Does Highmark Wholecare Medicaid allow LSW, LAPC or unlicensed therapy billing?
A Highmark Wholecare Medicaid card alone does not establish that permission. Its current 2026 provider manual assigns ordinary mental-health and substance-use treatment to the member’s county behavioral-health managed care organization (BH-MCO), subject to specific exceptions. An LSW, LAPC, other associate or student must satisfy the actual behavioral-health plan’s service, license, supervision and participation rules. This is a Pennsylvania Medicaid benefit distinction, not a statement that those clinicians are universally prohibited.
Which plan decides whether the supervised psychotherapy is payable?
Start with the county BH-MCO identified for the member. Wholecare’s manual lists these organizations and their contacts on pages 13–16 and directs providers to coordinate physical and behavioral care. The appropriate behavioral-health plan must establish whether the clinician’s exact credential and proposed psychotherapy service qualify. Neither Wholecare enrollment nor a supervisor’s independent license substitutes for that determination.
What are the exceptions to the behavioral-health carve-out?
Pages 70–71 retain specific Wholecare responsibilities, including emergency-room services; contracted opioid-use-disorder Centers of Excellence and Maternal Health Homes for SUD treatment; certain physical-health visits for SUD medication prescribing and mental-health/SUD screening; home health care; pharmacy; and emergency transportation. These exceptions do not turn ordinary office psychotherapy into a Wholecare benefit. Emergency-room evaluations for voluntary or involuntary psychiatric commitments are separately assigned to the BH-MCO. Check the exact service rather than treating every encounter involving a mental-health diagnosis the same way.
Can the practice simply use the supervisor’s NPI?
This Wholecare benefit-allocation passage does not provide an associate or student rendering-NPI rule, a supervisory modifier, or an approval to submit a supervisor-rendered psychotherapy claim. Obtain the applicable BH-MCO’s instructions for the service and clinician before deciding whose NPI belongs on the claim. Clinical supervision and payer claim identity are separate questions.
Does this answer apply to Medicare or every Highmark plan?
No. This answer is limited to Highmark Wholecare Pennsylvania Medicaid. It does not determine Medicare eligibility, a Wholecare Medicare product’s rules, Highmark commercial coverage or another state’s Health Options program. It also does not establish eligibility under any county BH-MCO merely because that organization appears in Wholecare’s contact list.

Reviewed October 30, 2026 against the current provider-manual resource, whose retained PDF is labeled 2026. Pages 13–16 identify county behavioral-health contacts; pages 70–71 explain the coverage split and exceptions. The manual supports this benefit answer, while the exact associate/student psychotherapy permission remains a question for the responsible behavioral-health plan.
