social-work supervision and contract-work FAQ

CADC — Certified Alcohol and Drug CounselorNot an unrestricted independent mental-health credential. Title 38 permits a conditional private-practice path with qualifying experience, continuing education, and LADC supervision. The statute separately prohibits CADC private or independent practice for co-occurring disorders.Confirm the intended addiction services, supervision, and setting with the board; do not read the conditional pathway as general psychotherapy authority.Establish service-specific payer eligibility before billing.Addiction-counseling private-practice conditions, Title 38
Addiction candidates, practicum students, and other traineesCandidate or educational status does not authorize an independent therapy practice.Addiction candidates need board authorization for supervised experience; educational exceptions remain setting- and profession-specific.An addiction candidate who owns a counseling business cannot act as clinical director or oversee its clinical services under Title 38.Resolve the actual training arrangement and payer rules first.Addiction-counseling private-practice conditions, Title 38

National steps

These steps are not unique to Oklahoma, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Decide Type 1 NPI versus Type 2 NPI and whether the practice will bill under an individual or organization.
  • Create or update CAQH and keep attestations current.
  • Align EIN, W-9, business bank account, and payer-facing business identity.
  • Confirm HIPAA privacy/security, Notice of Privacy Practices, consent forms, Good Faith Estimate workflows, and documentation setup.
  • Select EHR, clearinghouse, billing workflow, payment processor, ERA/EFT workflow, and denial-management process.
  • Build a credentialing packet with license, malpractice, NPI, CAQH, W-9, bank documentation, taxonomy, service address, telehealth policies, and supervisor information if applicable.
  • Track payer applications, effective dates, portals, claim test dates, ERA/EFT status, and recredentialing deadlines.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Choose the structure with the relevant board and an Oklahoma attorney before filing, especially for mixed-profession ownership or nonclinician investors. The Professional Entity Act covers these licensed professions and imposes formation, naming, ownership, and clinical-management requirements. Professional entity formation includes regulating-board certification; the Secretary of State provides professional LLC and corporation forms. The Act defines specific combinations of related professions, so employing multiple disciplines does not by itself establish permission for joint ownership. Do not assume every therapist must form a PLLC or that an ordinary LLC avoids professional restrictions. File the required trade-name report when operating under a name different from the legal name, including an individual using a practice name. Maintain the registered agent and ongoing entity filings, including the LLC annual certificate when applicable. Check local office or home-practice requirements and keep legal name, DBA, tax records, bank, NPI, CAQH, and payer records consistent.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Complete entity formation before obtaining that entity’s EIN; the IRS issues EINs directly without a fee. Choose federal tax treatment separately from the legal entity and plan income, self-employment, estimated, and employer taxes with an accountant. Use the Oklahoma Tax Commission New Business Center to identify the state accounts and permits that actually apply. Review withholding and unemployment-tax registration before hiring, along with workers’ compensation and other employer obligations. Check taxable ancillary sales or purchases rather than assuming every therapy practice needs the same sales-tax permit. Maintain separate bookkeeping, consistent W-9 information, and a calendar for returns, deposits, renewals, and entity filings.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

Confirm the client’s physical location at each appointment and the clinician’s authority there, while also checking requirements where the clinician is physically practicing. Oklahoma began accepting and issuing Counseling Compact privileges October 1, 2026. Eligible full LPCs must obtain the actual destination-state privilege before practice; membership or an application is insufficient. Oklahoma home-state eligibility includes active unencumbered licensure, primary residence, and the required background check. Incoming Oklahoma applicants follow the board’s application and jurisprudence process. Candidates and other therapy professions do not gain LPC compact rights. Psychologists using PSYPACT must maintain the appropriate authorization, including APIT for interjurisdictional telepsychology, and follow Oklahoma’s current notice instructions; acknowledgment is not a license. Document identity, location, informed consent, privacy, emergency contacts, and connection-failure procedures. Use appropriate HIPAA safeguards and required vendor agreements; determine separately whether SUD records fall under 42 CFR Part 2. For SoonerCare, current OAC 317:30-3-27 requires prior written parent/guardian consent for minors with specified information, renewed annually or when that information changes, plus parent/guardian notification after the service; school settings have separate provisions. Review applicable minor-consent and confidentiality law alongside these requirements. Verify covered codes, modifiers, service location, authorization, and documentation. Audio-only follows a separate rule and is not interchangeable with video.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Oklahoma requires every person with reason to believe a child under 18 is being abused or neglected to report immediately through the designated Human Services hotline. Do not wait for proof or an internal investigation. Maintain a separate vulnerable-adult reporting procedure: reasonable cause to believe a vulnerable adult is experiencing abuse, neglect, or exploitation triggers reporting as soon as the person becomes aware. Vulnerability concerns the statutory incapacity or impairment criteria, not age or a mental-health diagnosis alone. Human Services lists 1-800-522-3511 for abuse and neglect reporting and an online APS reporting route. Call 911 for immediate danger. Train staff on the relevant threshold, reporting destination, documentation, and confidentiality limits; internal notification does not replace the required report.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

Choose the enrollment category before building an insured caseload. OHCA state enrollment, individual rendering-provider records, group affiliation, and SoonerSelect plan contracting/credentialing are separate steps. Confirm the member’s actual fee-for-service or assigned managed-care coverage for each service date. OHCA’s CVO FAQ describes a common Availity application for Medicaid-only providers, with different handling for existing plan credentialing or delegated arrangements; a complete application does not establish an effective contract. Independent LBHP enrollment recognizes clinical social workers, LPCs, LMFTs, LBPs, and LADCs within scope. Adult coverage is a critical distinction: OAC 317:30-5-281 limits adult outpatient services in that independent LBHP category to specified preoperative assessments, while its children’s section includes psychotherapy. Psychologists have a different coverage rule, and qualifying outpatient behavioral-health agencies follow their own rules. This is not a general exclusion of adult Medicaid therapy: obtain category-, member-, service-, and plan-specific confirmation before promising coverage. Outpatient agencies require qualifying accreditation or ODMHSAS certification and other organizational standards; an organizational NPI alone does not make a small group an eligible agency. OHCA’s 2026 supervised-provider special provisions limit that rendering-provider pathway to contracted outpatient behavioral-health agencies, ITU outpatient clinics, or therapeutic foster-care agencies, with approved supervision, facility affiliation, and an on-site fully licensed contracted supervisor. An ordinary solo or group office should not assume it qualifies. Psychology interns and postdoctoral fellows have separate rules. A cosignature does not authorize a supervisor-NPI substitution. Verify medical necessity, current authorization/CDC requirements, documentation, code, rendering identity, claim destination, and effective dates. For commercial insurance, confirm the exact network, products, tax ID, clinician/location affiliation, credentialing, contract, and welcome/effective-date evidence. BCBSOK’s onboarding process distinguishes solo and group applications and does not guarantee a contract. Keep CAQH and supporting documents current, and arrange EFT/ERA and claim workflows before advertising participation.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.

Where Bomi can help in Oklahoma

Bomi supports therapy practices in every state across the U.S. This guide organizes the Oklahoma-specific setup questions that sit around the national NPI, CAQH, billing, and credentialing workflow.
