Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

Clinical authority, ownership, and payment are separate decisions

An entity filing, EIN, or NPI does not grant permission to practice or guarantee payer participation. The table addresses professional readiness; confirm the actual license, service, ownership, supervision, and payer requirements before accepting clients.

LPC is a full license; LPCC is the clinical specialty

North Dakota’s LPC is not an associate license. Its LPCC credential has additional clinical requirements and an expressly defined clinical counseling scope. Do not equate the two or infer clinical authority solely from a payer’s credential list.

Supervision does not create private-practice or billing permission

LAPC qualifying experience cannot be earned in a self-owned practice. Private social-work practice is reserved to LCSWs, including qualifying partner/shareholder arrangements. For MFT interns, provisional licenses, residents, and trainees, the actual authorization and approved setting control. A supervisor’s signature does not establish permission to use that supervisor’s NPI.

Addiction practice needs more than an individual license

The current private-practice registration rule includes LCACs, licensed master addiction counselors, and LACs registered as clinical supervisors. Obtain the separate board registration and address HHS treatment-program licensing. Older board summaries can omit the LAC route added in October 2025.

LPCC — Licensed Professional Clinical CounselorThe clinical counseling specialty, with scope expressly including diagnosis and treatment of emotional and mental disorders.Requires the underlying LPC qualification plus additional clinical training, experience, and examination.Confirm active licenses and any restrictions before setting the practice’s services.Organize the approved clinical credential and payer application records.LPCC clinical specialty and scope
LPC — Licensed Professional CounselorA full professional counselor license, distinct from both LAPC and the LPCC clinical specialty.Stay within authorized scope and competence; resolve any proposed diagnostic or clinical service that is unclear with the board.Do not advertise LPCC status or assume an insurer’s LPC enrollment grants the clinical specialty.Match applications to the actual credential and covered services.Counselor licensure and supervised pathways
LAPC — Licensed Associate Professional CounselorA supervised pathway, not an independent clinical-practice credential. The qualifying supervised experience for LPC licensure cannot be earned in a practice in which the applicant holds an ownership interest.Hiring a supervisor does not remove the qualifying-experience ownership restriction.Confirm the permitted employment and payer arrangement before enrollment work.LPC and LAPC supervision and ownership rules
LCSW — Licensed Clinical Social WorkerThe North Dakota credential authorized for private practice of social work.Maintain clinical competence and any individual license conditions. The private-practice definition also covers qualifying fee-based work in a private agency where the social worker is a partner or shareholder.Support individual and group enrollment after the legal structure is settled.Social-work private-practice definition
LMSW — Licensed Master Social WorkerDoes not authorize private practice of social work; LCSW-reserved clinical activities require LCSW supervision.Use the required clinical supervision arrangement and distinguish it from independent practice.Medicaid’s recognition of LMSWs does not override the LCSW-only private-practice rule.Verify service-specific enrollment, supervision, and rendering-provider rules.Social-work scope, private practice, and electronic services
LBSW — Licensed Baccalaureate Social WorkerA generalist social-work credential, not independent clinical psychotherapy or private-practice authority.Work within the statutory scope and applicable setting requirements.Do not build an independent clinical practice around a baccalaureate license.Check the specific benefit and provider type rather than assuming psychotherapy eligibility.Social-work scope, private practice, and electronic services
LMFT — Licensed Marriage and Family TherapistThe full MFT license supports independent practice within the profession’s scope, including diagnosis and treatment in its marriage and family systems framework.Use the actual active license and observe any restrictions.Business formation and payer participation remain separate from professional licensure.Prepare individual and practice enrollment records for eligible services.Marriage and family therapy licensure and scope
LAMFT — Licensed Associate Marriage and Family TherapistA postgraduate supervised pathway; it is distinct from the full LMFT license.Complete the required supervised work experience under the approved arrangement.Do not represent associate status as unrestricted independent LMFT authority.Resolve associate-service coverage and required claim identifiers with each payer.MFT associate, provisional, and training definitions
Licensed psychologistIndependent psychology practice within competence and license conditions.A psychology license is separate from supervised resident registration.Industrial-organizational psychology authority or a school exemption is not general outpatient psychotherapy authority.Track the clinical license, practice location, and payer participation separately.Psychologist and resident statutory scope
Registered psychology resident; provisional or limited psychology authorizationResident practice is supervised. Provisional or limited permissions depend on the actual board-issued authorization and expiration.Residents must meet registration and supervision requirements and identify their status and supervisor.A pending application or temporary permission is not an unrestricted ongoing license.Confirm the exact authorization and payer treatment before scheduling insured care.Psychology resident and provisional licensing rules
LMAC — Licensed Master Addiction Counselor; existing LCAC — Licensed Clinical Addiction CounselorEligible credential categories for additional board private-practice registration; the license alone is insufficient.Remain within addiction counseling scope and obtain the separate registration.Resolve HHS treatment-program licensing before operating an addiction treatment practice.Organize enrollment after individual, private-practice, and program approvals are clear.Current addiction private-practice eligibility, amended October 2025
LAC — Licensed Addiction CounselorAn LAC registered as a clinical supervisor may apply for private-practice registration under the rule amended October 1, 2025; an LAC alone is insufficient.Do not rely on an older summary that omits this route, or treat eligibility to apply as approval.Confirm both board registration and applicable treatment-program licensing.Verify the exact enrollment category and services, without assuming independent payer eligibility.Current addiction private-practice eligibility, amended October 2025
Students, counseling/MFT interns, and addiction trainees or internsEducational and training exceptions do not authorize an independent therapy practice.Remain in the qualifying board-approved or educational arrangement; addiction internships involve supervised work in a licensed treatment facility.A degree, exam result, or informal supervision agreement does not create a full license.Resolve legal authority and the payer’s training-service rules before billing.Addiction counselor licensure and private-practice registration

National steps

These steps are not unique to North Dakota, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Decide Type 1 NPI versus Type 2 NPI and whether the practice will bill under an individual or organization.
  • Create or update CAQH and keep attestations current.
  • Align EIN, W-9, business bank account, and payer-facing business identity.
  • Confirm HIPAA privacy/security, Notice of Privacy Practices, consent forms, Good Faith Estimate workflows, and documentation setup.
  • Select EHR, clearinghouse, billing workflow, payment processor, ERA/EFT workflow, and denial-management process.
  • Build a credentialing packet with license, malpractice, NPI, CAQH, W-9, bank documentation, taxonomy, service address, telehealth policies, and supervisor information if applicable.
  • Track payer applications, effective dates, portals, claim test dates, ERA/EFT status, and recredentialing deadlines.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Choose the structure with the licensing board and a North Dakota attorney before filing, especially for a mixed-profession group or nonclinician ownership proposal. Sole proprietorship, professional corporation, and professional LLC options have different ownership and governance implications. Chapter 10-31 limits professional organizations and combinations of professions; an ordinary LLC filing does not settle those restrictions. A professional LLC formation requires licensing-board certification. The Secretary of State also identifies a restriction on simultaneously owning or serving as an executive of multiple professional organizations rendering the same professional service, which matters when retaining a solo entity while joining a group as an owner. Register the public name through FirstStop as required: a licensed sole proprietor’s trade-name test considers first and last name, while an organization using a different name may need a trade-name filing. Maintain entity reports and board certifications; the SOS states that the professional-license certificate must accompany PLLC annual reports starting January 1, 2027. Check local office or home-practice requirements and keep legal name, registered trade name, W-9, NPI, bank, EHR, and payer records aligned.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Complete state entity formation before applying for its EIN where an entity is being formed; the IRS issues EINs directly without a fee. Keep the IRS legal-name and tax-ID documentation used for banking and enrollment. Choose tax treatment separately from entity type, and plan federal income, self-employment, and estimated taxes with a tax professional. North Dakota income-tax filings and other accounts depend on structure, activities, and workforce. Register for employer withholding through ND TAP when applicable; review sales/use-tax obligations for any taxable sales or purchases rather than assuming every practice needs the same permit. Hiring also requires unemployment-insurance and workers’ compensation review. Job Service directs employers to register within 20 days after first employing workers; check WSI coverage requirements and applicable exclusions before payroll begins.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

Confirm the client’s physical location at each session and the clinician’s authority there. North Dakota treats services to a person in the state as practice in the state regardless of the clinician’s location. Chapter 43-51 contains a narrow remote-continuation exception for an existing relationship formed in the practitioner’s licensing jurisdiction; confirm its profession-specific application before relying on it, rather than using it to recruit new North Dakota clients. Electronic social-work practice can also remain subject to North Dakota law when the practitioner is in North Dakota and the client is elsewhere. Compact membership alone grants no permission: as of this review, North Dakota appears on the Counseling Compact member list but is not among the commission’s operational privilege states. Check current availability and an actually issued privilege. Psychologists using PSYPACT must verify the required authority to practice interjurisdictional telepsychology and home/receiving-state location conditions; an E.Passport alone is insufficient. Document telehealth consent, privacy limitations, the client’s location, emergency contacts, and a connection-failure plan according to the profession and payer. Use appropriate HIPAA safeguards and vendor arrangements; assess Part 2 requirements for covered SUD records. Medicaid’s telehealth policy separately requires enrolled billing and servicing providers, medical necessity, appropriate secure technology, and service-specific coverage. Verify current codes, modifiers, place of service, and audio-only conditions rather than assuming video and telephone are interchangeable.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Build separate child and vulnerable-adult reporting procedures into intake and staff training. Covered mental health professionals and social workers must immediately report qualifying knowledge or reasonable suspicion of child abuse or neglect, including a resulting death, to HHS or its authorized agent. An oral report needs a written follow-up within 48 hours only if requested; this is not permission to delay the first report. HHS lists the statewide Child Abuse & Neglect Reporting Line as 1-833-958-3500, weekdays 8 a.m.–5 p.m. Central Time, and directs callers to 911 for immediate danger. Plan for after-hours reporting; intake office hours do not extend the legal deadline. The designated-agent option for qualifying larger employers does not permit approval barriers or restraint of a staff report. Vulnerable-adult duties concern substantial mental or functional impairment, not age alone. Required oral or written reports of qualifying abuse, neglect, or circumstances reasonably leading to them must be made as soon as possible. HHS accepts online reports, SFN 1607, or calls to 1-855-462-5465, option 2; adult intake is staffed weekdays 8 a.m.–5 p.m. Central Time with after-hours messages. APS is not an emergency service: contact law enforcement first for immediate danger of serious injury. Explain confidentiality limits and apply any additional protections for SUD records when disclosing information.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

Start by checking for an existing individual or group Medicaid record; do not create a duplicate enrollment. North Dakota uses the Health Enterprise MMIS application and a provider-type supporting packet processed by Noridian. Establish individual and group enrollments, clinician affiliations, taxonomy, legal/tax identity, and effective dates as applicable. The January 2026 Other Licensed Providers policy includes psychologists, LCSWs, LMSWs, LPCCs, LPCs, LMFTs, and LACs, but recognition never expands professional scope or removes private-practice restrictions. The current October 2026 enrollment policy requires eligible servicing and billing NPIs to be enrolled on the date of service. It permits supervisor-NPI billing only in specified circumstances; eligible providers required to enroll must use their own NPI. Behavioral-health rehabilitation has its own practitioner qualifications, service table, employing-entity requirements, and no-supervisor-NPI substitution rule for enrolled rehabilitation practitioners. An LAPC rehabilitation pathway is not general independent psychotherapy authorization. Distinguish outpatient care, rehabilitation, SUD treatment, and 1915(i) benefits; verify member eligibility, covered service, authorization, documentation, and rendering-provider rules. Traditional state-administered fee-for-service Medicaid and BCBSND-administered Medicaid Expansion use different processes. Expansion participation requires both state enrollment and BCBSND participation; an existing commercial provider needs the Expansion addendum. Route each claim from verified current benefits and administrator, not a generic Medicaid label. The June 2026 moratorium lists new NEMT agencies statewide and QSP/DD agencies in Burleigh and Cass counties; it is not a blanket therapist enrollment freeze. For commercial plans, confirm the specific network and behavioral-health administrator, practitioner credentialing, contracting, group/location affiliation, fee schedule, and effective date. BCBSND uses CAQH authorization and supporting documents for practitioner applications. A complete CAQH profile or application is not an active contract. For supervised clinicians, obtain the payer’s actual service, credential, and claim-identifier requirements before building an insured caseload.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.

Where Bomi can help in North Dakota

Bomi supports therapy practices in every state across the U.S. This guide organizes the North Dakota-specific setup questions that sit around the national NPI, CAQH, billing, and credentialing workflow.

Once clinical authority, supervision, and the business arrangement are established, Bomi can help organize provider records and move eligible payer applications and billing workflows forward. Bomi does not decide professional scope, approve ownership or supervision, provide legal or tax advice, or guarantee a network contract, rate, effective date, or payment.

  • Align clinician and group NPI, CAQH, W-9, practice location, and payer enrollment information.
  • Track state enrollment, plan participation, affiliations, and effective-date evidence separately.
  • Prepare eligibility, claims, remittance, denial, and balance workflows for approved services.
Next step: talk through credentialing and billingBring your North Dakota setup stage, payer goals, and current NPI / CAQH status. Bomi can help turn the setup into a working revenue pipeline.

Bomi can help with CAQH, payer applications, claims, denials, and revenue workflows

Use the state guide to confirm the setup requirements, then bring in Bomi when you are ready for credentialing and insurance billing operations.

Credentialing is available only to practices enrolled in Bomi Billing, including practices using Bomi EHR. We do not offer credentialing on its own.
