Does BCBS Wyoming Allow Provisional Therapist Supervisory Billing?

Does BCBS Wyoming permit provisional-provider billing?

Yes, its May 28, 2026 provider manual contains an affirmative exception for provisionally licensed providers and residents. Page 45 says BCBSWY does not credential them until fully licensed and directs claims under the supervising provider’s NPI until full licensure.

The exception matters: the preceding general incident-to paragraph restricts physicians submitting for other noncredentialed clinicians. Reading that restriction alone would miss the separate provisional-provider pathway. Member benefits and the provider agreement still govern whether the actual service is covered.

What about a PPC, PCSW or PMFT?

Wyoming’s licensing board identifies Provisional Professional Counselor, Provisional Clinical Social Worker and Provisional Marriage and Family Therapist pathways for relevant master’s graduates who need the examination and supervised experience. It separately lists Provisional Addictions Therapist.

Those are state credential identities, not a payer-issued approval list. BCBSWY’s provisional paragraph does not enumerate these therapy credentials, accepted supervisor disciplines or every service setting. The broad exception should therefore be applied only after the specific credential, lawful supervision and member benefit are established.

Should the provisional therapist use their own NPI?

The manual distinguishes provisional clinicians from NPPs who are already credentialed. Credentialed NPPs eligible for their own NPI must submit directly under their own NPI. For the provisional category, page 45 instead specifies the supervising provider’s NPI and says claims in the provisional provider’s own name are not recognized before credentialing is complete.

The paragraph does not supply a complete electronic billing, rendering and supervising-loop map. It also does not specify a supervision modifier. Do not invent a field allocation or modifier from another insurer’s instructions.

Does the neighboring Q6 rule apply?

No blanket Q6 instruction is given for provisional therapy. Q6 appears in the separate locum-tenens paragraph, which describes substitute-provider arrangements. Its proximity on page 45 does not make it a modifier for a provisional counselor or social worker. The same caution applies to importing Wyoming Medicaid’s HL rules into BCBSWY claims.

Are students and unlicensed employees included?

The exception names provisionally licensed providers and residents. It does not establish an ordinary student-practicum or generic unlicensed-employee pathway. A provisional license is distinct from simply having a degree, an application pending or a supervisor. The board’s application descriptions also distinguish examination-only routes that do not permit supervised practice while awaiting testing.

Which scope is still conditional?

This is the BCBSWY in-network provider-manual pathway, not a certification of every BlueCard home plan, Federal Employee Program benefit or WyoBlue Advantage product. The manual itself says the member’s benefit plan controls coverage. The license-specific and product-specific conditions remain necessary even though the general provisional exception is affirmative.

Reviewed October 30, 2026. The source cover says revised May 28, 2026; the June date in its filename alone does not establish when this particular exception began.

Frequently Asked Questions

Does BCBSWY prohibit all incident-to claims?

No. Page 45 states a general restriction but separately directs provisional-provider and resident claims under the supervising provider NPI.

Should a provisional therapist automatically use Q6 or HL?

No. The provisional paragraph gives no universal supervision modifier. Q6 is in the separate locum-tenens section, and Wyoming Medicaid instructions do not establish BCBSWY policy.
