Start with license readiness

Your license type determines whether you can practice independently, need supervision, or should set up inside a group practice first. Confirm the current rules with the state board before accepting clients.

This table is a setup readiness starting point, not a substitute for current board review.

Use the current 2026 counseling rules

The Allied Mental Health Board announced the LSMHC application and revised LMHC/LMFT rules on July 31, 2026. The current regulation PDFs include later October 2026 updates. They define qualifying clinical field experience sites by their programs, regulatory status and supervision arrangements; older advice categorically excluding private practices should not replace these rules. A proposed placement still needs to satisfy every applicable requirement. Applicants with earlier filings should check the Board’s transition policy for the version governing their application.

Clinical authority, ownership and billing are separate decisions

Identify the exact services, clinician credentials, supervisor and employment model before signing a lease or payer application. An LCSW employee arrangement, an LSMHC training placement and an independently licensed owner are different models. The table focuses on psychotherapy and alcohol/drug counseling; rehabilitation counseling, educational psychology, behavior analysis and prescribing professions have their own scopes. For CAC, CADC or CADC-II certification, ask BSAS which license or exemption applies before planning independent services; certification and LADC licensure are distinct. A business filing, NPI, supervisor signature or payer credentialing decision does not expand a clinician’s legal authority.

CredentialIndependent practice?Supervision issue?Entity/business noteBomi relevanceOfficial source
Licensed Mental Health Counselor (LMHC)The full counseling license permits mental health counseling within professional scope and competence. Confirm active status and any individual restrictions.Post-degree training requirements must be completed for full licensure. Supervising someone else requires the qualifications and arrangements in the current rules.Review professional-entity and clinic-exemption requirements separately from the individual license.Payer setup can proceed once the practice model and enrollment eligibility are established.262 CMR 2: Mental health counseling
Licensed Supervised Mental Health Counselor (LSMHC)A supervised license, not the full LMHC credential. Massachusetts opened this application route in July 2026.Practice under a qualifying supervisor and satisfy the applicable site, experience and supervision rules. A pending application is not an issued license.Do not assume that forming a business permits unsupervised care or makes a proposed training site acceptable.Any billing path needs separate confirmation for this credential, employer and payer.Supervised counselor authority, §165
Licensed Marriage and Family Therapist (LMFT)The full license permits marriage and family therapy within its scope, including nonmedical psychotherapy. A graduate or applicant has not yet earned this status.Prelicensure experience must meet current approved-supervisor and clinical-field-site requirements. Do not assume another state’s associate title applies here.Confirm the ownership, public name and facility status of a solo or group practice.Confirm the individual and group participation requirements for each target plan.262 CMR 3: Marriage and family therapy
Licensed Independent Clinical Social Worker (LICSW)May provide clinical social work autonomously within scope. This is Massachusetts’ independent clinical social-work credential.Use consultation when appropriate and meet applicable requirements when supervising others.Independent clinical authority does not resolve professional-entity or clinic-licensure questions. An eligible credential for the MassHealth independent behavioral-health pathway.258 CMR 12: Social-work scope
Licensed Certified Social Worker (LCSW)Cannot independently provide clinical social work. In Massachusetts, LCSW means Licensed Certified Social Worker.Clinical services must be provided as an employee of an agency, health care facility or unrelated individual under qualifying LICSW supervision. LCSWs cannot clinically supervise other mental-health practitioners. An independent-contractor label or privately hired supervisor does not replace the employee requirement.Resolve the lawful employment and payer billing arrangement before submitting applications.258 CMR 12.02: LCSW scope
Licensed Social Worker (LSW) / Licensed Social Work Associate (LSWA)These credentials have limited nonclinical roles; neither authorizes an independent psychotherapy practice.Work within the applicable supervised employee role and the functions permitted for the specific license level.Business registration cannot expand clinical scope.Do not treat these credentials as independent therapy enrollment categories.258 CMR 12.03–12.04: LSW and LSWA
Psychologist with Health Service Provider (HSP) certificationIndependent psychotherapy and other psychological health services require both a psychology license and HSP certification.Confirm HSP status and competence for the services offered; a doctorate or license alone is insufficient for independent health services.Keep the license, HSP credential and professional practice identity aligned.Use the psychology enrollment pathway and the payer’s credential requirements.Psychology Board: licensure and HSP policy
Psychologist without HSP / psychology traineeA licensed psychologist without HSP may provide health services under an HSP psychologist while accruing qualifying experience. Unlicensed trainees have a separate, limited supervised training exemption.Use a qualified direct supervisor and an accurate training title. Confirm any temporary or restricted authorization’s actual terms.Training or temporary status is not a general solo-practice route.Supervision and claim eligibility must each be verified for the setting.Psychology Board: supervised practice and titles
Temporary psychologist licenseAuthority depends on the temporary license actually issued and its conditions; an application or out-of-state license alone is insufficient. The Board may issue a temporary license for no more than one year. Confirm the permitted services and any HSP or supervision requirements.Temporary authorization does not settle ownership or facility requirements.Confirm that the payer accepts the issued credential for the relevant dates of service.Temporary psychology licensure, §124
Licensed Alcohol and Drug Counselor I (LADC I)Permits independent alcohol and drug counseling within that professional scope; it is not a general mental-health license. The credential may support supervision of other alcohol and drug counselors under the applicable rules.Check whether the proposed service model also requires substance-use-program or facility approval.Confirm the specific payer pathway; LADC I is not included in MassHealth’s LICSW/LMHC/LMFT definition of LIBHC.BSAS alcohol and drug counselor credentials
LADC II / Licensed Alcohol and Drug Counselor AssistantLADC II practice requires clinical supervision. The assistant role provides recovery-based services under direct clinical and administrative supervision.Neither credential is the LADC I independent-practice license.Use a setting and duties consistent with the actual license. A certification does not itself become a state license.Check the program and payer’s staffing and claim rules before offering covered services.BSAS supervised counselor credentials
Next step: confirm your state board rulesUse the official Massachusetts licensing source before you decide whether to setup independently, stay supervised, or join a group first.

National steps

These steps are not unique to Massachusetts, but they still need to match the state, address, entity, tax, payer, and EHR details you use for setup.

  • Decide Type 1 NPI versus Type 2 NPI and whether the practice will bill under an individual or organization.
  • Create or update CAQH and keep attestations current.
  • Align EIN, W-9, business bank account, and payer-facing business identity.
  • Confirm HIPAA privacy/security, Notice of Privacy Practices, consent forms, Good Faith Estimate workflows, and documentation setup.
  • Select EHR, clearinghouse, billing workflow, payment processor, ERA/EFT workflow, and denial-management process.
  • Build a credentialing packet with license, malpractice, NPI, CAQH, W-9, bank documentation, taxonomy, service address, telehealth policies, and supervisor information if applicable.
  • Track payer applications, effective dates, portals, claim test dates, ERA/EFT status, and recredentialing deadlines.
Next step: clean up NPI and CAQHMake sure your individual NPI, organizational NPI decision, taxonomy, CAQH profile, W-9, and practice address are consistent before payer applications start.

State-specific steps

Business entity options

Choose the business structure after checking the professional rules for every owner and service. For an LLC rendering professional services, 950 CMR 112.11 requires the filing to identify the services and relevant licensed members or managers, and to include certification from the applicable regulating board. Confirm the correct board process and any professional-corporation alternative before filing. Separately assess clinic licensure with the Department of Public Health. Under Chapter 111, §52, the solo/group-practice exclusion depends on the practice being wholly owned and controlled by its associated practitioners; names using “clinic,” “dispensary” or “institute” can change the analysis. Do not assume that employing a licensed therapist makes an investor-owned business exempt. If operating under a name different from the legal business name, file the applicable business certificate with the city or town clerk. That DBA certificate is not a professional or facility license. Check local office-use requirements and keep entity records, public name, EIN, bank account, W-9, NPI, practice locations and payer records consistent. Calendar entity annual reports, including an LLC’s anniversary-date filing, and reassess the structure when adding owners, locations or services.

Next step: align entity details with billing recordsBefore payer setup, make sure your entity name, EIN, W-9, NPI, CAQH, EHR, and bank details will all tell the same story.

State and tax registration

Register applicable tax accounts through the Department of Revenue’s MassTaxConnect system after deciding the business structure and activities. An EIN and state tax registration serve different purposes. Review income or corporate excise obligations, estimated payments and any sales/use obligations created by what the practice actually sells or purchases; do not assume every activity of a therapy business is tax-exempt. Hiring creates additional payroll planning, including withholding and Paid Family and Medical Leave where applicable. Check unemployment insurance, workers’ compensation and worker classification before engaging staff. Keep tax and payroll registrations consistent with the legal employer and confirm the details with a Massachusetts tax professional.

Next step: prepare EIN, banking, and payer paperworkUse the setup checklist to track financial setup before credentialing asks for tax and payment details.

Telehealth notes

For each session, establish where both the clinician and client are physically located and confirm authority in each relevant jurisdiction. The Massachusetts Psychology Board treats practice as occurring where the client is located and requires Massachusetts licensure or a valid statutory exemption for care to a client here. A Massachusetts license alone does not authorize treating someone in another state. Put telehealth consent, privacy, identity/location checks, emergency contacts, connection-failure procedures and appropriate documentation into the workflow; verify the payer’s covered modality and billing requirements separately. The revised counseling rules address teletherapy and supervision experience, so an expired pandemic-era hours policy is not the current starting point. As of October 21, 2026, Massachusetts is not on the Counseling Compact’s operational-state list or PSYPACT’s participating-state legislation list, and the Social Work Compact is not yet issuing multistate licenses. Recheck the relevant commission and boards before relying on a compact, temporary exception or cross-state arrangement. Enacted legislation and an individual’s issued practice authorization are different things.

Next step: configure EHR and telehealth workflowsSet the client-facing workflow for scheduling, consent, documentation, payments, and billing before the first appointment.

Mandated reporting notes

Set up separate child, older-adult and disability-reporting procedures. For suspected child abuse or neglect, mandated reporters must immediately make the required oral report to the Department of Children and Families and submit a written report within 48 hours. Call the DCF area office serving the child during business hours; after hours, weekends and holidays use 800-792-5200. For an adult aged 60 or older living in the community, follow the Executive Office of Aging & Independence’s mandated-reporting process. Call 800-922-2275 immediately when required, then send the written form to the local Adult Protective Services agency within 48 hours. The agency permits its online mandated-reporting process for nonemergencies; a successfully submitted online report does not require a separate call or form. For adults aged 18–59 who meet Chapter 19C’s current disability and care-dependence definition, report suspected caregiver abuse or neglect to the Commission for the Protection of Persons with Disabilities (CPPD), formerly DPPC, at 800-426-9009. The June 2026 law changed the agency name and disability-definition wording; older consolidated pages may lag. Ask CPPD promptly about uncertain jurisdiction rather than relying on a diagnosis label alone. Chapter 19C, §10 requires an immediate oral report of a reportable condition and a written report within 48 hours, subject to the statute’s specific privilege provision. Death reporting has additional requirements; consult the current law and agency instructions for the situation. Facility-related abuse may require a different reporting route. Call 911 for immediate danger, document the report and follow-up, and do not assume notifying a supervisor fulfills the applicable reporting duty.

Next step: build forms and policy review into setupKeep clinical policies, privacy workflows, intake forms, and billing policies on the same setup checklist.

Insurance credentialing notes

Start with the member’s actual coverage and behavioral-health administrator. MassHealth’s licensed independent behavioral health clinician (LIBHC) category includes LICSWs, LMHCs and LMFTs. Psychologists have a separate provider pathway. The LIBHC rules cover services personally provided by an eligible clinician; they do not establish a blanket route to bill an LCSW, LSMHC or trainee’s work under a supervisor’s identity. LADC I and program-based services need their own eligibility review. LIBHC Bulletin 1 removed the Medicare-provider prerequisite for this MassHealth enrollment category effective October 1, 2025, while preserving other eligibility and third-party billing obligations. Older regulation text may still show that prerequisite. Use current MassHealth provider enrollment instructions, confirm approval and effective dates, and complete individual-to-group/location links where required. ORPASO-only and managed-care-only enrollment do not authorize direct fee-for-service billing to MassHealth. The corrected October 2026 All Provider Bulletin 421 announces additional enrollment-related claim enforcement for dates of service beginning January 1, 2027; existing billing and rendering-provider enrollment requirements already apply. Recheck coverage when a member changes plans: Bulletin 417 addresses 2026 transitions from managed care to fee-for-service, and Bulletin 422 describes January 2027 ACO changes. Confirm fee-for-service enrollment and any transferred authorization instead of assuming a managed-care contract follows the member. For PCC Plan and Primary Care ACO members, MBHP administers behavioral-health coverage. Other MassHealth managed-care arrangements can use different networks and administrators, so state enrollment alone does not establish plan participation. Commercial payers also distinguish credentialing from contracting: Blue Cross Blue Shield of Massachusetts requires both and uses CAQH for professional-provider credentialing. For a 2027 launch, note Point32Health’s October notice: Tufts Health Plan Commercial ends after December 31, 2026, while other Tufts products continue. US Family Health Plan administration moves to BrightonOne on January 1, 2027 and requires direct contracting for continued network participation. Confirm the exact product, clinician, group, location and participation date, plus any written supervised-provider policy. Before care, check eligibility, benefits and authorization requirements; before billing, check documentation, coding, payer routing and ERA/EFT setup. Clinical licensure, supervision, credentialing, contracting and medical necessity are distinct requirements. None alone guarantees reimbursement.

Next step: Map your payer listChoose target payers, gather packet details, and track every application through effective date and claim readiness.
