Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

Read the underlying license and certificate together

Nebraska’s counseling, MFT and social-work titles do not map directly to every other state’s license names. An LMHP with the corresponding CPC, CMFT or CMSW certificate may use the licensed professional counselor, licensed marriage and family therapist or licensed clinical social worker title. The LIMHP combinations support the corresponding licensed independent titles. Verify the actual license and certificate rather than treating “LCSW,” “LPC” or “LMFT” as proof of LIMHP authority. Neither LMHP nor LIMHP authorizes prescribing or practicing psychology.

Build consultation and supervision into the practice

LMHP consultation is a defined professional collaboration: the consultant makes the diagnosis, and the professionals develop a treatment plan assigning responsibilities. The permitted consultant depends on the service; psychotherapy addressing organic-illness consequences calls for a qualified physician or psychologist. For provisional practice, check the supervisor’s qualifications for the particular license or certificate sought. Title 172, Chapter 94 requires at least one hour of face-to-face supervision weekly and permits qualifying secure video supervision. An LMHP pursuing LIMHP must continue required supervision of major-disorder client contact until the LIMHP is issued.

Prepare consent, records and emergency procedures

Before taking clients, establish informed consent, documentation, release-of-information, emergency coverage and practice-closure procedures. Nebraska’s mental-health rules require signed, dated informed consent in the client file. Review profession-specific recordkeeping and retention requirements, privacy safeguards and any longer payer obligations. For a group, maintain a current record of every clinician’s license, certificate, approved role, supervisor when needed and payer participation.

Keep the art-therapy transition dates explicit

As of October 24, 2026, DHHS announces art-therapy application updates for November 2, 2026 and a credentialing transition by January 1, 2027. Review the current CAT/PCAT transition instructions before advertising an art-therapy credential. Do not treat future application changes as a credential already issued to the clinician.

Licensed Independent Mental Health Practitioner (LIMHP)Independent mental-health practice within scope and competence, including major-disorder diagnosis and treatment.Full LIMHP status does not carry the LMHP consultation restriction. Confirm any individual license conditions.Clinical authority does not settle professional ownership or entity requirements.Match the active license and professional certificate to the payer category.Independent mental-health practice, §38-2113
Licensed Mental Health Practitioner (LMHP)A full license with statutory consultation limits; it is not a provisional license.Major-disorder diagnosis, specified psychotherapy and organic-illness work require the consultation described in §38-2115.A solo business does not remove consultation duties.Check the payer’s accepted category and enrollment arrangement.Mental-health practice limitations, §38-2115
Provisionally Licensed Mental Health Practitioner (PLMHP)Supervised postgraduate practice; no autonomous clinical authority.Use a qualified supervisor for the credential sought. Disclose provisional status and supervision to clients.Business registration does not replace supervision or upgrade the license.Resolve provisional-provider eligibility before assigning insured visits.Mental-health licenses and provisional practice
Certified Professional Counselor (CPC)The certificate protects the professional title; mental-health authority follows the accompanying practice license.CPC alone does not authorize mental-health practice or remove LMHP consultation limits.Review the license and certificate together when selecting the public name.Record both CPC and LMHP/LIMHP accurately.Professional certifications and practice licenses
Certified Marriage and Family Therapist (CMFT)Certification alone does not authorize mental-health practice.MFT certification has its own experience and supervisor qualifications. The accompanying license controls mental-health scope. The MFT title does not establish an approved ownership structure.Verify the actual license and certificate combination.Marriage and family therapy certification
Certified Master Social Worker (CMSW)Social work within §38-2119 may be practiced without an LMHP; certification alone does not authorize clinical mental-health practice.Psychotherapy and other mental-health services require separate qualifying authority.Nonclinical social-work authority is distinct from clinical independence.Do not treat CMSW alone as an independent psychotherapy credential.Social-work practice and exclusions, §38-2119
Certified Social Worker (CSW)No private, independent or autonomous social-work practice; no mental-health authority from this certificate alone.Stay within the applicable social-work role and credential limits. An LLC does not expand the certificate’s scope.Distinguish CSW from clinical social-worker enrollment categories.Social-work credential definitions
Provisionally Certified Master Social Worker (PCMSW)Supervised social-work certification pathway.Confirm the supervisor and experience requirements. Clinical mental-health work needs the appropriate additional license.Provisional certification does not authorize unsupervised psychotherapy.Track PCMSW and PLMHP separately when both apply.Provisional master social-work certification
Licensed psychologistIndependent psychology practice within competence and issued-license conditions.A degree or pending application does not establish an active license.Review professional services and ownership separately.Confirm the exact psychology credential and payer category.Psychology licensing
Provisionally licensed psychologist or psychological assistantProvisional psychology is supervised postdoctoral practice. An assistant role does not confer independent psychology authority.Psychological assistants perform authorized activities under a licensed psychologist; provisional practice has its own supervision requirements.Neither role becomes independent through a practice affiliation or business filing.Verify the particular supervised role, service and payer rules.Supervised psychology credentials
Temporary or special psychology authorizationOnly the activities and duration allowed by the actual issued authorization. The 30-day authorization for qualifying out-of-state psychologists is separate from other temporary or special licenses.Obtain approval before relying on a limited permission for a Nebraska practice.Check expiration, scope restrictions and payer acceptance.Psychology temporary and special permissions
Licensed Alcohol and Drug Counselor (LADC)Alcohol-and-drug counseling within the specialty’s authorized functions; not a general mental-health license.Assess whether the planned services require an additional mental-health credential or referral.Program or facility requirements must be considered separately.Confirm the addiction-service enrollment category and setting.Alcohol and drug counselor licensing
Provisionally Licensed Alcohol and Drug Counselor (PLADC)Supervised addiction-counseling pathway.Follow addiction-specific supervision requirements while completing qualifying experience. An entity filing does not create unrestricted therapy authority.Verify provisional addiction-provider participation and claim requirements.Provisional alcohol and drug counseling
Next step: confirm your state board rulesUse the official Nebraska licensing source before you decide whether to setup independently, stay supervised, or join a group first.

National steps

These steps are not unique to Nebraska, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Decide Type 1 NPI versus Type 2 NPI and whether the practice will bill under an individual or organization.
  • Create or update CAQH and keep attestations current.
  • Align EIN, W-9, business bank account, and payer-facing business identity.
  • Confirm HIPAA privacy/security, Notice of Privacy Practices, consent forms, Good Faith Estimate workflows, and documentation setup.
  • Select EHR, clearinghouse, billing workflow, payment processor, ERA/EFT workflow, and denial-management process.
  • Build a credentialing packet with license, malpractice, NPI, CAQH, W-9, bank documentation, taxonomy, service address, telehealth policies, and supervisor information if applicable.
  • Track payer applications, effective dates, portals, claim test dates, ERA/EFT status, and recredentialing deadlines.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Choose the proposed services, owners and clinical decision-makers before selecting a sole proprietorship, LLC or corporation. Use the Secretary of State’s formation or foreign-registration process for the actual entity. Professional-service LLC rules require appropriately licensed people to render the professional services and a professional registration or electronic license-verification process before providing them. Review the annual professional-registration or verification update separately from ordinary entity reports. Have counsel and the relevant regulator assess the proposed ownership, management and mixed-profession arrangement; an accepted business filing does not approve every clinical structure. Nebraska LLC organization notices require publication for three successive weeks and filing proof with the Secretary of State. If registering a trade name, its separate process calls for one publication and proof filed within 45 days of registration. Do not apply that trade-name deadline to every LLC publication filing. Check names before ordering signage or payer materials; registration does not resolve all trademark rights. LLC biennial reports are due April 1 in odd-numbered years, while business and professional corporations report by March 1 in even-numbered years. Calendar the requirements for the selected entity and keep its registered agent current. Check local zoning, occupancy, signage and home-office requirements. Added residential or structured treatment programs need a separate facility and program review. Keep the legal name, public name, EIN, banking, W-9, NPIs and payer locations consistent.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Nebraska Department of Revenue registration depends on the practice’s activities. DOR requires registration for businesses with employees and for specified taxable activities, while a business with no employees that only provides a service may not need a DOR account. Use the online registration process or Form 20 for the accounts that actually apply, including employer withholding. Review income or entity taxes, purchases subject to use tax, retail sales and separately taxable services with a Nebraska tax professional. Do not assume every therapy practice needs a sales-tax permit or that all revenue from a therapy business is exempt. Before hiring, address unemployment insurance, payroll withholding, new-hire reporting and applicable workers’ compensation requirements. Review employee-versus-contractor classification from the actual working relationship. A contractor label does not resolve employment obligations or professional supervision requirements.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

Confirm the client’s and clinician’s physical locations and the professional authority required in each jurisdiction. Telehealth does not expand an LMHP’s scope or remove provisional supervision. Use secure technology, verify identity and location, and establish local emergency contacts and a connection-failure plan. The Nebraska Telehealth Act’s consultation provisions require written information about consent, confidentiality, access to records and the right to refuse telehealth without losing care. Medicaid’s current Title 471, Chapter 47 adds operational requirements: after initial verbal consent, obtain the signed statement within ten days and retain it in the record. The statute permits verbal consent, but practices should not ignore the additional published Medicaid rule; obtaining signed consent before treatment avoids relying on an incomplete follow-up process. For children’s behavioral telehealth, the Medicaid rule requires an appropriately trained person immediately available in person unless the parent or guardian waives that requirement. Document the waiver, and address a threat of harm with the required safety plan before the initial service. Medicaid audio-only coverage is limited to qualifying individual behavioral-health or crisis services for an established client when clinically appropriate; it is not blanket coverage for every remote encounter. Confirm eligible codes, documentation, payer authorization and billing instructions. As of October 24, 2026, the Counseling Compact operational list marks Nebraska “No,” and the Social Work Compact is not issuing multistate licenses. Nebraska participates in PSYPACT, but eligible psychologists still need individual authority: E.Passport and APIT for telepsychology, or IPC and TAP for temporary in-person practice. Check current profession-specific permissions before accepting an out-of-state client; compact legislation alone does not authorize a session.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Nebraska’s child-reporting duty applies to any person with reasonable cause to believe a child has experienced abuse or neglect or is exposed to circumstances reasonably resulting in it. Report promptly to appropriate law enforcement or DHHS rather than waiting for proof or an internal investigation. The Child and Adult Abuse and Neglect Hotline is 800-652-1999, available day and night; DHHS also provides an official online reporting option. Under §28-711, an oral telephone report must be followed by a written report. Follow the receiving agency’s process; the statute does not provide a 24- or 48-hour initial-report grace period. Psychologists and other mental-health professionals are also mandatory reporters of suspected abuse, neglect or exploitation of a vulnerable adult under §28-372. Adult Protective Services covers qualifying adults age 18 or older with substantial functional or mental impairment or qualifying guardianship; older age alone does not establish the category. Use the same hotline or appropriate law enforcement. If DHHS requests written follow-up after an adult telephone report, it is due within 48 hours. That follow-up period is not permission to delay the initial report. Call 911 for immediate danger. Document the concern, channel, timing and follow-up while limiting disclosure to what the reporting process requires.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

For Nebraska Medicaid, use Maximus’ Provider Data Management System (PDMS). DHHS stopped accepting paper enrollment applications on June 1, 2025; older payer instructions mentioning paper forms should not override current state directions. The Medicaid Provider Manual distinguishes individual/solo enrollment from group-member enrollment: LIMHPs and licensed psychologists appear among solo types, while LMHPs, LADCs and several provisional or supervised credentials appear as group members. Confirm the exact clinician category, practice location and group affiliation before submitting claims. Group billing uses the organization’s Type 2 NPI with the actual rendering clinician’s Type 1 NPI as required; a supervisor’s signature or identifier does not establish payment eligibility. Medicaid’s adult psychiatric and children’s behavioral-health chapters were amended June 28, 2026. Apply the current age- and service-specific supervision, treatment-plan and provider-agreement rules rather than assuming an old summary applies to every client. Heritage Health’s plans are Nebraska Total Care, Molina Healthcare of Nebraska and UnitedHealthcare Community Plan of Nebraska. State enrollment and participation with each applicable managed-care plan are separate steps; shared credential verification does not create a single contract across all plans. For commercial insurance, distinguish credential review, contracting, clinician-to-group affiliation, approved locations, network products and written effective dates. Blue Cross and Blue Shield of Nebraska uses CAQH and tells applicants to wait for its acceptance letter before providing member services or submitting claims. It also has a specific provisional-provider process, so do not assume all provisional clinicians are excluded or can bill automatically through a supervisor. Confirm how existing participation applies to a new practice or tax ID. Verify patient eligibility, benefits, authorization and telehealth requirements for the service date. An NPI, CAQH profile, submitted application or old employer’s contract does not establish that the new practice is in network.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.
