Does South Dakota Medicaid Cover Supervised Therapy at CMHCs?

Does South Dakota Medicaid cover supervised therapists at a CMHC?

Yes, conditionally within an enrolled, accredited community mental health center (CMHC). The current CMHC manual requires clinical supervision for staff who do not meet its clinical-supervisor definition. It also sets separate qualifications for therapy and other services. This is an agency-specific pathway, not a general permission for a private practice to bill any unlicensed person under a supervisor.

Which qualifications apply to individual, group and family therapy?

The therapy row covers individual, group, family, and parent or guardian therapy. It lists “A master’s degree in psychology, social work, counseling, or nursing; a social work license.” Use that service-specific requirement with the applicable professional licensing rules. The manual does not translate that wording into a complete list of associate license abbreviations or establish that every unlicensed graduate qualifies.

Who can supervise?

A CMHC clinical supervisor must have at least a master’s degree in psychology, social work, counseling, or nursing, hold a current license in that field, and have two years of supervised postgraduate clinical experience in a mental health setting. Staff with an associate, bachelor’s or master’s degree who do not qualify as clinical supervisors must receive clinical supervision. A degree alone does not make someone the clinical supervisor.

Can student interns provide reimbursable psychotherapy?

This source does not establish a student-intern psychotherapy exception. Being supervised is not a substitute for the therapy row’s practitioner qualifications. Likewise, the manual permits different qualifications for collateral contacts, care coordination, recovery support and other services; those rows do not authorize the same staff to deliver psychotherapy. A student’s exact therapy eligibility remains unconfirmed here, rather than categorically prohibited.

Is the supervisor the rendering provider on the claim?

Do not infer that from the supervision requirement. The manual says eligible servicing and billing providers’ NPIs must be enrolled with South Dakota Medicaid. The passages reviewed do not establish a universal supervisor-as-rendering rule or an associate-specific modifier. Resolve the actual servicing-provider enrollment and claim instructions for the CMHC service before submitting a claim; a supervisor signature is not an NPI substitution instruction.

What documentation and coverage conditions matter?

The CMHC must be enrolled with Medicaid and accredited by the Division of Behavioral Health. Confirm recipient eligibility and the covered service. The manual’s initial treatment-plan requirements include completion within 30 days of intake, the responsible staff member’s signature, credentials and date, and the clinical supervisor’s signature and credentials when that staff member is not a clinical supervisor. Supervision does not waive the remaining service, medical-necessity or documentation requirements.

Does this answer apply to an independent therapy practice?

No conclusion about independent-practitioner supervisory billing follows from this CMHC chapter. A private practice, a school program and an accredited CMHC have different coverage contexts. The peer-support section also names particular supervisors, including counselors working toward a mental-health designation and social-work PIP candidates; that peer-service list is not a psychotherapy eligibility list.

Source and review date

Reviewed October 30, 2026 against the Community Mental Health Center Services manual linked from South Dakota Medicaid’s current billing-manual directory. Its running header reads “UPDATED May 25.” Pages 1–2 describe provider and service qualifications; page 10 covers initial treatment-plan signatures. The clinical-staff pathway is supported; a complete license-by-license enrollment and rendering map remains unresolved.

Frequently Asked Questions

Does South Dakota Medicaid allow an unlicensed therapist under supervision?

The CMHC manual supports supervised staff only when the person meets the specific service qualifications and applicable licensing requirements. It does not establish blanket eligibility for all unlicensed graduates or students.

Can an LPC or social-work PIP candidate supervise psychotherapy?

The CMHC clinical-supervisor definition requires the relevant master’s degree, current field license and two years of supervised postgraduate clinical experience. The peer-support supervisor list is a separate service rule and cannot by itself answer psychotherapy supervision.

Should a CMHC put the supervisor’s NPI on every therapy claim?

The reviewed manual requires enrolled eligible servicing and billing NPIs. It does not establish a universal supervisor-rendering instruction; resolve the exact service and enrollment instructions before filing.
