Can QTTs and LPCs-in-Training Bill iCare Wisconsin Medicaid?

By George Ruan • October 30, 2026

iCare’s Wisconsin Medicaid guidance recognizes qualified treatment trainees (QTTs), so an LPC-in-training, LMFT-in-training or qualifying social worker does not automatically need independent licensure before delivering covered therapy. The pathway is conditional: the clinician must meet Wisconsin’s QTT qualifications, enroll correctly and satisfy iCare participation and service requirements. It is not a blanket instruction to bill every supervised clinician under a supervisor’s rendering NPI.

Which Wisconsin licenses and trainees fit the QTT pathway?

iCare’s 2024 provider bulletin describes ForwardHealth’s QTT clarification, including enrollment, allowable services, settings and claims reimbursement. The state’s detailed QTT guidance is Update 2024-22. Current ForwardHealth enrollment criteria identify LPCs-in-training, LMFTs-in-training, APSWs and ISWs working toward clinical licensure, plus qualifying unlicensed doctoral psychologists working toward psychologist licensure. A graduate degree and the applicable supervised-practice pathway are required; the credential alone does not establish plan participation.

A student who has not completed a graduate degree is not a QTT under this rule. Student services have separate requirements, and these sources do not establish an iCare student-billing exception. APSWs or ISWs who require supervision but are not seeking clinical-social-work licensure belong in the separate certified-psychotherapist enrollment category.

Whose NPI is rendering, and which modifiers apply?

ForwardHealth’s QTT billing rule distinguishes the billing provider from the person delivering therapy. An allowable Medicaid-enrolled agency, hospital, FQHC, billing group or independent professional bills for the service; the QTT’s own NPI is reported as rendering. Current QTT enrollment is rendering-only and requires an individual Type 1 NPI. A supervisor’s involvement is not a reason to replace the trainee’s identity with the supervisor’s rendering NPI.

Update 2024-22 identifies U6 for master’s-level QTTs and HP for doctoral-level QTTs. Apply the professional-level modifier and any other requirements for the specific covered service. The update permits psychotherapy and related mental-health services within scope and does not turn every service, setting or code into a payable claim. It also permits services through the supervising provider’s billing group; that billing-group relationship is different from naming the supervisor as the person who rendered therapy.

Supervision and iCare participation still matter

QTTs require supervision by Medicaid-enrolled, licensed providers and must comply with their profession’s licensing-board supervision rules. Medicaid reimbursement does not guarantee that the board will count the supervised hours toward licensure. Confirm the trainee’s enrollment, supervisor qualifications, group affiliation, covered service and any authorization requirements before starting billable care.

iCare’s July 2026 Provider Reference Manual covers both Medicare and Medicaid administration and requires providers to verify the member’s current coverage. The QTT pathway discussed here is Wisconsin Medicaid guidance, including the relevant BadgerCare Plus or Medicaid SSI benefit. It does not establish Medicare QTT reimbursement, nor does it resolve the additional authorization and enrollment requirements for Family Care Partnership or other long-term-care waiver services.

What evidence supports this answer?

Reviewed October 30, 2026. The payer-specific link is iCare’s own QTT bulletin; current state enrollment criteria and the July 2026 iCare manual supply the enrollment and plan-administration context. These documents do not guarantee a particular clinician’s contract status, fee or claim payment.
