Does First Choice SC Cover Supervised LMSW and Intern Therapy?
By Bomi Health • October 30, 2026
Yes, conditionally. First Choice, the South Carolina Medicaid plan from Select Health, describes covered supervised psychotherapy for LMSWs and specified intern categories. Its October 2026 manual does not give every unlicensed therapist or graduate student permission to bill a supervisor.
Sections
LMSWs: an organized setting and an involved physician or NP
The manual’s LMSW pathway is for recognized organized settings, such as hospitals, schools and Department of Mental Health facilities. It requires direct physician or nurse-practitioner supervision within the same agency. The supervisor must authorize care, remain involved and review progress with the LMSW at least every 90 days. The employment or written-contract relationship, initial evaluation and covered physician/NP service conditions also apply.
Counselor and marriage-and-family interns: match the supervisor
The manual uses LPC/I and LMFT/I terminology and separately names applicants for LISW licensure. LPC/S supervisors supervise LPC/I counselor interns; LMFT/S supervisors supervise LMFT/I marriage-and-family interns; LISW-CP-S supervisors supervise LISW applicants. These are the manual’s supervisor designations, not permission based on an ordinary LPC, LMFT or LISW-CP license alone. Do not read the list as permission for every profession to supervise every other profession.
This pathway requires supervision at the same location, immediate accessibility, co-signed clinical notes and a written crisis protocol. Send the plan the supervised professionals’ names and credentials before services begin. The manual also limits supervisor caseloads; a license title alone does not complete these requirements.
A student placement is a different question
A graduate student without the relevant credential is not automatically an LPC/I, LMFT/I or LISW applicant. The manual separately describes psychologists supervising unlicensed people who provide psychological services. That does not establish a general student-counseling pathway under any therapist.
Which NPI and modifier go on the claim?
The cited supervision section establishes eligibility and clinical oversight, but does not supply a universal rendering-NPI substitution or modifier for these pathways. Do not copy another plan’s supervisor-NPI instructions. Match the approved setting, clinician and service to First Choice’s current claim instructions before submission.
School-based RBHS has its own provider and service rules. A school pathway should not be used to claim that the same arrangement is approved in an ordinary private practice.
Frequently Asked Questions
Does First Choice SC allow every unlicensed therapist?
No blanket permission is established. Its manual describes specific LMSW, intern and psychologist-supervised pathways with qualification and setting requirements.
Can I apply these rules to SelectHealth in Utah?
No. This article covers First Choice South Carolina Medicaid, not the similarly named insurer in Utah.
