Does Keystone First CHIP Allow Supervised Therapist Billing?

By Bomi Health • October 30, 2026

Yes, conditionally. Keystone First CHIP documents a supervised-staff pathway for qualified graduate-trained and named licensed practitioners working under a credentialed, enrolled psychologist. Its current attestation requires individual plan approval before the practitioner sees CHIP enrollees. A separate psychiatrist form has its own conditions. Neither pathway makes every student or unlicensed therapist eligible.

Which therapists does the policy name?

The July 2026 behavioral health supplement names LSW, LCSW, LPC, LMFT and unlicensed practitioners when a credentialed, MA- or CHIP-enrolled psychologist proposes to supervise staff. This is a notification and approval pathway, not automatic permission attached to a license. The graduate-psychology employment route requires qualifying education and compliance with the applicable professional rules.

What does unlicensed mean here?

The psychologist attestation identifies unlicensed master’s-level practitioners and requires graduate training in psychology for its unlicensed staff arrangement. The supplement explains that qualifying graduate coursework can include terminal master’s programs; doctoral-program enrollment is not required. Student status or an unrelated graduate degree alone does not establish eligibility.

Pennsylvania’s §41.58 has its own boundaries: it does not govern people completing psychology licensure experience, practitioners regulated by other professional boards, or specified exempt settings. Those categories need their applicable qualifications; they cannot all be treated as one unlicensed category. The payer’s awkward wording about preparation for licensure does not establish a separate blanket student benefit.

What must happen before the first visit?

The supervising psychologist must satisfy the plan’s credentialing and Pennsylvania enrollment requirements. Submit an individual attestation with a current resume and primary-source education documentation. The form requires verification of applicable professional qualifications, absence of Medicare/Medicaid sanctions, required training and ongoing supervision. Crucially, the person must not see Keystone First CHIP enrollees until the plan communicates approval. Submitting the form is not approval.

How many staff can a psychologist supervise?

The supplement and psychologist attestation impose a three-full-time-equivalent limit for this billing arrangement. For unlicensed employees governed by §41.58, the psychologist retains professional responsibility, provides regular face-to-face supervisory consultation and remains available for emergencies. The employee’s supervised status must be disclosed, including in billing. This is not independent practice by the employee.

Is psychiatrist supervision the same pathway?

The plan publishes a separate psychiatrist attestation naming LSW, LCSW, LPC, LMFT and unlicensed master’s-level practitioners, as well as CRNP and PA. It requires plan approval before visits, verified education and sanctions, appropriate training, documented ongoing supervision, active psychiatrist involvement and responsibility for treatment. Services must stay within the person’s competence and applicable professional scope. Do not transfer the psychologist’s three-FTE rule to this separate form or treat its physician-assistant provisions as a therapist staffing limit.

Whose billing identifier does the policy specify?

The supplement describes billing applicable supervised services through the licensed clinician’s MA identification number (MAID). The attestations use the supervisor’s Pennsylvania PROMISe Provider Identification number (PPID). These statements do not supply a complete electronic rendering-NPI and modifier specification. Confirm the service-specific claim requirements rather than substituting an NPI for PPID by assumption.

Which services and product does this cover?

The supplement lists individual, family and group counseling and master’s-prepared therapist visits among outpatient benefits. Covered-service, medical-necessity and authorization requirements still apply; for example, psychological testing and intensive outpatient treatment have separate authorization requirements. An approved staff arrangement is not authorization for every service.

This answer concerns Keystone First CHIP, with coverage by Vista Health Plan in southeastern Pennsylvania. PerformCare handles specified behavioral-health functions for this CHIP product. It does not extend to Keystone First Medicaid, Community HealthChoices or a different PerformCare program. Checked October 30, 2026, the current provider indexes link the July 2026 supplement and the two attestations. The review date is not a new policy effective date.

Frequently Asked Questions

Does Keystone First CHIP allow LSW, LPC or LMFT supervised therapy?

Those licenses, along with LCSW, are named in the plan’s supervised-staff process. Individual approval, applicable professional qualifications, the supervisor’s enrollment and credentialing, and covered-service requirements remain necessary.

Can an unlicensed master’s graduate see CHIP members?

There is a conditional pathway for qualified unlicensed staff. The psychologist route requires graduate training in psychology and applicable Chapter 41 compliance; the separate psychiatrist form has its own conditions. The plan must approve the individual before visits. Student status alone is insufficient.

Can visits begin while the attestation is pending?

No. Both current attestation forms require notification of Keystone First CHIP approval before the practitioner sees its enrollees.

Does the policy say to use the supervisor’s rendering NPI?

It specifies MAID or PPID billing for applicable supervised services. The reviewed passages do not provide a universal rendering-NPI and modifier mapping.
