Does Blue Cross Vermont Cover Supervised Therapy Trainees?
By Bomi Health • October 30, 2026
Yes, conditionally. Blue Cross and Blue Shield of Vermont’s CPP_37 policy covers medically necessary covered services by qualifying post-degree trainees working toward licensure. The supervisor must be enrolled, credentialed and in good standing with Blue Cross VT, remain involved in patient care and provide regular ongoing supervision. This is not a blanket benefit for every unlicensed therapist or student.

Which unlicensed clinicians qualify?
For the master’s-level route, the trainee must already have earned the degree, be completing supervised practice hours toward a license eligible for Blue Cross VT participation, and be on the roster of nonlicensed and noncertified psychotherapists—or its equivalent outside Vermont. A graduate working toward clinical mental health counseling, social work or marriage and family therapy licensure must satisfy those conditions; a job title alone does not establish eligibility.
The policy also identifies psychiatric APRNs completing hours toward psychiatric mental health nurse practitioner or psychiatric clinical nurse specialist licensure; psychology trainees completing supervised hours, with the roster requirement unless already licensed as a clinical mental health counselor, marriage and family therapist or social worker; and Apprentice Addiction Professionals (AAPs) or Alcohol and Drug Counselors (ADCs) working for a Vermont designated agency toward an eligible license. The designated-agency condition is specific to the AAP/ADC category.
Does this include students still earning their degree?
Not generally. CPP_37 says services delivered as part of an education or training program are not generally covered or eligible for reimbursement. Its central pathway is post-degree supervised practice. Do not treat that wording as an absolute prohibition on every educational arrangement, or assume that Vermont Medicaid’s separate student rules apply to Blue Cross VT.
Trainees must disclose their status to members. This policy does not cover someone who cannot practice independently but is not completing post-degree licensure hours, and it does not apply when the provider is eligible to submit claims directly to Blue Cross VT.
Who may supervise, and whose claim is billed?
The supervisor categories include a psychiatry-board-certified physician, psychiatric nurse practitioner, psychologist, clinical social worker, marriage and family therapist or clinical mental health counselor. The policy also recognizes specified equivalent out-of-jurisdiction professionals and a supervisor trained by a regional or national organization approved by the Vermont board. These categories do not waive profession-specific supervision law.
The supervisor bills the trainee’s covered services with modifier HO, except the initial evaluation, which is billed without HO. CPP_37 does not specify the exact CMS-1500 NPI boxes or electronic rendering loop; confirm those implementation fields through the plan’s claim instructions instead of importing another payer’s layout.
Which benefits and dates does this answer cover?
CPP_37 is effective August 1, 2024. The member’s benefit and provider contract still control payment. FEP benefits may differ. For Vermont-based services to out-of-state Blue members, the policy describes local-plan billing rules while the member’s plan determines benefit eligibility; that is not automatic coverage for every Blue product.
Frequently Asked Questions
Can a nonlicensed master’s graduate bill Blue Cross Vermont?
Qualifying post-degree trainees can have covered services billed by an enrolled, credentialed supervisor when the degree, roster, eligible-license and supervision requirements are met.
Is HO required on the initial evaluation?
No. CPP_37 directs the supervisor to use HO for trainee services except the initial evaluation, which is billed without HO.
Does the policy cover all student interns?
No blanket student permission is established. Services within education or training programs are not generally covered; the main pathway is post-degree supervised practice.
